Management of the pregnant, insulin-dependent diabetic woman

The use of patient-monitored glucose determinations was found to be effective in achieving and maintaining normoglycemia in pregnant diabetics. Their mean glucose levels fell from 169 mg/dl to 87 mg/dl. Urinary glucose excretion was 1.4 g per 24 hours. Hemoglobin A1c levels decreased to the normal range 5 weeks after normoglycemia was attained. The women's hormonal profiles became normal after normoglycemia was achieved and remained normal until delivery. The average weight gain for the mothers was 12.2 kg; the mean infant birth weight was 2988 g. None of the infants manifested respiratory disease, hypoglycemia, hypocalcemia, or erythremia. The patients accepted the program readily. Data on the number of daily glucose determinations and clinic attendance demonstrate excellent compliance.
