A literature review assesses the question of whether exercise is always good therapy for osteoporosis. Study results indicate that use increases bone mass and disuse diminishes it, suggesting that osteogenic exercise should be diverse, rigorous, but non-repetitive. Aspects of osteoblasts, osteoclasts, and osteocytes are discussed, as are mineral and prostaglandin metabolism. It is concluded that exercise as treatment for osteoporosis is recommended for the elderly where osteoporosis is partly due to "sleepy" osteoblasts, but not for young subjects when used excessively.
