Potenciales interacciones farmacológicas entre antidepresivos y analgésicos empleados para manejo del dolor dental. Revisión temática | Potential drug interactions between antidepressants and analgesics used for dental pain management. Thematic review

El manejo del dolor dental es una práctica habitual en odontología, que requiere el empleo de analgésicos como los AINES y en algunos casos los opioides débiles. Actualmente, el número de pacientes que se encuentran bajo tratamiento con antidepresivos ha incrementado de manera significativa, lo que abre la posibilidad de que se induzcan interacciones farmacológicas relevantes cuando se prescriben medicamentos durante la consulta odontológica. Esta revisión temática tuvo como objetivo identificar y analizar las interacciones farmacológicas que se pueden generar por la administración simultánea de antidepresivos y analgésicos comúnmente prescritos en odontología. Se realizó una revisión temática usando las siguientes bases de datos PubMed, Embase y LILACS. La búsqueda permitió identificar que la combinación de tramadol con antidepresivos, en especial ISRS y tricíclicos, aumenta significativamente el riesgo de síndrome serotoninérgico. Asimismo, la coadministración de AINES como el ibuprofeno con antidepresivos como fluoxetina o sertralina incrementa el riesgo de hemorragias gastrointestinales debido a efectos sinérgicos sobre la agregación plaquetaria. Estas interacciones se relacionan principalmente con mecanismos farmacodinámicos y farmacocinéticos, donde participa la inhibición del citocromo P450 (CYP2D6 y CYP3A4), que altera la metabolización de los fármacos. El desconocimiento de los odontólogos respecto a las interacciones entre AINES y antidepresivos puede comprometer la seguridad del paciente, afectando tanto la eficacia del tratamiento del dolor como la estabilidad del tratamiento antidepresivo. Se resalta la necesidad de que los odontólogos realicen una historia farmacológica detallada y consideren alternativas más seguras como el paracetamol, en pacientes que están recibiendo terapias con antidepresivos, además de recurrir a interconsultas con el médico tratante cuando sea necesario. Este conocimiento es esencial para garantizar una atención odontológica segura, individualizada, integral y basada en la evidencia.

The management of dental pain is a common practice in dentistry, requiring the use of analgesics such as NSAIDs and, in some cases, weak opioids. Currently, the number of patients undergoing treatment with antidepressants has increased significantly, raising the possibility of relevant drug interactions when medications are prescribed during dental consultations. This thematic review aimed to identify and analyze the drug interactions that can be generated by the simultaneous administration of antidepressants and analgesics commonly prescribed in dentistry. A thematic review was conducted using the following databases: PubMed, Embase, and LILACS. The search identified that the combination of tramadol with antidepressants, especially SSRIs and tricyclics, significantly increases the risk of serotonin syndrome. Likewise, the co-administration of NSAIDs such as ibuprofen with antidepressants such as fluoxetine or sertraline increases the risk of gastrointestinal bleeding due to synergistic effects on platelet aggregation. These interactions are mainly related to pharmacodynamic and pharmacokinetic mechanisms involving the inhibition of cytochrome P450 (CYP2D6 and CYP3A4), which alters drug metabolism. Dentists' lack of knowledge about the interactions between NSAIDs and antidepressants can compromise patient safety, affecting both the efficacy of pain treatment and the stability of antidepressant treatment. There is a clear need for dentists to take a detailed medication history and consider safer alternatives such as paracetamol in patients receiving antidepressant therapy, as well as consulting the treating physician when necessary. This knowledge is essential to ensure safe, individualized, comprehensive, and evidence-based dental care.
