Non-insulin-dependent diabetes: 10-Years outcome in relation to initial response to diet and subsequent sulfonylurea therapy

A total of 13 "responders" of 38 non-insulin-dependent diabetes (NIDD) patients within 130% of desired body weight receiving diet therapy with energy and carbohydrate restriction improved in fasting glucose, glucose tolerance, insulin secretion, and other clinical signs. At diagnosis, the responders were heavier than non-responders. After 10 years, 29 of the patients were reexamined; 22% of the responders and 70% of the non-responders were now on insulin. Microangiopathy incidence was significantly higher in non-responders. The results suggest that NIDD is due to incomplete insulin deficiency that will undergo remission in response to diet therapy in about a third of the cases; in the remainder, sulfonylurea therapy temporarily increases insulin and can be used to delay insulin therapy.
