Non-glucose sugars used as substitute sweeteners in diabetic diets appear to be safe at moderate levels, but their long-term effectiveness and safety in quantity has not been established. Xylitol in large amounts at a single sitting, however, can cause osmotic diarrhea and some animal studies suggest that chronic ingestion may be tumorigenic. Fructose and sorbitol in limited amounts are acceptable and may eventually be found to provide advantages for dietary management of persons of normal body weight. Overweight persons are advised not to use them. Until more data are available and the sweeteners can be recommended for significant use in diabetic diets, levels no higher than 50-60 g/day are considered safe since they have no known side effects. Amounts above that level can cause diarrhea.
