Chronic otitis externa and media in cats and dogs have different etiology while final therapy remains very similar. Almost 80% of all chronic otitis externa in dogs turn into otitis media with time, while inflamammatory polyps and other tumors of middle ear, auditory tube and auricle are main reasons for chronic otitis externa and media in cats. It is relatively easy to make an induction for surgical management of chronic otitis in cats, in contrast, medical, antimicrobial, dermatological, antialergic, antiautoimmune and other treatments still are strongly positioned in veterinary opinion as possible solutions for chronic otitis. Length of ear disease, number of unsuccessful treatments and detection of middle ear involvement, most easily and accurately with CT, are firm basis to make an indication for total ear canal ablation and tympanic bulla osteotomy as final and permanent treatment of incurable ear disease
