Recent findings and the current state of understanding are reviewed concerning the adverse effects of excess body weight on human health. Topics include: the utility of published weight tables; the confounding influences of cigarette smoking; the relevance of the length of obesity; and the concept of desirable body weight. A number of US studies all indicate weight associated with the greatest longevity tends to be below the average population weight under consideration, and that "slimmer is better" if underweight is not associated with a medical history of malnutrition, smoking, or alcohol or drug abuse. Data indicate that obesity is a significant independent predictor for cardiovascular risk, with smoking having an effect separate from that of overweight. Other factors also are discussed. (wz)
