This paper is meant to review our knowledge of the roles of insulin resistance and insulin dynamics in the association between regional adiposity and its metabolic health risks. In addition I attempt to discuss potential mechanisms for the aberrations in insulin metabolism and encourage significant questions as to the nature of these defects, to consider their interrelationships with visceral obesity and its etiologic determinants, and finally to set the stage for future research in this area.
