Outpatient management of diabetes mellitus with patient education to increase dietary carbohydrate and fiber

A 6-week study of 52 non-insulin-dependent diabetic outpatients assessed the effect of teaching 3 diet plans on fiber intake, diabetes control, and blood lipid levels. The diet plans were: the American Diabetes Association (ADA) diet; the ADA diet with increased high fiber and high carbohydrate foods (IF); and the IF diet with an oat bran supplement. Patients instructed to increase dietary fiber doubled their fiber intake. Increased fiber and carbohydrate and decreased fat intakes correlated with decreased fasting blood glucose levels, while increased fiber intake correlated with decreased levels of blood cholesterol and high-density lipoprotein cholesterol. Modifications in fiber, carbohydrate, and fat intakes, however, were not associated with body weight, blood insulin, or hemoglobin A1c changes.(wz)
