Granuloma leproide canino em animais de Caxias do Sul : relato de caso

As micobactérias são causas incomuns de doenças de pele em cães e gatos. Em geral, as infecções micobacterianas em cães apresentam-se como granuloma leproide canino (GLC) e tuberculose cutânea. O GLC é uma dermatose nodular que pode acometer as regiões cutânea e subcutânea, geralmente localizadas nas orelhas. Assim, este trabalho tem por objetivo relatar dois casos de GLC em caninos. Foram atendidos na Clínica Veterinária de Pequenos Animais da Universidade de Caxias do Sul (CVet - UCS) dois cães com histórico clínico de lesão na face externa da orelha. Ambos frequentavam as dependências da CVet-UCS, e possuíam o hábito de interagir com os gatos que residiam no mesmo ambiente. O primeiro caso foi de um canino, fêmea, castrada, sem raça definida (SRD), com oito anos, pesando 25 kg. Realizou-se biópsia incisional para exame histopatológico, sendo observada acentuada quantidade de bacilos álcool-ácido resistentes no interior de macrófagos e livres em meio ao infiltrado através da coloração histoquímica de Ziehl-Neelsen, determinando o diagnóstico sugestivo de GLC causado por Mycobacterium sp. Institui-se tratamento com doxiciclina durante seis meses; meloxicam e dipirona durante quatro dias. Após o tratamento, o animal apresentou cura clínica e boa cicatrização. O segundo caso foi referente a um canino, macho, castrado, SRD, com aproximadamente 10 anos, pesando 8 kg. Foi realizado biópsia e a amostra coletada foi enviada para exame histopatológico, sendo observado através da coloração histoquímica de Ziehl-Neelsen acentuada quantidade de bacilos álcool-ácido resistentes compatíveis com Mycobacterium sp. no citoplasma de macrófagos. Outra porção da amostra foi enviada para identificação molecular por sequenciamento do agente envolvido, sendo identificado Mycobacterium sp. Optou-se por não realizar tratamento com antimicrobiano prolongado para GLC, devido a característica benigna da infecção e também pela prévia completa remoção do nódulo. Duas semanas após a realização do procedimento o animal apresentou melhora clínica e não haviam mais lesões no local. Através destes dois casos conclui-se que o GLC deve ser considerado como importante diagnóstico diferencial das lesões nodulares no pavilhão auricular que afetam cães na região de Caxias do Sul, principalmente aqueles que vivem contato com o meio externo. Além disso, enfatiza-se a utilização de exames complementares, empregando a coloração histoquímica de Ziehl-Neelsen e identificação molecular do agente, para a confirmação do diagnóstico.

Mycobacteria are uncommon causes of skin disorders in dogs and cats. In general, mycobacterial infections in dogs present as canine leproid granuloma (CLG) and cutaneous tuberculosis. CLG is a nodular dermatosis that can affect the cutaneous and subcutaneous regions, usually located in the ears. Thus, this work aims to report two cases of CLG. Two dogs with a clinical history of lesions on the outer surface of the ear were treated at the Veterinary Clinic for Small Animals at the University of Caxias do Sul (CVet - UCS). Both frequented the CVet-UCS facilities, interacting with cats that lived in the same environment. The first case was a canine, female, spayed, mixed breed, eight years old, weighing 25 kg. An incisional biopsy was performed for histopathological examination, and a marked amount of acid-alcohol resistant bacilli was observed inside macrophages and free in the middle of the infiltrate through Ziehl-neelsen histochemical staining, suggesting CLG caused by Mycobacterium sp. Treatment with doxycycline was instituted for six months; meloxicam and dipyrone for four days. After treatment, the animal presented clinical cure and good healing. The second case concerned a canine, male, neutered, mixed breed, approximately 10 years old, weighing 8 kg. A biopsy was performed and tissue submitted to histopathological examination through Ziehl-Neelsen histochemical staining, which indicated a marked amount of alcohol-acid resistant bacilli in the cytoplasm of macrophages, compatible with Mycobacterium sp. Another portion of the tissue sample was sent for molecular identification of the agent involved, being identified Mycobacterium sp. In this case, a prolonged antimicrobial treatment for CLG was excluded, due to the benign nature of the infection and the complete removal of the nodule. Two weeks after the procedure, the animal showed clinical improvement and there were no more lesions at the site. Through these two cases it is concluded that the GLC should be considered as an important differential diagnosis of nodular lesions in the ear pinna that affect dogs in the region of Caxias do Sul, especially those that have contact with the external environment. In addition, the use of complementary tests is emphasized, using Ziehl-Neelsen histochemical staining to confirm the diagnosis.
