Five-year findings of the hypertension detection and follow-up program. I. Reduction in mortality of persons with high blood pressure, including mild hypertension

A systematic antihypertensive treatment 5-year program significantly reduced mortality and reduced blood pressure to within the normotensive range for more than 50% of the patients. Over 10,000 persons with high blood pressure were randomly assigned to Stepped Care (SC)--the special treatment--or Referred Care (RC) for conventional treatments. The SC group received medication increased stepwise to bring their diastolic blood pressure within a predetermined level, and were examined at intervals. Mortality after 5 years was 17% lower for the SC group than for the RC group, and 20% lower for the mild hypertensive subgroup. After 5 years, 64.9% of the SC group achieved blood pressure at or below their goals, compared to 43.6% for the RC group. The findings indicate that the systematic management program has a strong potential for reducing mortality among the large number of people in the population who have high blood pressure.
