Estadiamento e caracterização da proteinúria em cães naturalmente infectados com Leishmania infantum | Staging and characterization of the protein in dogs naturally infected with Leishmania infantum

Canine visceral leishmaniasis (CVL), a disease with high prevalence in Brazil, has as one of its main consequences renal injury due to tubular and glomerular lesions resulting mainly from the deposition of immune complexes. The presence of asymptomatic infected animals hinders the diagnosis that must be made through the combination of physical, parasitological, serological and molecular tests. After diagnosis of the infection, its staging is essential for a better composition of the therapeutic management. For that, markers are needed that can indicate the evolution and prognosis of kidney disease, one of the most important occurrences in CVL, which causes death in many infected animals. Thus, in the search for an early biomarker of renal injury, this study aimed to determine the urinary protein profile of dogs infected with leishmaniasis treated and without treatment. For this, 30 dogs from an outpatient clinic at a Veterinary Hospital in Belo Horizonte were evaluated. All animals underwent clinical, ultrasound and laboratory tests that included renal biomarkers such as urinalysis, urinary protein and creatinine ratio (UPC), symmetrical dimethylated arginine (SDMA), blood urea nitrogen (BUN) and serum creatinine. In addition, urinary proteins were characterized using the SDS-PAGE electrophoresis technique, and thus, a urinary protein profile was developed comparing healthy, uninfected patients with dogs infected with Leishmania infantum with and without treatment. The results showed that the hematological and biochemical parameters showed similar behavior between the groups of uninfected dogs and treated infected dogs, however a very heterogeneous pattern of urinary proteins can be observed and differed between healthy non-infected animals and animals infected with L. infantum with or without treatment. The results suggest that the classification of proteinuria can be a tool that helps in the staging of animals infected with L. infantum and can differentiate them in terms of the severity of existing kidney injuries

A leishmaniose visceral canina (LVC), doença com alta prevalência no Brasil, tem como uma de suas principais consequências a injúria renal devido a lesões tubulares e glomerulares decorrentes principalmente da deposição de imunocomplexos. A presença de animais infectados assintomáticos dificulta o diagnóstico que deve ser realizado através da combinação de exames físico, parasitológico, sorológicos e moleculares. Após diagnóstico da infecção é primordial seu estadiamento para melhor composição do manejo terapêutico. Para tanto, são necessários marcadores que possam indicar a evolução e prognóstico da doença renal, uma das mais importantes ocorrências na LVC, que causa morte em muitos animais infectados. Na busca por um biomarcador precoce de injúria renal, este trabalho teve como objetivo determinar o perfil proteico urinário de cães infectados com leishmaniose tratados e sem tratamento. Para tanto, foram avaliados 30 cães oriundos do atendimento clínico ambulatorial de um Hospital Veterinário em Belo Horizonte. Todos os animais passaram por exames clínico, ultrassonográfico e laboratorial que incluíram biomarcadores renais como urinálise, relação proteína e creatinina urinárias (RPC), arginina dimetilada simétrica (SDMA), nitrogênio ureico sanguíneo (BUN) e creatinina sérica. Além disso, as proteínas urinárias foram caracterizadas através da técnica de eletroforese por SDS-PAGE, e assim, foi elaborado um perfil proteico urinário comparando pacientes hígidos não infectados, com cães infectados com Leishmania infantum com e sem tratamento. Os resultados demonstraram que os parâmetros hematológicos e bioquímicos apresentaram comportamento semelhante entre os grupos de cães não infectados e de cães infectados tratados, entretanto um padrão muito heterogêneo de proteínas urinárias pode ser observado e diferiu entre animais hígidos não infectados e animais infectados com L. infantum com ou sem tratamento. Os resultados sugerem que a classificação da proteinúria pode ser uma ferramenta que auxilia no estadiamento de animais infectados por L. infantum podendo diferenciá-los quanto à gravidade de lesões renais existentes.
