Epidemiologische Fallstudie zur Bovinen Neonatalen Panzytopenie | Epidemiological case study on Bovine Neonatal Pancytopenia

German. Die Bovine Neonatale Panzytopenie (BNP) ist eine Krankheit junger Kälber, die erstmalig in Süddeutschland im Jahr 2006 (Anonym 2011, Friedrich et al. 2008) beschrieben wurde. Charakteristisch für das klinische Bild der BNP ist die hämorrhagische Diathese mit Blutaustritt aus der Haut ohne augenscheinliche Verletzungen. Als Ursache für die BNP wird ein alloimmunes Krankheitsgeschehen angesehen, welches durch die Impfung von Kühen mit einem neuartigen BVD-Impfstoff (PregSure® BVD (Pfizer GmbH, Berlin, Deutschland) verursacht wurde. Anders als bei anderen Impfstoffen wurde hier eine Rinderzelllinie in Kombination mit einem neuartigen, hochwirksamen Adjuvans zur Herstellung des Impfstoffes verwendet, sodass geimpfte Tiere Antikörper gegen diese Zellen ausbildeten. Sind gewisse Voraussetzungen erfüllt, kann das dazu führen, dass Kälber mit dem Kolostrum Antikörper aufnehmen, die gegen vom Vater ererbte Zellstrukturen gerichtet sind, was zu dem oben genannten Krankheitsbild führt. Ziel der vorliegenden Studie war es, den Einfluss ausgewählter Faktoren (z.B. Management, Jahreszeit) auf die Entstehung von BNP in Kälbern zu untersuchen. Das Kolostrummanagement, die Anzahl der Kalbungen, die Impfungen bzw. das Impfregime und die Totgeburten der Muttertiere, die jahreszeitliche Verteilung und der genetische Hintergrund der BNP-Fälle wurden bei Rindern der Rasse Deutsch Holstein detailliert untersucht. Zu diesem Zweck wurden epidemiologische Untersuchungen in Form einer retrospektiven Kohortenstudie (Juli 2007 - Juli 2010) in zwei Milchviehbetrieben (AE1; AE2) einer Agrargenossenschaft in Brandenburg mit einem hohen Vorkommen von BNP-Fällen durchgeführt. In dem Studienzeitraum wurden 954 Kälber in der AE1 und 1260 Kälber in der AE2 geboren. 40 von den insgesamt 2214 Kälbern erkrankten an BNP. Die Forschungsergebnisse der Studie zeigten, dass mehrere Faktoren die Chance der Entstehung von BNP erhöhen. Dazu gehörten die Verfütterung von Mischkolostrum (OR = 2,49), die Anzahl der Kalbungen, die Anzahl der Wiederholungsimpfungen und die Jahreszeit. Signifikant häufiger gebaren Muttertiere, die in der zweiten Trächtigkeitshälfte (Monate 5 bis 9) geimpft worden waren, folgend Kälber mit BNP (Fisher Test, p < 0,01). In dem untersuchten Betrieb wurden signifikant mehr Fälle in den Sommermonaten ermittelt als in den Wintermonaten, was auf die Anwesenheit von stechenden Insekten zurückgeführt wurde, welche zu sichtbaren Blutungen der Haut beitrug (Fisher Test, p < 0,001). Dies entspricht den Erkenntnissen anderer Forschungsgruppen (Defra 2011, Friedrich et al. 2009a, Klemt et al. 2010, Pardon et al. 2009a). Die Hypothese, dass die Impfung gegen die Blauzungenkrankheit (BTV) einen Einfluss auf die Entstehung von BNP-Kälbern hat, konnte nicht bestätigt werden. Ein weiterer Forschungsgegenstand der Arbeit war die Abstammung der Kälber. Die Ergebnisse zeigen, dass die Chance für Kälber, an BNP zu erkranken, signifikant erhöht war, wenn sie von einzelnen Bullen abstammten (Fisher Test, p ≤ 0,05). Die Beobachtungen der vorliegenden Arbeit unterstützen die Forschungsergebnisse von Benedictus et al. (2015), wonach Antikörper gerichtet gegen Moleküle des MHC-Klasse I Komplexes aus dem Kolostrum an Stammzellen des Kalbes binden und diese zerstören. Die Vermehrung von Impfviren auf homologen Zellkulturen stellt laut Benedictus et al. (2015) ein Risiko dar, das vermieden werden sollte. Darüber hinaus wird empfohlen, das Kolostrum von Müttern, die Kälber mit BNP geboren haben, zu verwerfen. Gleiches gilt für die weit verbreitete Tränkung von Mischkolostrum, die unterbleiben sollte.

English. Bovine Neonatal Pancytopenia (BNP) is a disease of neonatal calves, which has been described in Southern Germany in 2006 for the first time ever (Anonym 2011, Friedrich et al. 2008). Characteristic for the clinical picture of BNP is the haemorrhagic diathesis with blood leaking from the skin without evident trauma. BNP is caused by an alloimmune disorder following the vaccination with a novel BVD vaccine (PregSure® BVD, Pfizer GmbH, Berlin, Germany). Differently to other vaccines a bovine cell line in combination with a novel, highly efficient adjuvant was used hereby for the production of the vaccine, inducing the generation of antibodies against these cells in vaccinated animals. If certain conditions are met in calves, this may lead to the intake of colostrum that contains antibodies, which are directed against paternal cell structures, resulting in the disease pattern mentioned above. Objective of the present study was to investigate the impact of selected factors (e.g. management, season) on the occurrence of BNP. The colostrum management, the number of calvings, the vaccinations, respectively the vaccine management and the stillbirth of the dams, the seasonal distribution and the genetic background of BNP cases were studied in-depth in Deutsch Holstein cattle. For this purpose epidemiological analysis in form of a retrospective cohort study (July 2007 – July 2010) was undertaken in two dairy cattle herds (AE1; AE2) of an agricultural cooperative in Brandenburg, Germany with a high incidence of BNP cases. During the study period 954 calves were born in AE1 and 1260 calves in AE2. 40 of a total of 2214 calves fell ill with BNP. The results of the present study revealed that multiple factors enhance the chance of the development of BNP. Counted among those factors were the feeding of mixed colostrum (OR = 2.49), the number of calvings, the number of booster vaccinations and the season. Dams that were vaccinated in second half of the gestation (months 5 to 9), had subsequently born calves with BNP significantly more often (Fisher Test, p < 0.01). On the farm studied there were significantly more cases detected during the summer months than during the winter months, which could be traced back to the presence of biting insects that induced visible bleedings of the skin (Fisher Test, p < 0.001). These findings correlate with conclusions of other research groups (Defra 2011, Friedrich et al. 2009a, Klemt et al. 2010, Pardon et al. 2009a). The hypothesis of the vaccination against the Bluetongue disease virus (BTV) having an impact on the occurrence of BNP calves could not be confirmed. A further objective of research was the pedigree of the calves. The results show that the chance of calves to contract BNP was significantly increased by descending from individual sires (Fisher Test, p ≤ 0.05). The observations of the present dissertation confirm the results of research of Benedictus et al. (2015) who proposed that antibodies are directed against molecules of the MHC class I complex of the colostrum and bind to stem cells of the calf destroying those cells. According to Benedictus et al. (2015), the propagation of vaccination viruses on homologous cell cultures displays a risk that should be avoided. Beyond that it is recommended to discard the colostrum of dams that gave birth to a calf with BNP. The same recommendation applies for the wide-spread custom of feeding mixed colostrum, which should be omitted.
