Etablierung der kombinierten Transplantation syngener Hepatozyten mit allogenem Lebertransplantat im Rattenmodell zur Untersuchung der Toleranzinduktion | Establishment of the combined transplantation of syngeneic hepatocytes with an allogeneic liver graft in a rat model for the investigation of tolerance induction

Die orthotope Lebertransplantation (OLT) stellt den Goldstandard bei der Therapie terminaler Lebererkrankungen dar. Die zum Transplantaterhalt obligate Immunsuppression geht mit einer Bandbreite an unerwünschten Nebenwirkungen einher und schränkt die Langzeitergebnisse der OLT dadurch maßgeblich ein. Eines der Hauptprobleme der Ganzorgantransplantation ist des Weiteren der ausgeprägte Mangel an geeigneten Spenderorganen. Die Hepatozytentransplantation ist eine vielversprechende alternative Therapieoption bei irreversiblen Leberschäden und wurde bereits in klinischen Studien untersucht. Ziel des vorliegenden Promotionsvorhabens ist die Kombination einer allogenen Lebertransplantation mit der Transplantation autologer Leberzellen. Die Isolierung geeigneter Spenderzellen aus den erkrankten Explantaten der Patienten hat den Vorteil des Erhalts autologer metabolisch aktiver Hepatozyten und idealerweise zusätzlich exprimierter hepatischer Progenitorzellen (hepatic Progenitor Cells, hPC). Eine gemeinsame Transplantation dieser Zelltypen (combined Liver Cell Transplantation, cLCTx) begünstigt die Integration der Leberzellen in das Wirtsparenchym und ermöglicht die erfolgreiche Repopulation der allogenen Spenderleber. Es wird postuliert, dass es durch das Engraftment der transplantierten Leberzellen zur teilweisen „Autologisierung“ des Lebertransplantats kommt und dadurch eine Reduktion der Immunsuppression möglich ist. Ziel dieser Studie ist die Generierung eines in vivo rejektionsfreien Neo-Hybriden Lebertransplantats (NHL). Um diese Hypothese zu untersuchen und eine Grundlage für die spätere klinische Umsetzung zu schaffen, soll das Konzept des NHL im Rattenmodell etabliert werden. Um die generelle Durchführbarkeit zu überprüfen wurde eine allogene Lebertransplantation einer, zuvor mit dem leberspezifischen Mitose- Inhibitor Retrorsin vorbehandelten, weiblichen Dark Agouti-Spenderleber in eine weibliche Lewis-Empfängerratte vorgenommen. Zusätzlich zu dem allogenen Lebertransplantat erhielt das weibliche Empfängertier eine kombinierte Transplantation syngener Hepatozyten und hPC eines männlichen Lewis- Zellenspenders. Die Proliferationsstimulation der metabolisch aktiven Hepatozyten und hPC erfolgte durch eine Vorbehandlung des Zellspenders mit 2 -Acetyl-Aminofluorene (2-AAF) in Kombination mit einer 60%-igen Leberteilresektion (Partial Hepatectomy, PH). Die cLCTx in das Milzparenchym erfolgte entweder direkt im Anschluss an die OLT (intraoperativ) oder zeitversetzt in einem Abstand von maximal 24 h zur Lebertransplantation. In Anschluss an die kombinierte Leber- und Leberzelltransplantation (cLTx) erhielten die Empfängertiere eine konstante Immunsuppression mit Cyclosporin A (CsA). Durch die selektive Blockade des Zellzyklus in der allogenen Spenderleber, soll es zum umfangreichen Engraftment der syngenen Hepatozyten und hPC und somit zur erfolgreichen Repopulation im Sinne eines NHL kommen. Zum Nachweis der Integration der transplantierten Leberzellen in das Parenchym des allogenen Transplantats wurden immunhistologische Markierungen der hPC- spezifischen Oberflächenantigene Thy-1, OV-6 und EpCam vorgenommen. Außerdem erfolgte anhand der immunhistologischen PCNA-Färbung eine Proliferationsanalyse der transplantierten Leberzellen als Bestätigung des Engraftments. Für die Detektion der syngenen Hepatozyten und hPC im weiblichen Lebertransplantat wurde eine Fluoreszenz in situ Hybridisierung (FisH) zum Nachweis der Y-Chromosomen-positiven Zellkerne zu festgelegten Zeitpunkten (Tag 8, 15, 30 und 90) nach der cLTx durchgeführt. Über den Beobachtungszeitraum zeigte sich eine kontinuierliche Zunahme des prozentualen Anteils Y-Chromosomen-positiver Kerne im Lebergewebe mit 7,75 % +/- 0,79 % SEM an Tag 8, 7,97 % +/- 0,85 % SEM an Tag 15, 10,20 % +/- 0,91 % SEM an Tag 30 und 19,61 % +/- 0,78 % SEM an Tag 90 nach der cLTx. Die erfolgreiche Repopulation des allogenen Lebertransplantat durch die transplantierten syngenen Hepatozyten und hPC zeigte sich anhand des im Zeitverlauf zunehmenden prozentualen Anteils der mittels FisH nachgewiesenen transplantierten Leberzellen. Im Vergleich zu den übrigen Gruppen zeigte Gruppe D (längster Beobachtungszeitraum mit 90 Tagen) jeweils einen signifikant unterschiedlichen prozentualen Mittelwert der YChromosomen- positiven Zellkerne mit einem p-Wert < 0,001. Die Überlebensrate von 75,86 % bei 29 durchgeführten cLTx belegt die erfolgreiche Etablierung dieses Rattenmodells. Der durch die cLTx erworbene syngene Charakter des Neo-Hybriden Lebertransplantats macht dieses innovative Rattenmodell zu einer vielversprechenden Grundlage für die zukünftige Untersuchung der Induktion einer operativen Toleranz.

Liver transplantation (LTx) is the standard therapy for patients with terminal liver failure. Nonetheless long-term survival is still affected by serious side effects of immunosuppression. Hepatocyte transplantation (HcTx), a possible alternative to LTx until now has not shown long-lasting clinical success. The aim of this study was to create a novel rat model for investigating the feasibility of combining an orthotopic LTx and transplantation of autologous liver cells isolated from the patient’s own diseased liver. Therefore, we performed allogeneic LTx with Retrorsin- pretreated liver grafts from female donors (Dark Agouti) into female recipients (Lewis). Following this procedure syngeneic hepatocytes and hepatic progenitor cells (hPC) were isolated from 2-AAF/PH -preconditioned male Lewis rats and these combined liver cells were transplanted into the recipient’s spleen (cLCTx). Once these cells have engrafted, it is postulated that these syngenic cells will repopulate the allogeneic liver graft as they have a selective advantage over the donor tissue. It is hypothesized that this will lead to a neo-hybrid liver graft (NHL), reducing the necessity of immunosuppression and possibly inducing tolerance to both the entire engrafted tissue and the allogeneic donor matrix. Aim of this study was to investigate the feasibility of the NHL concept in the Dark Agouti/Lewis rat model under stable immunosuppression (cyclosporin A). Animals were sacrificed at four different time-points (day 8, 15, 30, 90) after LTx. In order to analyse the degree of cell engraftment and repopulation, Y-chromosome detection of transplanted male cells was performed by Fluorescence in situ Hybridisation (FisH). Immunohistochemical analyses of hPC specific markers (OV-6, Thy-1 and EpCam) in combination with proliferation specific staining (PCNA) were used to identify transplanted cells. Correlating immunohistochemical results of hPC specific markers and proliferation stains with FisH demonstrated efficient engraftment of transplanted cells. The results of FisH-based detection of the transplanted liver cells were as follows: day 8 with 7.75% +/- 0.79% SEM, day 15 7.97% +/- 0.85% SEM, day 30 10.20% +/- 0.91% SEM and day 90 with 19.61% +/- 0.78% SEM after combined cLCTx and LTx. The percentage of Y-chromosome- positive cell nuclei obtained on day 8, 15 and 30 each demonstrated significant difference (p < 0,001) in comparison to day 90. The increasing quantity of Y-chromosome-positive cell nuclei detected during follow-up confirmed successful repopulation of the liver graft and thus leading to the establishment of a NHL. Animals surviving the first 3 days after combined liver transplantation and liver cell transplantation (cLTx) showed stable liver function until they were sacrificed. Survival rate after cLTx (n= 29) was 75.86% thus underlining the feasibility of this newly established rat model. The Neo-Hybrid Livergraft therefore provides a valuable foundation for future experimental research models regarding the induction of operational tolerance.
