Efecto antiepiléptico de la saxitoxina en la cepa de ratón CD-1

"Los fármacos antiepilépticos (FAE) regulan la sobreexcitación de las células nerviosas para evitar o reducir la frecuencia de las crisis epilépticas, sin embargo, es común que los FAE comerciales presenten efectos adversos, lo que hace necesario proponer fármacos alternativos. La saxitoxina (STX) es una toxina paralizante que bloquea los canales de sodio dependientes de voltaje (VGSC), los cuales son un blanco común para encontrar nuevos antiepilépticos. Para evaluar las crisis epilépticas se utilizan modelos de crisis aguda como el pentilentetrazol (PTZ) en roedores. Una dosis de 100 mg/kg de PTZ inyectada vía intraperitoneal (i.p.) presenta consecutivamente tres crisis: 1) mioclonus, 2) crisis clónico generalizada (CCG) y 3) crisis tónico-clónico generalizada (CTCG). El objetivo fue evaluar el efecto antiepiléptico de la STX en ratones CD-1 utilizando el modelo del PTZ y comparar su efecto con el de la fenitoína (FNT), un antiepiléptico comercial, que al igual que la STX, bloquea los VGSC. Adicionalmente se evaluó si la STX llega al cerebro, mediante cromatografía líquida de alta resolución, para identificar si persiste después de la administración del PTZ. Se establecieron 5 dosis subletales (1.8, 3.9, 6, 7.5, 8.5 µg/kg), una dosis letal media (DL50) de 9.5 µg/kg de STX y una dosis de 50 mg/kg de FNT. El desarrollo experimental consistió en inyectar vía i.p. los vehículos de la STX y la FNT como controles, y las dosis correspondientes de STX y FNT. Después de 30 min se administró i.p. el PTZ (100 mg/kg) a cada uno de los grupos experimentales, y durante los siguientes 30 min se evaluó el porcentaje de ratones que presentaron las crisis y el tiempo que tardaron en iniciar las crisis (latencia). Los resultados mostraron que la STX no evitó las crisis y no se detectó en el cerebro de los ratones, aunque no se puede descartar que hayan quedado niveles traza de STX que no se identificaron con el método de extracción y análisis propuesto. La dosis subletal de STX de 8.5 µg/kg mostró un aumento significativo en la latencia del mioclonus con respecto a su vehículo y a la FNT. La misma dosis disminuyó el porcentaje de ratones que presentaron la CCG en comparación con la FNT. La FNT evitó en el 100% de los ratones, la aparición de la CTCG. Los ratones que sobrevivieron a la DL50 de STX mostraron una disminución en el porcentaje de organismos que presentaron la CTCG..."

"Antiepileptic drugs (AED) regulate the over-excitation of nerve cells to avoid or reduce the frequency of epileptic seizures; however, AED present adverse effects, making it necessary to propose alternative drugs. Saxitoxin (STX) is a paralyzing toxin that blocks voltage-gated sodium channels (VGSC), a frequent target for finding new antiepileptics. Acute seizure models such as pentylenetetrazole (PTZ) in rodents can be used to evaluate seizures. A dose of 100 mg/kg of PTZ injected intraperitoneally (i.p.) presents three consecutive seizures: 1) myoclonus, 2) generalized clonic seizures (GCS), and 3) generalized tonic-clonic seizures (GTCS). The objective was to evaluate the antiepileptic effect of STX in CD-1 mice using the PTZ model and to compare its effect with phenytoin (PHT), a commercial antiepileptic that also blocks VGSCs. It was also evaluated if STX reaches the brain, using high-performance liquid chromatography, to identify if it persists after administration of PTZ. Five sublethal doses were established (1.8, 3.9, 6, 7.5, 8.5 µg / kg), a mean lethal dose (LD50) of 9.5 µg/kg for STX, and a dose of 50 mg/kg for PHT. The experimental design consists of injecting intraperitoneally (i.p.) STX and PHT vehicles as controls, and the corresponding doses of STX and PHT. After 30 minutes, the PTZ (100 mg/kg) was administered i.p. to each of the experimental groups; during the following 30 minutes, the percentage of mice that presented the seizures and the time it took to start the seizures were evaluated. The results determined that STX did not prevent seizures and was not detected in the mice's brains, although it cannot be ruled out that they have had trace levels of STX that are not identified. The sublethal STX dose of 8.5 µg/kg showed a significant increase in myoclonus latency compared to its vehicle and PHT. The same treatment decreased the percentage of mice that included GCS compared to PHT. PHT prevented the appearance of GTCS in 100% of the mice. Mice that survived STX LD50 showed a decrease in the percentage of organisms that presented the GTCS. This work is the first report where the STX is proposed as FAE, observing significative effects on latency and the percentage of mice that present seizures. STX has a narrow therapeutic margin; more studies are needed to elucidate the antiepileptic effects of STX or any of its analogs that have less toxic potency and a higher therapeutic range."
