High-carbohydrate, high-fiber diets for insulin-treated men with diabetes mellitus

Carbohydrate-restricted diets traditionally recommended for patients with diabetes mellitus and recommendations for liberalization of complex carbohydrate intake need to be evaluated in light of metabolic studies. After consuming a control diet with 43% of calories as carbohydrates for about 7 days, 20 men, who were on insulin therapy were given a high-carbohydrate, high plant fiber diet for about 16 days. On the control diet, plasma glucose and urine glucose values and insulin doses were fairly constant. On the test diet the daily insulin dose averaged 11 units/day as compared to 26 units/day on the control diet. Insulin was discontinued for 9 patients receiving 15 to 20 units/day and for 2 receiving 32 units/day. Fasting and 3-hr postprandial plasma glucose values were lower in most patients, serum cholesterol levels dropped from 206 mg/dl to 147 mg/dl, but fasting serum triglyceride average values did not change significantly. Thus, a high-carbohydrate, high plant fiber diet may be the therapy of choice for certain patients with the maturity-onset type of diabetes.
