Preliminary study of the Hip Dysplasia incidence based on Clinical and radiographical examination in Large Breed Dogs Referred to Veterinary Teaching hospital of Ferdowsi University of Mashhad

Background: Hip dysplasia (HD) is an inherited, non-congenital disease that is particularly prevalent in large and giant breeds of dogs. The vast majority of dogs afflicted with HD show no clinical signs and it can be a highly debilitating condition for dogs. Objective: Aim of this study is a preliminary study on hip dysplasia based on clinical and radiographical examination in referral large breed dogs referred to veterinary teaching hospital of Ferdowsi University of Mashhad. Methods: Thirty six large breed dogs were examined for HD using Standard ventrodorsal hip extended view, and graded into five categories (A, B, C, D and E) using the FCI scoring system. PennHIP method was performed for measuring Distraction Index (DI). The Ortolani method was done to evaluate hip joint laxity. Results: HD was found in 72.2% of the dog, which included 11.1% unilateral and 61.1% bilateral. 73% of the afflicting cases were male. HD was reported In 11 dogs among 14 dogs with clinical signs and among 22 dogs without clinical signs 15 dogs were affected by HD. Ortolani test represented false negative in 33 cases and false positive in 6 cases. Conclusion: This study revealed the radiological evaluation of hip joint is essential in susceptible breeds for diagnosis and screening program of HD

Assessment of luteinizing hormone receptor expression in structural support tissues of canine hip and femorotibial joints

OBJECTIVE To determine whether luteinizing hormone receptors (LHRs) are expressed in canine femoral head subchondral bone (FHSB), hip joint round ligament (RL), cranial cruciate ligament (CCL), and femorotibial joint synovium (FJS) specimens. SAMPLE 1 specimen each of the FHSB, RL, CCL, and FJS obtained from the left hind limbs of 19 fresh canine cadavers. PROCEDURES 1 section of each FHSB, RL, CCL, and FJS specimen was processed with rabbit polyclonal IgG anti-human LHR antibody, and 1 section was treated with negative control reagents. Percentage immunoexpression of LHRs in FHSB and FJS sections was analyzed by assessment of 100 bone marrow cells or synoviocytes in 3 adjacent hpf (400×). In each RL and CCL section, immunoexpression of LHRs in fibrocytes was semiquantitatively analyzed on the basis of the mean of the product of percentage staining score (from 0 [no staining] to 3 [> 50% of cells stained]) and staining intensity score (from 0 [no staining] to 2 [moderate to strong staining]) for 3 adjacent hpf. RESULTS All tissues examined had variable LHR expression. Expression of LHRs in FHSB, CCL, or FJS specimens did not differ between sexes or between sexually intact and gonadectomized dogs. However, RL specimens from female dogs had significantly greater LHR expression scores, compared with findings for male dogs. CONCLUSIONS AND CLINICAL RELEVANCE Results indicated that LHRs are expressed in structural support tissues of canine hip and femorotibial joints. Further research is required to determine the LHRs' function, mechanism of action, and potential contribution to the pathogenesis of hip dysplasia or CCL rupture in dogs.

OBJECTIVE To evaluate the 3-D geometry of canine pelves and to characterize the long-term effects of juvenile pubic symphysiodesis (JPS) on pelvic geometry by comparing the pelvic configuration between littermates that did and did not undergo the procedure. ANIMALS 24 Labrador Retriever, Golden Retriever, or Labrador Retriever–Golden Retriever crossbred service dogs from 13 litters. PROCEDURES At 16 weeks old, puppies with a hip joint distraction index ≥ 0.5 were randomly assigned to undergo thermal JPS (n = 9), mechanical JPS (7), or a sham (control) surgical procedure (8). Ten years later, each dog underwent a CT scan of the pelvic region. Modeling software was used to create 3-D reconstructions from the CT scans, and various pelvic measurements were made and compared among the 3 treatments. RESULTS Compared with the control treatment, thermal and mechanical JPS increased the hemipelvis acetabular angle by 4°, the acetabular angle of lateral opening by 5°, and the orientation of the medial acetabular wall in a transverse plane by 6°, which indicated that JPS increased dorsal femoral head coverage by the acetabulum. Both JPS procedures decreased the pelvic canal area by approximately 20% and acetabular inclination by 6° but did not alter acetabular retroversion. CONCLUSIONS AND CLINICAL RELEVANCE Results indicated that thermal and mechanical JPS were equally effective in altering the 3-D pelvic geometry of dogs. These findings may help guide future studies of alternatives for optimizing canine pelvic anatomy to minimize the risk of hip dysplasia and associated osteoarthritis.

Objective—To assess relationships of acetabular volume (AV), femoral head volume (FV), and portion of the femoral head within in the acetabulum (FVIA) with each other and with degrees of hip joint laxity and degenerative joint disease from youth to maturity in dogs predisposed to developing hip joint osteoarthritis (OA). Animals—46 mixed-breed half- or full-sibling hound-type dogs. Procedures—The distraction index (DI), AV, FV, FVIA, and degree of osteoarthritis (OA score) were quantified in 1 hip joint at 16, 32, and 104 weeks of age. Relationships among variables were evaluated within and between ages. Ratios corresponding to OA scores were compared within ages. Differences among 16-week ratios corresponding to 32-week OA scores and among 16- and 32-week ratios corresponding to 104-week OA scores were evaluated. Results—Significant positive relationships existed between FV and AV across ages as well as between FVIA/FV and FVIA/AV and between DI and OA score across and within most ages. Such relationships also existed within these variables across most ages. Negative relationships of DI and OA scores with FVIA/FV and FVIA/AV within and among all ages were significant. Sixteen-week AVs, FVs, and FVIAs were greater and FV/AVs and OA scores were less than 32- and 104-week values. The 32-week FVIA/FV was less than 16- and 104-week values, and the 32-week FVIA/AV was less than the 104-week value. The FVIA/FV and FVIA/AV were lower and the DI was higher with higher OA scores within and among most ages. Conclusions and Clinical Relevance—Structural volumes in lax canine hip joints changed predictably relative to each other during growth, despite degenerative changes. Measures developed in this study may augment current diagnosis and treatment strategies for hip dysplasia in dogs.

Objective-To determine the ratio of ventral-to-dorsal transverse diameters between the wings of the sacrum on ventrodorsal radiographic views of the pelvis in large dogs and to validate the reliability of this morphometric analysis for functional interpretation. Sample Population-Pelvic specimens from 40 large-breed dogs and radiographs of 113 large-breed dogs. Procedure-In an anatomic and radiographic evaluation, the transverse dorsal diameter (TVDS) and transverse ventral diameter (TVV) between the wings of the sacrum were evaluated in sacrum specimens and on corresponding radiographs of the pelvis and sacrum. The ratio between TVV and TVDS (VD ratio) was calculated. Intraobserver reliability was determined by calculation of the coefficient of variation. In a retrospective radiographic evaluation, the VD ratio was determined in Rottweilers, Golden Retrievers, and German Shepherd Dogs. Correlations between VD ratio and breed, age, and sex were tested. Results-The VD ratio was significantly higher in Rottweilers than in Golden Retrievers and German Shepherd Dogs, denoting an oblique alignment of the sacral wings in Rottweilers (ie, the dorsal aspects of the sacral wings were located more medially than the ventral aspects) and an almost sagittal alignment in the other breeds. The VD ratio was significantly associated with age but not with sex. Conclusion and Clinical Relevance-Sagittal alignment of the wings of the sacrum is considered to be biomechanically less efficient. These results provide a basis for further studies to evaluate radiographic assessment of the sacroiliac joints similar to the evaluation for hip dysplasia.

OBJECTIVE To evaluate the effects of selective hip joint denervation on gait abnormalities and signs of hip joint pain in dogs. ANIMALS 6 healthy adult hound-type dogs. PROCEDURES Minimally invasive denervation was performed on the right hip joint of each dog. Two weeks later, sodium urate was injected into the right hip joint to induce synovitis. Dogs were evaluated clinically and by use of instrumented gait analysis before and 2 weeks after minimally invasive denervation and 4, 8, and 24 hours after induction of synovitis. Dogs were euthanized, and necropsy and histologic examination were performed. RESULTS No kinetic or kinematic gait modifications were detected 2 weeks after minimally invasive denervation. Denervation did not eliminate signs of pain and lameness associated with sodium urate–induced synovitis. Results of histologic examination confirmed that denervation was an effective method for transecting the innervation of the craniolateral and caudolateral aspects of the hip joint capsule. CONCLUSIONS AND CLINICAL RELEVANCE In this study, minimally invasive denervation did not result in gait modifications in dogs. Denervation did not abolish the signs of pain and lameness associated with generalized induced synovitis of the hip joint. Further studies are required before conclusions can be drawn regarding the clinical usefulness of hip joint denervation for dogs with hip dysplasia.

Hips of the Golden and Labrador retrievers in Korea were evaluated with the University of Pennsylvania Hip Improvement Program (PennHIP) and the severity of joint laxity and degenerative joint disease (DJD) were compared to the PennHIP database. The distraction index (DI) of domestic Golden and Labrador retrievers was significantly higher than the DI of the PennHIP database. In the two breeds, the Prevalence of DJD increased according to the DI. However, the severity of DJD did not show a positive correlation with the DI. Overweight dogs and dogs kept indoors showed more severe DJD and more prevalence of clinical signs. This report is thought to be the first case presentation of a large population of Golden and Labrador retrievers in Korea and the findings are representing the overall level of canine hip dysplasia (CHD) of domestic Retrievers, as the PennHIP method was not available in Korea until 2001. We can respect that the stock of retrievers can have a tighter hip joint through control of CHD using an accurate diagnostic method keyed to a phenotype especially concern for joint laxity using PennHIP method and an organized screening program. The clinical manifestation of dysplastic dog can be reduced through control of bodyweight and the environment with regular monitoring the hips with concern for joint laxity using PennHIP method.

A study was done to determine whether radiographic-distraction measurement of coxofemoral joint (hip) laxity at 4 and 8 months of age can serve as a predictor of hip dysplasia in older Labrador Retrievers. The method of Smith, Biery, and Gregor was used for radiologic examination of hips and for evaluation of radiographs. Mean (+/- SEM) distraction laxity (ie, distraction index) for 10 adult disease-free dogs was 0.29 +/- 0.05, whereas a group of 8 dogs with dysplastic hips had mean distraction index of 0.60 +/- 0.10 (P < 0.05). Mean distraction index at 4 months of age for 11 pups of 4 litters from matings between dogs with normal hips was 0.39 +/- 0.07, and was 0.54 +/- 0.04 for 31 pups of 7 litters from matings between dogs with hip dysplasia. The distraction index and, thus, joint laxity at that age was significantly (P = 0.0351) different for the 2 groups. The distraction index at 4 months correlated positively with the distraction index at a later age at necropsy (r = 0.43; P = 0.0289). Distraction index < 0.4 at 4 months of age predicted normal hips in 88% of cases and distraction index > 0.4 predicted hip dysplasia in 57% of the dogs. Logistic regression modeling indicated that the odds of a hip being normal decreased with increasing distraction index, and thus, with increasing joint laxity. The logistic regression models provided a reasonable mathematical description of the data. Based on the logistic model of the data, distraction indexes between 0.4 and 0.7 at either 4 or 8 months of age were not associated strongly enough with evidence of disease to be clinically reliable in predicting, on an individual basis, the outcome for dysplastic hip conformation when dogs were older. Index > 0.7 was associated with high probability for developing dysplastic joints and distraction index < 0.4 predicted normal hips with high probability.

Effects of increased dietary chloride and reduced sodium and potassium ion concentrations on coxofemoral joint conformation, as assessed by radiography, were examined in growing dogs. Dietary electrolyte balance was quantified by dietary anion gap (DAG), defined as Na+ + K+ - Cl- in milliequivalents per 100 g of food. Diets had anion gap ranging from 8 to 41 mEq/100 g of food. One hundred sixty-seven pups from 27 litters representing 5 breeds were studied during the period of rapid growth. The extent of subluxation of the femoral head was measured on radiographs, using the method of Norberg. On average, less subluxation of the femoral head (P < 0.05) was observed when diets with lower DAG were fed. Differences in DAG balance did not result in different rates of weight gain; therefore, the reduction in coxofemoral joint subluxation attributable to low DAG was unrelated to weight gain. Norberg angles measured at 30 weeks of age were highly correlated with coxofemoral joint status at 2 years of age, as measured by the Swedish diagnostic system and the scoring system of the Orthopedic Foundation for Animals (/r/ greater than or equal to 0.70, P < 0.0002, n = 24). This diet-related improvement in coxofemoral joint sub-luxation would be expected, on average, to delay or mitigate the characteristic clinical and radiographic signs of hip dysplasia in growing dogs.
