Ocorrências de câncer que geraram concessões de benefícios no Regime Geral da Previdência Social, 2008 a 2014 | Cancer occurrences that generated benefit concessions in the General Social Security Regime, 2008 to 2014

O câncer concernente ao ambiente de trabalho não tem sido devidamente mensurado no Brasil, mesmo com o conhecimento da existência de associação entre exposições ocupacionais e o desenvolvimento desta doença, o que leva a não implementação de estratégias de prevenção e vigilância do câncer no ambiente de trabalho. O afastamento do trabalho de um indivíduo diagnosticado com câncer e protegido pela Previdência Social gera concessão de benefícios com impacto no sistema previdenciário. O objetivo desta dissertação foi analisar a distribuição de ocorrências de câncer que geraram concessões entre os beneficiários da Previdência Social no Brasil, de 2008 a 2014; com ênfase na região sudeste. Para isso foi realizado um estudo descritivo com dados do INSS, determinando-se a proporção de benefícios concedidos por câncer no Brasil e nos Estados da região sudeste e construindo-se mapas da distribuição espacial das principais neoplasias malignas, tendo os estados do Brasil como unidade de análise. A concessão dos benefícios previdenciários superou a dos acidentários. Na concessão dos benefícios por todas as causas, houve um predomínio de beneficiários do sexo masculino, mas nos benefícios concedidos por câncer, o sexo feminino prevaleceu; exceto para os benefícios acidentários. Observou-se mais benefícios por câncer em idades mais avançadas. A renda dos beneficiados ficou concentrada em até 3 salários mínimos. As maiores proporções de benefícios previdenciários por câncer ocorreram nas regiões Norte e Centro-Oeste; dos benefícios acidentários 19 estados não concederam nenhum deles por câncer. Na região sudeste houve diferença estatisticamente significativa (p<0,05) na distribuição dos benefícios entre os estados; e o ramo de atividade, comerciário, rural e transporte e carga foram os que mais concederam benefícios por câncer. Apesar da maior parte da população protegida pela Previdência Social ser composta por homens, o câncer gera um maior número de benefícios previdenciários nas mulheres. Diferentemente do que se esperava só houve 6 concessões de benefícios por neoplasia no ramo de atividade industrial. Nossos dados demonstram a necessidade de mais estudos sobre o câncer de glândula tireoide e o ramo comerciário na região sudeste do Brasil. Adicionalmente, corroboram para as discussões sobre os impactos econômicos e sociais que o câncer gera na população segurada pela Previdência Social do Brasil.

Cancer related to the work environment has not been properly measured in Brazil, even with the knowledge of the existence of association between occupational exposures and the development of this disease, which leads to the non-implementation of strategies for cancer prevention and surveillance in work environment. The work absence of a cancer individual protected by Social Security generates benefits grant, with impact on the social security system. The objective of this dissertation was to analyze the cancer distribution that generated concessions among Social Security beneficiaries in Brazil, from 2008 to 2014; with emphasis on the southeast region. A descriptive study was carried out with INSS data, determining the proportion of benefits granted by cancer in Brazil, and in the states of the southeast region, and constructing maps of the spatial distribution of the main malignancies, with the Brazilian states as analysis unit. The granting of social security benefits surpassed that of accidents. In the benefits grant for all causes, there was a predominance of male beneficiaries, but in the benefits granted by cancer, the female prevailed; except for accidental benefits. More cancer benefits were seen at more advanced ages. The income of the beneficiaries was concentrated in up to 3 minimum salaries. The highest proportions of social security benefits for cancer occurred in the north and centerwest regions; of the accident benefits 19 states did not award any of them for cancer. In the southeast region there was a statistically significant difference (p <0.05) in the distribution of benefits among the states; and the branch of activity, trader, rural and transport and cargo were the ones that gave the most cancer benefits. Although, most of the Social Security protected population is composed by men, cancer generates a greater number of social security benefits in women. Contrary to what was expected, there were only 6 benefits granted for neoplasia in the industrial activity branch. Our data demonstrate the need for further studies on thyroid gland cancer and the commercial branch in the southeastern region of Brazil. Additionally, they corroborate to the discussions about the economic and social impacts that cancer generates in the Brazilian Social Security insured population.
