Accumulated knowledge on the chemical composition, biochemistry and epidemiology of the high-density lipoproteins (HDL) has provided evidence for an inverse correlation between plasma HDL cholesterol content and coronary artery disease, which is independent of other risk factors. Precisely how HDL cholesterol functions in lipid transport is not yet clear, in light of the identification of different HDL subclasses. Further clinical and epidemiological research is needed to clarify the link between HDL and coronary atherosclerosis. The Lipid Research Clinics Program Prevalence Study has assessed the relation of HDL to populational characteristics. HDL was positively associated with age, routine physical activity, education and alcohol intake, and negatively associated with body fatness, carbohydrate intake and cigarette use. No correlations, however, between HDL and dietary fats, cholesterol or protein were observed. Thus, changes in coronary risk associated with alcohol, cigarettes, obesity, exercise and oral contraceptives may be related to changes in HDL metabolism.
