Evaluation of the diagnostic yield of dental radiography and cone-beam computed tomography for the identification of dental disorders in small to medium-sized brachycephalic dogs

OBJECTIVE To evaluate the diagnostic yield of dental radiography (Rad method) and cone-beam CT (CBCT) methods for the identification of 31 predefined dental disorders in brachycephalic dogs. ANIMALS 19 client-owned brachycephalic dogs admitted for evaluation and treatment of dental disease. PROCEDURES 31 predefined dental disorders were evaluated separately and scored by use of dental radiography and 3 CBCT software modules (serial CBCT slices and custom cross sections, tridimensional rendering, and reconstructed panoramic views). A qualitative scoring system was used. Dental disorders were grouped into 10 categories for statistical analysis. Point of reference for presence or absence of a dental disorder was determined as the method that could be used to clearly identify the disorder as being present. Accuracy, sensitivity, specificity, and positive and negative predictive values were calculated with the McNemar χ2 test of marginal homogeneity of paired data. RESULTS When all 3 CBCT methods were used in combination, the diagnostic yield of CBCT was significantly higher than that of dental radiography for 4 of 10 categories (abnormal eruption, abnormally shaped roots, periodontitis, and tooth resorption) and higher, although not significantly so, for all categories, except for 1 (loss of tooth integrity). CONCLUSIONS AND CLINICAL RELEVANCE CBCT provided more detailed information than did dental radiography. Therefore, CBCT would be better suited for use in diagnosing dental disorders in brachycephalic dogs.

OBJECTIVE To determine the onset, duration, and extent of regional nerve blocks performed by administration of lidocaine or lidocaine-bupivacaine into the infraorbital canal in dogs. ANIMALS 6 healthy hound-type dogs. PROCEDURES Under general anesthesia, stimulating needles were inserted into the gingiva dorsolateral to both maxillary canine (MC) teeth and the maxillary fourth premolar (MPM4) and second molar (MM2) teeth on the treatment side. A reflex-evoked muscle potential (REMP) was recorded from the digastricus muscle after noxious electrical stimulation at each site. After baseline measurements, 1 mL of 2% lidocaine solution or a 2% lidocaine-0.5% bupivacaine mixture (0.5 mL each) was injected into the infraorbital canal (at approx two-thirds of the canal length measured rostrocaudally). The REMPs were recorded for up to 7 hours. The REMP data for the contralateral (untreated control) canine tooth were used to normalize results for all stimulation sites. RESULTS With both treatments, nerve block for MC teeth on the treated side was achieved by 5 (n = 5 dogs) or 10 (1) minutes after injection, but nerve block for ipsilateral MPM4 and MM2 teeth was successful for only 3 dogs and 1 dog, respectively. Mean duration of nerve blocks for MC teeth was 120 and 277 minutes following injection of lidocaine and lidocaine-bupivacaine, respectively. CONCLUSIONS AND CLINICAL RELEVANCE Local anesthesia, as performed in this study, successfully blocked innervation of MC teeth, but results for MPM4 and MM2 teeth were inconsistent. This specific technique should not be used during tooth extractions caudal to the MC teeth.

Objective: To determine the prevalence and types of tooth resorption in dogs with oral tumors and to compare findings with those for control dogs. Animals: 101 dogs with oral tumors and 128 control dogs that did not have oral tumors and for which dental radiographs were available. Procedures: Exclusion criteria for dogs included systemic disease, long-term administration of anti-inflammatory drugs, traumatic occlusion, severe semigeneralized or generalized periodontitis, and endodontic disease. For each dog with an oral tumor, histologic sections of biopsy specimens of tumors were examined. Dental radiographic images of dogs were examined, and the presence and type of tooth resorption were determined for each tooth. Statistical analyses were performed to compare data regarding prevalence of tooth resorption. Results: Teeth at tumor sites in dogs with nonodontogenic tumors were significantly more frequently affected with external inflammatory resorption, compared with teeth at tumor sites in dogs with odontogenic tumors. Teeth at sites distant from tumors in dogs with oral tumors were 3.2 times as likely to have external surface resorption (OR, 3.2; 95% confidence interval, 1.3 to 7.9) and 83.4 times as likely to have external inflammatory resorption (OR, 83.4; 95% confidence interval, 9.7 to 719.6) as teeth in control dogs. Conclusions and Clinical Relevance: Resorption of teeth at tumor sites and at sites distant from tumors was common in dogs with oral tumors. Results of the present study will contribute to an understanding of the complex effects of oral tumors on local and distant hard tissues.

The normal blood supply to the canine mandible and mandibular teeth was determined by microangiography and correlated histology. Branches of the inferior alveolar artery supplied the cortical bone of the mandibular body. Vessels from the periosteal and endosteal surfaces supplied symphyseal cortical bone. Direct vascular anastomoses were not found to cross the fibrous mandibular symphysis. Blood supply to the mandibular teeth was via dental arteries derived from the inferior alveolar artery, with interdental and interradicular arteries supplying the alveolar bone and periodontal ligament.

OBJECTIVE To compare the efficacy and duration of desensitization of oral structures following injection of various volumes of lidocaine-bupivacaine via an infraorbital approach in dogs. ANIMALS 6 healthy adult hound-type dogs. PROCEDURES In a randomized crossover study, each dog received 1, 2, and 3 mL of a 2% lidocaine-0.5% bupivacaine mixture (50:50 vol/vol) injected within and near the caudal aspect of the infraorbital canal with a 14-day washout period between treatments. Dogs were anesthetized, and each treatment was administered through a 22-gauge, 4.5-cm-long catheter, which was fully inserted through and then withdrawn 2 cm to the caudal aspect of the infraorbital canal. The reflex-evoked motor potential was measured for the maxillary canine tooth (MC), fourth premolar tooth (MPM4), second molar tooth (MM2), and hard palate mucosa ipsilateral to the injected treatment and for the contralateral MC (control) at predetermined times before and for 6 hours after treatment administration or until the block was no longer effective. For each oral structure, the proportion of dogs with desensitization (efficacy) and time to onset and duration of desensitization were compared among the 3 treatments (injectate volumes). RESULTS Treatment was not associated with efficacy, time to onset, or duration of desensitization. Regardless of treatment, MC and MPM4 were more frequently desensitized and mean durations of desensitization for MC and MPM4 were longer, compared with those for MM2 and the hard palate. CONCLUSIONS AND CLINICAL RELEVANCE the volume of local anesthetic used for an infraorbital nerve block had no effect on block efficacy or duration.

Objective—To evaluate the effectiveness of a topically applied gel containing essential oils (menthol and thymol) and polyphenolic antioxidants (phloretin and ferulic acid) for reducing halitosis in dogs. Animals—20 dogs. Procedures—A blinded crossover clinical trial was conducted. Dogs received a dental cleaning and examination (periodontal examination including periodontal probing and assessments of plaque, calculus, and gingivitis). Owners then applied a gel (active or placebo) to oral soft tissues twice daily for a 4-week period. Teeth of the dogs were cleaned again, and owners applied the other gel for a 4-week period. Clinicians scored halitosis immediately after the initial cleaning and at 4 and 8 weeks, and owners scored halitosis weekly. Results—Halitosis assessment by clinicians revealed that both groups had improvement in halitosis scores. Two dogs were removed because of owner noncompliance. In the active-to-placebo group (n = 9), halitosis was significantly reduced during application of the active gel but increased during application of the placebo. Seven of 9 owners reported increased halitosis when treatment was changed from the active gel to the placebo. In the placebo-to-active group (n = 9), halitosis decreased during application of the placebo and continued to decrease during application of the active gel. Seven of 9 owners reported a decrease in halitosis with the active gel. Conclusions and Clinical Relevance—An oral topically applied gel with essential oils and polyphenolic antioxidants applied daily after an initial professional dental cleaning decreased oral malodor in dogs.

Tooth surface and interradicular area (furcation) measurements were taken of 20 first mandibular molar teeth obtained randomly from canine cadavers. The lingual furcation entrance had a mean width of 1.2 +/- 0.3 mm. The buccal furcation entrance had a mean width of 1.3 +/- 0.4 mm. There was no significant difference between mean furcation entrance measurements. The horizontal attachment area between the cementoenamel junction and the coronal roof of the lingual furcation was 1.1 +/- 0.4 mm. The horizontal attachment area between the cementoenamel junction and the coronal roof of the buccal furcation (0.5 +/- 0.3 mm) was significantly different from the lingual horizontal attachment area. Distal root length and mesial root length were 16.0 +/- 1.5 mm and 16.1 +/- 1.6 mm, respectively. The mean coefficient of variation for variables measured was 14.0%. Tooth size did not have a significant effect on furcation entrance measurement. All teeth had a concavity coronal to the furcation area, which extended apically for a mean distance of 65.6% of the distal root length and 83.8% of the mesial root length. Furcation anatomy of the first mandibular molar is complex and may be a contributing factor in periodontal disease involving the interradicular area.

Tooth root and root trunk surface area measurements were taken from 20 first mandibular molar (M1) teeth obtained randomly from canine cadaver specimens. The mean root surface area for all teeth was 497.1 +/- 116.2 mm2. The cumulative surface areas of the mesial (MR) and distal (DR) roots were 251.4 +/- 70.2 mm2 and 211.7 +/- 51.9 mm2, respectively. The root trunk surface area was 34.0 +/- 21.4 mm2. Surface area measurements for individual roots and the root trunk were significantly different. Cumulative individual root surface area (MR + DR) was 93.2% of total root surface area. The MR surface area contributed most to total root surface area (50.6%) and cumulative individual root surface area (54.3%). The mean distance from the beginning of the root trunk to the furcation was 2.1 mm. On the basis of the linear variation of the percentage of root surface area of the M1, 44.6% of the total root surface area was 3 to 7 mm apical to the most coronal aspect of the cementoenamel junction. The MR was heart-shaped, compared with the more cylindrical shape of the DR.

OBJECTIVE To compare efficacy and duration of desensitization of oral structures with a lidocaine-bupivacaine mixture administered via a lateral percutaneous or modified infraorbital approach. ANIMALS 6 healthy adult hound-type female dogs. PROCEDURESIn this crossover study, dogs were randomized for side (left or right) and maxillary nerve approach (lateral percutaneous or infraorbital), with a 2-week washout period. Dogs were anesthetized, and a 2-mL mixture of 2% lidocaine and 0.5% bupivacaine (50:50 [vol/vol]) was administered with a 22-gauge, 4.5-cm-long catheter inserted through the infraorbital canal (infraorbital approach) or with a shielded stimulating needle to the maxillary nerve (percutaneous approach). Reflex-evoked motor potentials were measured for the maxillary canine tooth, fourth premolar tooth, second molar tooth, and hard palate mucosa ipsilateral to the injected mixture and for the contralateral maxillary canine tooth (control) at three 10-minute intervals before injection (baseline) and at predetermined times after injection for up to 6.7 hours. For each oral structure, the proportion of dogs with desensitization (efficacy) and time to onset and duration of desensitization were compared between approaches. RESULTS The proportion of dogs with successful nerve blockade did not significantly differ between infraorbital and percutaneous approaches and among the 4 oral structures. Time to onset of desensitization did not differ between approaches, but duration was significantly longer with the infraorbital approach. CONCLUSIONS AND CLINICAL RELEVANCE A modified infraorbital approach with the lidocaine-bupivacaine mixture had similar effects to a lateral percutaneous approach but provided a longer duration of desensitization. Neither approach was universally successful at desensitizing all oral structures.

OBJECTIVE To evaluate the usefulness of injection of indocyanine green (ICG) solution with near-infrared (NIR) fluorescence imaging for transcutaneous detection of sentinel lymph nodes (SLNs) and their associated lymphatic vessels in the oral mucosa of healthy dogs. ANIMALS 6 adult purpose-bred research hounds. PROCEDURES Each dog was sedated, and 1 mL of ICG solution was injected into the gingival mucosa dorsal to the right maxillary canine tooth. Subsequently, NIR fluorescence imaging was used to transcutaneously detect the lymphatic vessels and SLNs. The distance between the injection site and each SLN was measured. Time to first evidence of node fluorescence was recorded, and velocity of ICG movement was calculated. A slide preparation of a fine-needle aspiration sample of the fluorescing structure underwent cytologic examination (to confirm presence of lymphatic tissue) and NIR fluorescence imaging (to confirm presence of ICG). RESULTS The ipsilateral mandibular lymphocentrum was the SLN in all dogs. The time to visually detectable fluorescence ranged from 4 to 15 minutes (mean ± SD, 8.8 ± 3.76 minutes). The mean velocity was 1.94 ± 0.93 cm/min. Fluorescence was not observed in the contralateral lymph nodes. Each fluorescing structure was confirmed to be lymphatic tissue, and NIR fluorescence imaging revealed that ICG was present in the sampled SLN. CONCLUSIONS AND CLINICAL RELEVANCE Results indicated that injection of ICG solution with NIR fluorescence imaging can be used to transcutaneously identify SLNs along with associated lymphatic vessels in the oral mucosa of healthy dogs. Time from injection to identification of fluorescence was rapid with prolonged retention of material within the SLN, indicating that this procedure could be performed during surgery.
