Surgical resolution and evolution of megaesophagus due to persistence of fourth right aortic arch in a dog | Resolución quirúrgica y evolución de megaesófago por cuarto arco aórtico derecho persistente en un perro

English. We reported a case of megaesophagus for the fourth persistent right aortic arch in a puppy Cocker. The patient had a regular general condition, signs of regurgitation after eating, voracious appetite, weight loss and growth retardation. Simple and contrasted chest radiographic study revealed a distended esophagus frankly in his cervical and thoracic portions, cranial to the base of the heart, indicating a presumptive diagnosis of persistent right fourth aortic arch. We proceeded to surgical resolution for left intercostal thoracotomy with ligation of the ductus arteriosus, esophagus adhesion on base of the heart is freed and a chest drain was left for two days. The outcome was not as expected and the first 30 days of surgery were operated for the second time by thoracotomy with resection of the fifth rib, being adhesions of the ventral portion of the sternebrae megaesophagus. Flanges were released to proceed to posterior longitudinal elliptical removal of the entire ventral portion of esophageal dilation and sutured with simple points nonpenetrating. A chest drain was left for four days and a gastrostomy tube for fifteen days. The evolution was satisfactory with a medical release at four postoperative months.

Spanish; Castilian. Se reporta un caso de megaesófago secundario por cuarto arco aórtico derecho persistente en un cachorro Cocker. El paciente presentó un estado general regular, signos de regurgitación pos ingesta, apetito voraz, disminución de peso y retraso en el crecimiento. El estudio radiográfico simple y contrastado de tórax reveló un esófago francamente distendido en sus porciones cervical y torácica, craneal a la base del corazón. Se procedió a la resolución quirúrgica por toracotomía intercostal izquierda con ligadura del conducto arterioso, se liberó el esófago de adherencias sobre base del corazón y se dejó un drenaje torácico durante dos días. La evolución no fue la esperada y a los 30 días de la primera cirugía se intervino por segunda vez mediante toracotomía con resección de la quinta costilla, encontrándose adherencias de la porción ventral del megaesófago a las esternebras. Las bridas se liberaron para proceder a la posterior resección longitudinal elíptica de todo el receso ventral de la dilatación esofágica y se suturó en un plano a puntos simples no perforante. Se dejó un drenaje torácico durante cuatro días y una sonda por gastrostomía durante quince días. La evolución fue satisfactoria, con un alta médica a los cuatro meses posquirúrgicos.
