Nephrectomy in a cow with pyelonephritis

English. A seven-year old Holstein cow presented with right renal pyelonephritis. The urine analysis results provided the definitive diagnosis and the unilateral aspect of the condition was established because the blood-urea and blood-creatinine concentrations were normal and the left kidney had a normal structure on ultrasonographic examination. The right kidney was severely hypertrophic and misshapen. A nephrectomy via the right flank was performed. However, the animal did not survive (died three hours after the intervention subsequent to shock). Pyelonephritis is the most common renal disorder seen in cattle. Characteristics of the condition include haematuria, proteinuria, bacteriuria and high leukocyte count. Medical treatment consists of antibiotherapy for at least three weeks. When the kidney is severely affected and unilateral, nephrectomy is indicated
Show more [+] Less [-]Italian. Una vacca Holstein di sette anni è colpita da una forma di pielonefrite limitata al rene destro. La formulazione della diagnosi definitiva è possibile grazie all'esame delle urine, mentre il carattere monolaterale della lesione è definito dai valori normali di uremia e creatinemia, associati a una struttura ecografica normale del rene sinistro. Il rene destro, notevolmente ipertrofico e rimaneggiato, è oggetto di una nefrectomia con accesso dal fianco destro, che non permette di salvare l'animale, che muore tre ore dopo l'intervento per il grave stato di shock. La pielonefrite è la patologia renale riscontrata con maggior frequenza nei bovini. Sono caratteristici di tale patologia: ematuria, proteinuria, batteriuria e ammassi di leucociti. Il trattamento medico si basa sull'antibioticoterapia per almeno tre settimane. La nefrectomia è indicata quando la patologia renale è avanzata e monolaterale
