Blood levels of branched-chain amino acids and alpha-ketoacids in uremic patients given keto analogues of essential amino acids

Uremic patients with glucose intolerance also are likely to have an abnormal amino acids metabolism. Serum levels of the branched-chain ketoacids and alpha-ketoacids and their corresponding amino acids were determined in chronic renal failure patients and controls. Uremic patients had significantly lower branched chain ketoacid levels. An oral glucose tolerance test did not decrease alpha-keto-isocaproic acid and alpha-keto-beta-methyl-n-valeric acid levels in uremic subjects, unlike controls. Amino acid-supplemented or unsupplemented diets did not significantly change serum branched-chain ketoacid levels after 28 days. Patients receiving an unrestricted diet with supplementation all showed improvement on the oral glucose tolerance test. The various diets had no consistent effects either on the intravenous glucose tolerance test or on serum levels of insulin and C-peptide. The dosages of keto analogues used were concluded to be too low for normalization of the decreased serum branched-chain amino acid levels in uremia.
