Vergleich einer subkutanen Injektion oder Infusion mittels osmotischer Pumpe des Ghrelin-Analogons BMI-28131 bezüglich der Effekte auf Körpergewicht, Körperzusammensetzung und Herzfunktion im Rattenmodell der kardialen Kachexie | The effects of repeated subcutaneous injections vs. continuous application via osmotic pump of the ghrelin-analogue BIM-28131 on the body weight, composition and cardiac function in a rat model with CHI

Die kardial bedingte Kachexie ist eine häufige und ernst zu nehmende Komplikation der CHI, die mit einer ungünstigen Prognose verbunden ist und deren multifaktorielle Ätiologie noch nicht vollständig geklärt ist. Für die Behandlung der kardialen Kachexie gibt es bisher keine spezielle Therapie. Ein mögliches Behandlungsziel bei kardialer Kachexie könnte darin bestehen, das nichtödematöse Gewicht zu stabilisieren bzw. anzuheben. Dies könnte sich positiv auf das Krankheitsgeschehen und letztlich auf die Lebensqualität und Überlebensrate bei Patienten mit CHI auswirken. Ghrelin, welches zum größten Teil in der Magenschleimhaut synthetisiert wird, wirkt als physiologisches Appetitstimulanz und beeinflusst daneben die Wachstumshormonfreisetzung ins Blut. In vorliegender Studie wurde der Einfluss des Ghrelin-Analogons BIM-28131 auf den Gewichtsverlauf, die Körperzusammensetzung, die Organgewichte und die kardiologischen Funktionen in einem Rattenmodell mit CHI untersucht, wobei eine wiederholte subkutane Injektion mit einer kontinuierliche Applikation via osmotischer Pumpe verglichen wurde. Der bei den juvenilen Ratten erzeugte Myokardinfarkt mit ca. 48% linksventrikulärem Infarktareal führte bei allen Infarkttieren zu einer Herzinsuffizienz, jedoch konnte kein eindeutig kachektischer Zustand erzeugt werden. Die im Rahmen dieser Dissertation erzielten Ergebnisse zeigten jedoch, dass das Ghrelin- Analogon BIM-28131 einen günstigen Einfluss in der Verhinderung der Entwicklung eines kachektischen Zustandes bei Ratten mit CHI erkennen lässt. Die Verabreichung des Ghrelin-Analogons BIM-28131 führte zu einem Ansteigen des Körpergewichtes, unabhängig von der Applikationsart. Zudem zeigten die Ergebnisse ein Ansteigen der Fett- und Muskelmasse im Vergleich zu den placebobehandelten Tieren, wobei die Art der Applikation auf Fett- bzw. Muskelzunahmen unterschiedlichen Einfluss hatte. Entgegen den Ergebnissen anderer Studien mit Ghrelin konnte nach Verabreichung des Ghrelin-Analogons BIM-28131 – unabhängig von der Applikationsart – nur eine geringfügige Verbesserung der kardialen Funktionen beobachtet werden. Bei der zukünftigen Entwicklung einer speziellen Therapie bei kardial bedingter Kachexie könnten diese Ergebnisse Berücksichtigung finden, jedoch sollte im Sinne einer verbesserten Compliance für den Patienten alternative Applikationsformen Anlass zu weiterführender Forschung geben.

A serious complication of chronic heart failure (CHI) is cardiac cachexia, which is associated with a poor prognosis but its multifactorial etiology is not completely clarified yet. There is currently no therapeutic option for the treatment of the cardiac cachexia. An option for a treatment of the cardiac cachexia could be to stabilise or even increase the non-oedematous weight. This could positively affect the progress of the recovery and finally the quality of life, as well as the survival rate of patients with CHI. Ghrelin, which is produced in the gastric lining, is effective as a physiologic appetite stimulant and influences the release of growth hormone into the blood stream. This study determined the effects of repeated subcutaneous injection vs. continuous application via osmotic pump of the ghrelin analogue BIM-28131 on the body weight and composition, as well as the weight of organs and cardiac function in a rat model with CHI. Myocardial infarction of the left ventricule was induced in juvenile rats with a size of approx. 48%, which led to heart failure in all animals. However, it was not possible to induce a welldefined cachectic status. However, the results of this research showed that ghrelin analogue BIM-28131 had some beneficial potential for cachexia treatment in rats with CHI. The medication of the ghrelin analogon induced an increase in body weight independently of the type of application. In addition, the results showed an increase in fat and muscle mass compared to the placebo- treated rats, whereas the way of application had a different effect on fat- and muscle growth. In contrast to the results of other studies with ghrelin, the application of ghrelin analogue BIM-28131 could only slightly improve the cardiac function, independently of the way of application. In future, the results of the research should be considered when developing a special therapy for cardiac cachexia. However, alternative forms of application should also be taken into account in order to improve the compliance of patients.
