Påvisning av antibiotikaresistente bakterier i melkeprøver fra norske kyr ved bruk av fenotypiske og genotypiske metoder | Detection of antibiotic resistant bacteria in milk samples from Norwegian cows using phenotypic and genotypic methods

Bakgrunn: Verden over er antimikrobiell resistens et økende helseproblem. Mangelen på overvåkning hos husdyr og matvarer er en begrensning på å forstå omfanget. Forekomst av antibiotikaresistente bakterier hos matproduserende dyr øker sannsynligheten for kontaminering av animalske næringsmidler, som igjen kan representere en smittekilde til mennesker og miljøet. Kartlegging av resistens i næringsmidler er begrenset, inkludert melk fra norske kyr.Hensikt: Formålet med denne masteroppgaven er å undersøke forekomsten av antimikrobiell resistens blant bakterieisolater i melkeprøver fra friske, og syke kyr med klinisk mastitt. I tillegg ble prøvene screenet for metall -og desinfeksjonsresistens.Materialer og metoder: Melkeprøver (n = 76) fra ti friske kyr uten tegn på klinisk mastitt ble undersøkt ved to ulike prøveuttak. Prøvene ble innhentet fra Ås gård ved Norges miljø og biovitenskapelige universitet (NMBU), og analysert på Oxoid BrillianceTM CRE, BrillianceTM ESBL og MRSAselectII agar. I tillegg ble prøver fra kyr med påvist mastitt (n = 5) og isolater fra friske kyr uten tegn til mastitt (n = 31) undersøkt. Bakterieisolatene ble deretter identifisert med 16S rRNA sangersekvensering. For screening av de mest utbredte ESBL, karbapenemer og MRSA-gener ble multipleks-PCR anvendt. Totalt tre prøver ble inkludert til helgenomsekvensering med Illumina MiSeq. Antibiotika sensitivitetstesting ble benyttet for fenotypisk resistensbestemmelse.Resultater: Av totalt 76 melkeprøver fra friske kyr vokste ingen bakterier på selektive skåler. Det ble påvist fenotypisk og genotypisk antibiotikaresistens i melkeprøver fra friske og kyr med påvist mastitt. Ved fenotypisk screening på selektive skåler, vokste 2 av 31 (6,5 %) melkeisolater fra friske kyr på Oxoid BrillianceTM CRE agar. Isolatene ble identifisert som Enterococcus faecalis. Bakteriestammen viste fenotypisk resistens mot cefotaksim, cefepim, amikacin, gentamicin og streptomycin. I tillegg ble det gjort funn av genet tetM som koder for tetracyklinresistens. Bakterierisolater som vokste på ESBL-skåler ble identifisert som Pseudomonas spp., og to av disse viste fenotypisk resistens mot cefotaksim og streptomycin. Hvorav det ene isolatet hadde mobile gener (aph(6)-Id, aph(3'')-Ib) som koder for resistens mot aminoglykosider, og AmpC for β-laktam antibiotika.Konklusjon: Hovedfunn i denne studien viser lav forekomst av antibiotikaresistens i melkeisolater fra friske melkekyr. Påvisning av resistente bakterieisolater illustrerer et behov for ytterligere forskning i dens rolle i spredning av antibiotikaresistens i den norske matkjeden.

إنجليزي. Background: Worldwide, antimicrobial resistance is a growing health problem. The lack of monitoring in livestock and food is limited to understanding the scope. The presence of antibiotic-resistant bacteria in food-producing animals increases the likelihood of contamination of animal food, which in turn can represent and contaminate humans and the environment. Research of antimicrobial resistance in the Norwegian food chain is limited, including milk from dairy cows.Purpose: The purpose of this master's thesis is to investigate the presence of antimicrobial resistance among bacterial isolates in milk samples from healthy cows, and also cows with mastitis disease. In addition, the samples were screened for metal and disinfectant resistance.Materials and methods: Milk samples (n = 76) from ten healthy cows with no evidence of clinical mastitis were examined. The samples were obtained from Ås-farm at the Norwegian University of Life Sciences (NMBU) and analyzed on Oxoid BrillianceTM CRE, BrillianceTM ESBL and MRSAselectII agar. In addition, bacteria from cows with proven mastitis (n = 5) and isolates from healthy cows without signs of mastitis (n = 31) were examined. The bacterial isolates were identified by 16S rRNA sanger sequencing. For screening of the most prevalent ESBL, carbapenems and MRSA genes, multiplex primers were used. A total of three samples were included for whole-genome sequencing with Illumina MiSeq. Phenotypic resistance was decided by antibiotic sensitivity testing.Results: Of a total of 76 milk samples from healthy cows, none grew on the selective agar plates. Phenotypic and genotypic antibiotic resistance was detected in both healthy and cows with proven mastitis. By phenotypic screening on selective agar, 2 of 31 (6.5 %) bacterial strains from healthy cows grew on Oxoid Brilliance ™ CRE. The isolates were identified as Enterococcus faecalis. The bacterial strain showed phenotypic resistance to cefotaxime, cefepime, amikacin, gentamicin and streptomycin. In addition, the gene tetM encoding tetracyclineresistance was detected. Bacteria isolates from mastitis samples grew on ESBL plates and were identified as Pseudomonas spp. Two of them showed resistance to cefotaxime and streptomycin. One of the isolates contained mobile genetic elements (aph (6) -Id and aph (3 '') - lb) encoding resistance to aminoglycosides, and the gene AmpC for β-lactam.Conclusion: The main findings of this study show a low incidence of antibiotic resistance in milk isolates from healthy dairy cows. Detection of resistant bacterial isolates shows a need for further research into its role in the spread of antibiotic resistance in the Norwegian food chain.
