Untersuchung der genetischen Grundlage der Albuminurie und chronischen Nierenschädigung bei der Munich-Wistar-Frömter-(MWF)-Ratte | Characterization of the genetic basis of albuminuria and renal damage in the Munich Wistar Frömter (MWF) rat

Das Krankheitsbild der Albuminurie ist polygenetisch determiniert und geht mit renalen Endorganschäden und einem erhöhten kardiovaskulären Risiko beim Menschen einher. Des Weiteren ist die Albuminurie als möglicher Prädiktor von arteriosklerotischen Herz-Kreislauf- Erkrankungen in nicht-diabetischen Individuen bekannt und kann eine generalisierte vaskuläre endotheliale Schädigung reflektieren. Die primäre Hypertonie wiederum erhöht das Risiko, chronische Nierenerkrankungen zu entwickeln. Eine adaptive Reaktion auf erhöhten Blutdruck ist die vermehrte Stickstoffmonoxid (NO) Produktion, die präventiv gegen Endorganschäden vorbeugt. Ein Defizit oder Verlust von NO führt wiederum zu Bluthochdruck und Nierenschäden. Die MWF-Ratte stellt ein genetisches Inzuchtmodell dar, das neben einem angeborenen Nephrondefizit von 30-50%, eine spontane Albuminurie, eine moderate Hypertonie und eine endotheliale Dysfunktion entwickelt. In vorangehenden Studien wurden wichtige Albuminurie- QTL (Quantitative Trait Loci) auf Rattenchromosom (RNO)6 und RNO8 identifiziert. Durch den Transfer von RNO6 und RNO8 aus dem nierengesunden SHR-Stamm in den isogenetischen MWF-Hintergrund konnte die Albuminurie jeweils deutlich gesenkt und die strukturellen Nierenschäden reduziert sowie das Nephrondefizit eliminiert werden. Das Ziel der vorliegenden Arbeit war zu ermitteln, ob mittels eines reziproken Ansatzes QTL auf RNO6 und RNO8 in der Lage sind einen Albuminurie-Phänotyp im isolierten, albuminurieresistenten Hintergrund der SHR-Ratten zu induzieren. Um die Suszeptibilität von Nierenschäden zu untersuchen, wurde zwischen der 6. und 24. Woche (Studie A) eine Gruppe der Parentaltiere MWF und SHR und der konsomen Stämme SHR-6MWF und SHR-8MWF mit normalem Wasser (Kontrolle), eine zweite Gruppe mit 20 mg/l NG- nitro-Larginine methyl ester (L-NAME) behandelt und eine dritte Gruppe wurde unilateral nephrektomiert (Nx). Aufgrund einer hohen Letalität bei parentalen SHR und konsomen SHR-6MWF unter L-NAME-Behandlung vor Erreichen der 24. Woche, wurde die Studie von der 6. - 12. Woche wiederholt (Studie B). In der bis zur 24. Woche geführten Studie A zeigte sich, dass SHR und SHR-6MWF eine ausgeprägte L-NAME Sensitivität mit Todesfolge entwickelten, während MWF und SHR-8MWF die Behandlung mit dem NO-Inhibitor ohne Ausfälle tolerierten. Im Verlauf der Studie A demonstrierten SHR- und SHR-6MWF-Tiere unter L-NAME Behandlung ein stark beeinträchtigtes Allgemeinbefinden das mit torti collis, Manegebewegung und hoher Mortalität zwischen der 12. und 15. Woche einherging. Der Parentalstamm MWF wie auch der konsome Stamm SHR-8MWF zeigten hingegen keine Beeinträchtigung und tolerierten die L-NAME-Behandlung bis zum Studienende in der 24. Woche. MWF entwickelte bis zur 24. Woche in der Kontrollgruppe einen signifikanten progressiven Anstieg der Albuminurie gegenüber den albuminresistenten SHR und den konsomen Stämmen SHR-6MWF und SHR-8MWF. Der konsome Stamm SHR-8MWF zeigte unter Kontrollbedingungen spontan einen signifikanten Anstieg der Albuminurie im Vergleich zu SHR. Die Stämme MWF und SHR-8MWF entwickelten signifikant erhöhte Albuminurie-Werte in der L-NAME Gruppe verglichen mit der Kontrolle in der 24. Woche. In der 12. Woche der Studie B konnten mit Studie A vergleichbare Ergebnisse der Albuminbestimmung generiert werden. SHR und SHR-8MWF zeigten in Studie B erneut nur eine milde Erhöhung der Albuminurie gegenüber der Kontrollgruppe, während SHR-6MWF und MWF eine signifikant höhere Albuminausscheidung gegenüber der Kontrolle demonstrierten. Durch den Transfer von RNO6 erhöhte sich nicht nur die Albuminausscheidung unter L-NAME im Vergleich zur Kontrolle für SHR- 6MWF und SHR, es entwickelte sich zudem eine hohe Variabilität der Einzelwerte im konsomen Stamm SHR-6MWF. Durch den Transfer von RNO8 dagegen erreichte die Kontrollgruppe von SHR-8MWF signifikant höhere Albuminwerte verglichen mit SHR. Desweiteren zeigten die konsomen Stämme SHR-6MWF und SHR-8MWF signifikant mehr tubulointerstitielle Schädigungen unter L-NAME gegenüber der Kontrolle, während SHR und MWF nur milde tubulointerstitielle Schädigungen unter L-NAME Behandlung im Vergleich zur Kontrollgruppe entwickelten. Der systolische Blutdruck war bei SHR, SHR-6MWF und SHR-8MWF signifikant in der 12. Woche unter L-NAME-Behandlung verglichen mit der Kontrolle erhöht. Im MWFStamm konnte kein signifikanter Unterschied der Blutdruckwerte zwischen Kontroll- und L-NAME Gruppe ermittelt werden, jedoch zeigte dieser Stamm in beiden Gruppen signifikant niedrigere Blutdrücke gegenüber den übrigen drei Stämmen. Für SHR-6MWF konnte in der Kontroll- und Nx-Gruppe keine Albuminurie ermittelt werden. In der 18. und 24. Woche wurden dagegen unter Nx signifikant niedrigere Albuminwerte im Vergleich zu SHR bestimmt. Kontrastierend zeigte SHR-8MWF unter Kontroll- wie auch unter Nx-Bedingungen eine spontane Albuminurieentwicklung, die signifikant höher gegenüber SHR war, während der Blutdruck in der Nx-Gruppe jedoch deutlich niedrigere Werte erreichte. Bei der histologischen Beurteilung der Niere wiesen SHR-8MWF- und MWF-Tiere in der Kontrollgruppe eine signifikant erhöhte Glomerulosklerose im Vergleich zu SHR auf, während unter Nx nur MWF signifikant höhere Werte gegenüber SHR demonstrierte. Zusammenfassend zeigen diese Ergebnisse, dass MWF-RNO6 im isolierten SHR Hintergrund nicht in der Lage ist eine Albuminurie unter basalen Bedingungen bzw. unter 50%iger Reduktion der Glomeruli zu induzieren. Nach chronischer NO-Blockade wiesen SHR-Tiere trotz erhöhtem Blutdruck keine Albuminurie auf. Im Gegensatz dazu konnte beim konsomen Stamm SHR-6MWF durch NO-Blockade ein Albuminurie-Phänotyp mit erhöhten Blutdruckwerten ermittelt werden. Diese Ergebnisse lassen neben dem konfirmierten QTL für Albuminurie auf RNO6 einen möglichen zweiten QTL vermuten, der durch die chronische NO- Blockade induziert wurde und unabhängig agiert. Im Gegensatz dazu tolerierten SHR-8MWF und MWF die L-NAME-Behandlung während des gesamten Untersuchungszeitraums. In der vorliegende Arbeit konnte zum ersten Mal gezeigt werden, dass der isolierte Einfluss des QTL auf RNO8 eine spontane Albuminurie und Nierenschäden in einem nierengesunden Hintergrund wie dem des SHR-Stamms unter basaler als auch 50%iger Nephronreduktion induzieren konnte. Eine NO-Inhibition nahm hingegen keinen Einfluss auf den Phänotyp. Die Ergebnisse belegen, dass unter NO-Inhibition ein oder mehrere Gene auf RNO6 zur Entwicklung der Albuminurie führen. RNO8 dagegen agiert völlig unabhängig von genetischen Faktoren des MWF-Hintergrunds und ist in die Entwicklung der Albuminurie und struktureller Nierenschäden im isolierten SHR-Hintergrund involviert. In weiterführenden Studien soll die Pathophysiologie der Albuminurie und der Nierenschäden im MWF-Stamm weiter geklärt werden.

Albuminuria is an important polygenetic quantitative trait indicating renal target organ damage and an increased cardiovascular risk in humans. Furthermore, primary hypertension is a risk factor for the development of albuminuria and chronic kidney disease. Elevated blood pressure values may lead to a compensatory increase in nitric oxide (NO) production, which protects against end organ damages. In contrast, a reduction in NO availability leads to higher blood pressure and kidney damage. The Munich Wistar Frömter (MWF) rat is an inbred genetic model that develops spontaneous and progressive albuminuria, moderate hypertension, a congenital nephron deficit of 30- 50%, and endothelial dysfunction. Previous studies showed that in MWF rats albuminuria development is determined by an interaction between multiple quantitative trait loci (QTL). Replacement of two major loci on rat chromosome 6 (RNO6) or 8 (RNO8) leads to a marked suppression of early onset albuminuria in these animals. The aim of this study was to test whether QTL on RNO6 and RNO8 are capable of inducing the albuminuria phenotype in albuminuria-resistant SHR rats after isolation from the permissive disease background in MWF. In order to analyse the susceptibility to renal damage, one set of parental MWF and SHR and the consomic strains SHR-6MWF and SHR-8MWF was treated with normal tap water (Sham), while a second set was treated with 20 mg/l NG-nitro-L-arginine methyl ester (L-NAME) between 6 and 24 weeks of age, respectively, while a third set was unilaterally nephrectomised (Nx) (study A). Because of premature deaths of parental SHR and consomic SHR-6MWF under L-NAME treatment before the scheduled end of the observation period at week 24, the study was repeated between 6 and 12 weeks of age (study B). In response to L-NAME until week 24 (study A) the overall condition of SHR and SHR-6MWF rats were markedly impaired and they demonstrated premature death between week 12 and 15. In contrast, MWF and SHR-8MWF rats tolerated the L-NAME treatment between 6 and 24 weeks of age. Sham treated MWF developed a significant increase in UAE up to week 24 compared with albuminuria-resistant SHR and consomic SHR-6MWF and SHR-8MWF. In response to L-NAME MWF exhibited mean UAE levels that were significantly higher compared with Sham. Consomic SHR-8MWF animals demonstrated a significant increase of albuminuria in the Sham group compared to SHR Sham. Under L-NAME treatment UAE in MWF and SHR-8 MWF increased significantly compared to Sham in week 24. At 12 weeks of age (study B), UAE levels were comparable with the data obtained in study A. In study B, UAE under L-NAME treatment in SHR and SHR-8MWF were still low and only slightly higher compared with Sham, while SHR-6MWF and MWF exhibited significant higher UAE levels compared to Sham. Transfer of RNO6 resulted in significantly higher UAE values demonstrating a large variability in SHR-6MWF L-NAME compared with SHR-6MWF Sham and L-NAME-treated SHR. Transfer of RNO8 resulted in a significant higher mean UAE level in the Sham group of SHR-8MWF compared with SHR Sham. Furthermore, SHR-6MWF and SHR-8MWF showed significantly increased tubulo-interstitial damage in the L-NAME group compared with Sham, while SHR and MWF only exhibited low or no change of tubulointerstitial damage under L-NAME treatment compared to Sham. Systolic blood pressure (SBP) revealed a significant increase of SBP at 12 weeks of age in L-NAME treated SHR, SHR-6MWF and SHR-8MWF compared to Sham. MWF under L-NAME treatment showed no significant changes in blood pressure vales compared to Sham, but significantly lower SBP compared with the other strains in both the Sham and the L-NAME group. SHR-6MWF exhibited no albuminuria under Sham and Nx conditions. However, UAE was significantly lower in week 18 and 24 of age compared to SHR in the Nx group. In contrast, SHR-8MWF developed a significant increase in albuminuria under Sham and Nx conditions, with significantly higher values compared to SHR. In contrast, SBP of SHR-8MWF Nx was markedly lower. SHR-8MWF and MWF showed a significantly increased glomerular damage index compared to SHR in the Sham treated group, while under Nx only MWF exhibited significant glomerular damages compared to SHR. Taken together the results demonstrate that isolation of the RNO6 QTL by transfer from MWF into the SHR background is not capable of inducing an albuminuria phenotype during normal conditions or after 50% nephron reduction. After NO inhibition, normal SHR rats showed no albuminuria, although SBP was increased in these animals. In contrast, in response to NO inhibition consomic SHR-6MWF exhibited an albuminuria phenotype. Thus, transfer of MWF-RNO6 into SHR influences albuminuria and blood pressure development during NO inhibition. This suggests that these genetic mechanisms are probably different from the QTL responsible for the induction of albuminuria identified on RNO6 in the MWF strain. In contrast, SHR-8MWF and MWF animals tolerated treatment with L-NAME during the entire observation period. Moreover, this study shows for the first time the induction of spontaneous albuminuria and renal damage by transfer of albuminuria QTL on RNO8 into a resistant recipient background in a consomic rat strain under both normal conditions and 50% nephron reduction, but not under NO inhibition. These findings demonstrate that one or more genes on RNO6 contribute to the development of albuminuria during NO inhibition. Moreover, genes on RNO8 act independently of other genetic factors and were involved in the pathophysiology of both albuminuria and structural kidney damage in the SHR background. Further studies should elucidate the pathophysiology of albuminuria and renal damage in the MWF rat strain.
