Salud y género en los grupos domésticos. El caso de diabetes mellitus

Resumen en español: "Diabetes mellitus es una de las pocas patologías, diferente a las relativas a la reproducción biológica, más frecuentes en mujeres que en hombres, la incidencia creciente y las características epidemiológicas en la distribución la convierten en un problema de salud pública (Velázquez et al, 2003; Venn y Mann, 2004; Rull et al, 2005; SSA, 2005). La trascendencia de este problema se observa sobre los individuos, las familias y las instituciones; en las dimensiones biológica, psicológica y social. Las tasas de mortalidad indican que la defunción por esta causa está ocurriendo con un ritmo más acelerado en hombres que en mujeres, y aún cuando éstas demandan atención con mayor frecuencia a las instituciones de salud, el acceso a estos servicios no explica por si mismo el fenómeno. La esperanza de vida es mayor para las mujeres, pero se sabe poco respecto a la calidad de vida en los años posteriores al diagnóstico. El propósito de este trabajo es analizar, utilizando la perspectiva de género, las formas en que tanto mujeres como hombres enfrentan la enfermedad crónica y procuran su autocuidado en el interior del espacio doméstico, en un contexto de pobreza aunado a políticas de salud orientadas hacia la privatización. Se realizaron entrevistas en profundidad, con adultos enfermos (mujeres y hombres) residentes en Tuxtla Gutiérrez, con el propósito de aportar elementos que permitan comprender cómo se produce el autocuidado en el espacio doméstico. La información que se analiza proviene de entrevistas realizadas a dieciséis mujeres y siete hombres, muestra extraída de un censo nominal realizado en los cuatro meses previos (Nazar, 2005). Éstas, permitieron establecer el papel que juegan las identidades y roles de género, así como el acceso y control a los recursos (monetarios y no monetarios) y las redes sociales de apoyo; aspectos que se configuran en un conjunto diferenciado de oportunidades y obstáculos para que mujeres y hombres viviendo con diabetes mellitus logren llevar a cabo un autocuidado exitoso de la enfermedad. Se reúnen evidencias suficientes para afirmar que, en este grupo de participantes, los hombres tienen mayores oportunidades para llevar a cabo su autocuidado, aunque las restricciones provienen de su propia identidad de género, lo que finalmente, tiene un impacto negativo sobre su condición de vida. Por su parte, las mujeres tienen mayores obstáculos para el autocuidado, pero encuentran elementos dentro de su entorno doméstico para sobrellevar el peso de la enfermedad crónica, a pesar de las relaciones conflictivas en su acceso a los recursos y la violencia que ejercen sus parejas sobre ellas. Un aspecto relevante del trabajo es el análisis del grupo doméstico, desde la perspectiva de género y la tipología que se propone para dilucidar la manera en que la organización familiar participa en la estructuración de las oportunidades y obstáculos que enfrentan tanto mujeres como hombres para cuidar de sí mismas/os. Se propone un modelo que pretende contribuir a la explicación de la manera en que estos elementos culturales y económicos impactan sobre el autocuidado de hombres y mujeres viviendo con diabetes mellitus. Comprender los comportamientos adultos en su grupo doméstico, a propósito de diabetes mellitus, contribuye a documentar políticas públicas orientadas a la atención integral, con equidad, de ésta y otras enfermedades crónicas. Resumen en inglés: "Diabetes mellitus is one of the few illness, different to those related to biological reproduction, that It is more frequent in women than in men. The increasing incidence and the epidemiological characteristics in its distribution make diabetes mellitus a priority concern in public health (Velazquez et al, 2003; Venn and Mann, 2004; Rull et al, 2005; SSA, 20005). Its relevance can be observed among the individuals, the families and institutions in the biological, psychological and social dimensions. Mortality rates show that men are dying faster than women due to diabetes. Even though women seek for health services more frequently, access to health services can not explain the phenomena by itself. Life expectancy is longer for women, but little is known about their quality of life in the years after they have been diagnosed with diabetes. The purpose of this study is to analyze, using the gender focus, the ways that both, women and men confront a chronic illness and take care of themselves in their domestic environment, within a poverty context in a country with health policies oriented towards privatization. Twenty-three in-depth interviews were conducted in adults, both women and men with diabetes living in Tuxtla Gutierrez, Chiapas with the idea to obtain data that will lead us to understand how the self-care is conducted within the domestic space. The data that are analyzed come from in depth interviews done to sixteen women and seven men. The sample comes from a nominal census performed four months before (Nazar, 2005). Through grounded theory, empirical information let to establish the role played by identities and gender, as well as the as well as the access and control to resources (financial and non financial) and supporting social nets. All of them are issues that conform a differentiated group of opportunities and obstacles for women and men with diabetes achieve a successfully self-care of their disease. Evidence supports that in our group men have better opportunities than women to take care of their individual self-care, although restrictions come from their own gender identity. At the end all this has a negative impact on their living conditions. On the other hand, women have more obstacles for their self-care, but they find elements within their domestic environment to carry on with their chronic illness despite of the conflicting relations to resources access and the violence that their partners. A relevant aspect in this study is the analysis of the domestic group from the perspective of gender and the typology that is proposed to understand the way in which the family organization contributes in the opportunities and obstacles that both women and men confront for their self-care. A model is proposed to help understanding the way in which these cultural and economic elements impact over the self-care of women and men with diabetes. Understanding the adult behavior within their domestic group, respect to diabetes mellitus, help to assume public policies oriented towards a holistic attention, with equity for diabetes as well other chronic diseases.
