Untersuchungen zu den Verschlussverhaeltnissen im kaudalen Genitalkanal der Stute | Investigations on the Occlusive Means in the Caudal Genital Tract of the Mare

Die vorliegende Arbeit wurde an 5 geschlechtsgesunden Stuten ausgefuehrt. Zur Beschreibung der physiologischen Verschlußverhaeltnisse musste zunaechst ein Messgeraet entwickelt werden, da auf dem Markt kein entsprechendes Gerät verfuegbar ist und derartige Messungen noch nicht beschrieben wurden. Nach Entwicklung und Validierung des Meßgeraetes steht nunmehr ein Instrument zur Verfuegung, mit welchem die Verschlussverhaeltnisse des Genitalkanals erstmals beschrieben werden konnten. Die Messungen erfolgten in drei unterschiedlichen, genau definierten Lokalisationen. Dies waren der Scheidenvorhof, der Hymenalring sowie die kaudale Vagina unmittelbar kranial des Hymenalringes. Jede Stute wurde über 3 vollständige, ovulatorische Sexualzyklen am Tage der Ovulation, am 2. Tag post ovulationem (p.o.), 5. Tag p.o, 12. Tag p.o., 15. Tag p.o sowie ab dem 17. Tag p.o. taeglich bis zur erneuten Ovulation untersucht. Um einen Einfluß des hormonellen Status und der Konformation der aeusseren Geschlechtsorgane auf den intragenitalen Druck zu ermitteln, wurden vor der Druckmessung in den drei Lokalisationen die Streckenlaengen Anusmitte ý dorsale Kommissur, Anusmitte ý ventrale Kommissur, dorsale Kommissur ý ventrale Kommissur und nach Einlegen eines Besamungskatheters die Strecke dorsale Kommissur ý Beckenboden mittels eines Bandmaßes sowie der Neigungswinkel der Schamspalte mit einem Winkelmesser bestimmt. Zum Abschluß des Untersuchungsganges wurde durch eine transrektale Palpation unter anschließender Ultraschallkontrolle der ermittelten Befunde der Zyklusstand der Stute verifiziert. Schließlich wurde eine Blutprobe aus der Vena jugularis zur Bestimmung der Sexualhormonkonzentration entnommen. Im Vergleich der drei gemessenen Lokalisationen war bei 4 von 5 Stuten im Hymenalbereich der hoechste Verschlußdruck zu verzeichnen. Durch den Hymenalring ist nach SCHUBERT (1994) die deutlichste Reduktion pathogener Keime im Genitalkanal zu verzeichnen. Diese Interpretation ist durch den hier zu verzeichnenden hoechsten Verschlussdruck zu untermauern. Auf Grund seiner uebergeordneten Rolle beim Verschluss des Genitalkanals sollte bei einer chirurgischen Intervention beim mangelhaften Schamschluß diesem Verschlussmechanismus verstaerkt Rechnung getragen werden. Es empfiehlt sich, zumindest den Scheidenvorhof, aber besser noch die Funktion des Hymenalringes gezielt wieder herzustellen, um damit einem moeglichen Keimaufstieg entgegenzuwirken. Die Operationsmethode nach HERFEN und BOSTEDT (1998) scheint dabei ein erfolgversprechender Ansatz zu sein.

This study deals with the occlusive means in the caudal genital tract in 5 mares. Since there was no measuring device available, to reliably describe the pressure in the whole caudal genital tract, the first aim of the study was, to develop an adequate ýpressure-gaugeý. After successful development of such a measuring device, the second goal was to identify the ýsealý to the outside world, i. e. the area with the highest occlusive pressure. Therefore pressure measurement was performed in three well defined areas. These were the vestibulum vaginae, the hymenal ring and the area of the caudal vagina. Measurement was done on 3 consecutive ovulatory oestrous cycles, starting at the 2nd day post ovulationem, continuing on 5th, 12th, 15th, and from the 17th day on a daily interval until next ovulation. To investigate the influence of hormonal status and constitutional conditions of the external genitalia on the internal pressure values, before measuring, the length of the following distances were determined: Anus-dorsal commissure, anus-ventral commissure, dorsal commissure-pelvic brim and dorsal commissure-ventral commissure. Vulva´s angle of declination was also measured, using a protractor. After pressure measurement, the mares were examined transrectally and by ultrasonography to verify the state of the sexual cycle. Finally a blood sample was taken, to check the concentration of estrogen-17-β (E2) and progesterone (P4) in serum by RIA. Comparison of the values measured in the three locations resulted in the hymenal ring as the area with the highest occlusive pressure. The hymenal ring is not only the place of the most effective reduction in bacteria all over the genital tract in mares (SCHUBERT 1994), it is also the place of the highest occlusive pressure in 4 of 5 investigated mares. Accepting the hymenal ring being the most important occlusive seal to the outside world in the genital tract of the mare, Caslick´s vulvoplasty seems to be no longer an adequate method in the therapy of vulval incompetence/pneumovagina. The author advises the method published by HERFEN and BOSTEDT (1998). It seems to reestablish physiological conditions in the occlusion of the genital tract of the mare by strengthening the hymenal ring area.
