An enzyme sequence which is associated with the renal system (kallikrein-kinin systems) and produces kinins, has been implicated in renal vasodilation and natriuresis. A review of aspects of this enzyme system as it relates to sodium (Na) intake indicates that the deficit in kallikrein excretion observed in hypertension may be restored during successful anti-hypertensive treatment of Na balance (by dietary Na restriction or by thiazide diuretic treatment). It is not known, however, whether therapy-induced increases in kallikrein excretion result from the accompanying blood pressure reduction and hormonal changes, or whether a kallikrein excretion increase mediates blood pressure fall; further research is needed to clarify this important uncertainty.
