Nutrition interventions manual for professionals caring for older Americans

This manual is designed to facilitate cooperation between older individuals, their family members and friends, social services professionals, dietitians, physicians and other health professionals so that routine nutrition screening and interventions will be implemented. Summarizes interventions for the problems identified through use of the Nutrition Screening Initiative's determine your nutritional health checklist and level I and II nutrition screens. These intervention areas are social services, oral health, mental health, medications use, nutrition education and counseling, and nutritional support.
