Recent literature is reviewed concerning 2 plasma lipoproteins (apoprotein B (apoB), the major apoprotein of low-density lipoprotein (LDL), and apoprotein A-I (apoA-I), the major apoprotein of high-density lipoprotein (HDL)) of interest as risk factors for patients who develop ischemic heart disease. Literature data and findings are discussed relative to disorders associated with: (1) elevated LDL apoB and atherosclerosis (for cases of hyperapobetalipoproteinemia, postmyocardial infarction, endogenous hypertriglyceridemia, phytosterolemia, familial combined hyperlipidemia, and familial hypercholesterolemia; including the metabolic basis for elevated LDL apoB and its relation to atherosclerosis); (2) low plasma HDL and apoA-I and atherosclerosis; (3) elevated plasma apoA-I and decreased incidence of atherosclerosis; and with (4) low LDL and apoB and decreased atherosclerosis incidence. The data reviewed indicate that measurement of plasma levels of apoB and apoA-I provide additional information in the assessment of a patient at risk for coronary artery disease. (wz)
