Kann für Formaldehyd eine "sichere" Konzentration abgeleitet werden? - Analyse der Daten zur krebserzeugenden Wirkung

Nach inhalativer, oraler oder dermaler Exposition werden die physiologischen Formaldehyd-Konzentrationen (0,1 mM) durch Zufuhr von externem Formaldehyd aufgrund seiner ausgeprägten Reaktivität mit dem Gewebe und eines extensiven lokalen Metabolismus nicht erhöht. Bei inhalativer Aufnahme bewirkt Formaldehyd in den Geweben des ersten Kontakts lokale Effekte. Er erzeugt gentotoxische Effekte als prämutagene Ereignisse, die im Konzentrationsbereich bis zu 2 ppm eine lineare, mit geringer Steigung ansteigende Dosis- Wirkungsbeziehung besitzen. Bei höherer Konzentration erfolgt ein nichtlinearer Anstieg mit zunehmender Zytotoxizität. Chronische Studien bei der Ratte ergaben erhöhte Tumorbildung ab Konzentrationen von 6 ppm. Ergebnisse neuer epidemiologischer Studien unterstützen die Annahme einer Kausalbeziehung zwischen inhalativer Exposition gegenüber Formaldehyd und Krebs im Nasopharynx. Positive statistische Trendtests für bestimmte Expositionsmatrices zeigen eine Konzentrationsabhängigkeit der Tumor bedingten Todesfälle. Die formaldehydinduzierte Tumorentstehung benötigt zwei parallele Mechanismen: Genotoxizität und Zytotoxizität. Jedoch spielen Expositionsbedingungen, die zu einer regenerativen Zellproliferation im Nasopharynx führen, die bedeutendste Rolle für die Tumorenstehung. Für die Ableitung einer Expositionskonzentration, die als sicher im Hinblick auf die Auslösung von Tumoren im Nasopharynx gelten kann, sehen wir irritative Effekte als den empfindlichsten Parameter an, der mit der Tumorentstehung korreliert ist. Aus einer Reihe von Kurzzeit- und Langzeitstudien können 0,2 bis 0,3 ppm als gering wirksame Konzentration und 0,1 ppm als NOAEL abgeleitet werden. Chronic studies in rats show increased tumor rates from concentrations at and higher than 6 ppm. Results from recent epidemiological studies support a possible causal relationship between inhalation exposure to formaldehyde and nasopharyngeal cancer. Formaldehyde induced tumor induction requires concomitant occurrence of genotoxic and cytotoxic events but definite exposure conditions and concentrations at which cytotoxicity promotes sustained regenerative cell-proliferation in the respiratory epithelium of the nasopharynx play the most important role. A slight sensory irritation response can be observed at concentrations of 0.2 - 0.3 ppm and ocular and upper respiratory tract sensory irritation is not present below 0.1 ppm. Thus the concentration of 0.1 ppm is proposed as a Safe Level. Since 0.1 ppm is more than 10 times below the threshold level observed for cytotoxic damage in the nasal mucosa, this level seems sufficiently low to eliminate a reasonable chance of causing adverse morphological effects within the target tissue. Hence, we conclude that the level of 0.1 ppm seems to be suited to protect the whole population with regard to the carcinogenicity of formaldehyde.
