Some questions have arisen concerning the treatment of obese patients with triiodothyronine (T3) and a 320 kcal diet, which pinpoint the difficulties in verifying dietary compliance and caloric output, and a diminished sensitivity to T3 reflected in decreased clearance of T3 and decreased receptor uptake during caloric deprivation. The reduction in availability of receptors may explain the lack of metabolic response to thyrotoxic levels of serum T3. Further information on weight loss and RMR in a placebo group and T3 treated group reveals the correlation between weight loss and RMR during the final weeks of testing when weight loss decelerated. The final scatter of T3 levels implies that some patients do have reduced clearance rate; it should be noted that this group was a highly selected subgroup of obese patients.
