Short-term effects of ambient air pollution on pediatric outpatient visits for respiratory diseases in Yichang city, China

Previous studies have suggested that short-term exposure to ambient air pollution was associated with pediatric hospital admissions and emergency room visits for certain respiratory diseases; however, there is limited evidence on the association between short-term air pollution exposure and pediatric outpatient visits. Our aim was to quantitatively assess the short-term effects of ambient air pollution on pediatric outpatient visits for respiratory diseases. We conducted a time-series study in Yichang city, China between Jan 1, 2014 and Dec 31, 2015. Daily counts of pediatric respiratory outpatient visits were collected from 3 large hospitals, and then linked with air pollution data from 5 air quality monitoring stations by date. We used generalized additive Poisson models to conduct linear and nonlinear exposure-response analyses between air pollutant exposures and pediatric respiratory outpatient visits, adjusting for seasonality, day of week, public holidays, temperature, and relative humidity. Each interquartile range (IQR) increase in PM2.5 (lag 0), PM10 (lag 0), NO2 (lag 0), CO (lag 0), and O3 (lag 4) concentrations was significantly associated with a 1.91% (95% CI: 0.60%, 3.23%), 2.46% (1.09%, 3.85%), 1.88% (0.49%, 3.29%), 2.00% (0.43%, 3.59%), and 1.91% (0.45%, 3.39%) increase of pediatric respiratory outpatient visits, respectively. Similarly, the nonlinear exposure-response analyses showed monotonic increases of pediatric respiratory outpatient visits by increasing air pollutant exposures, though the associations for NO2 and CO attenuated at higher concentrations. These associations were unlikely modified by season. We did not observe significant association for SO2 exposure. Our results suggest that short-term exposures to PM2.5, PM10, NO2, CO, and O3 may account for increased risk of pediatric outpatient visits for respiratory diseases, and emphasize the needs for reduction of air pollutant exposures for children.

This study examined effects of short-term urban air pollution exposures on inflammation, thrombosis, and autonomic imbalance in subjects at risk for cardiovascular disease (CVD). We enrolled 61 patients with multiple CVD risk factors and measured high sensitive C-reactive protein (hs-CRP), fibrinogen, D-dimer, and heart rate variability (HRV) indices. Two health examinations for each participant were performed during December 2002 through September 2003. Changes in inflammation and thrombotic markers and HRV indices with exposures to PM2.5, organic carbon (OC), elemental carbon (EC), sulfur dioxide (SO2), nitrogen dioxide (NO2), and carbon monoxide (CO) at 1- to 3-day lags were analyzed using mixed models. The results showed inflammatory and thrombotic markers increased with 1- to 3-day lagged PM2.5 components and gaseous pollutants exposures. hs-CRP maximally increased 0.19 [95% confidence interval (CI): 0.07–0.31] and 0.15 (95% CI: 0.05–0.24) mg/L for an interquartile range (IQR) of 1-day lagged SO2 (2.3 ppb) and CO (0.5 ppm), respectively. D-dimer maximally increased 1.05 (95% CI: 0.13–1.75), 0.72 (95% CI: 0.09–1.21), 0.92 (95% CI: 0.13–1.50), and 0.90 (95% CI: 0.07–1.61) mg/dL for an IQR of 1-day lagged OC (3.9 μg/m3), EC (2.0 μg/m3), SO2, and NO2 (13.4 ppb), respectively. The HRV indices, including low frequency, very low frequency, and the ratio of low frequency to high frequency decreased 19.8 (95% CI: 4.4–32.7), 12.9 (95% CI: 0.8–23.4), and 17.6 (95% CI: 5.4–28.2)% for an IQR of 1-day lagged PM2.5 (20.2 μg/m3), respectively. Our findings demonstrated PM2.5 components and gaseous pollutants exert prolonged inflammatory and thrombotic reactions, while PM2.5 exert an immediate autonomic imbalance.

Little information exists on emission factors (EFs, quantities of pollutants emitted per unit of fuel consumed) for brick kilns in China, although brick kilns are important emission sources of many air pollutants, and 45% of the world's bricks are produced in China. In this study, EFs of carbon dioxide (CO2), carbon monoxide (CO), sulfur dioxide (SO2), nitrogen oxides (NOx), particulate matters (PMs), black carbon (BC), organic carbon (OC), and polycyclic aromatic hydrocarbons (PAHs) for brick kilns were derived based on field measurements of a total of 18 brick kilns of major types in China. This was the first study to quantify EFs of both stack and fugitive sources based on a modified carbon balance method that was developed for this study. The EFs of most pollutants, especially the incomplete combustion products in fugitive emissions, were much higher than those for stack emissions, indicating a substantial underestimation of total emissions when leakage is not taken into consideration. This novel method can be applied to quantify emissions from other similar sources with both stack and fugitive emissions.

Five different domestic heating boilers (automatic, over-fire, with down-draft combustion and gasification) and three types of fuel (lignite, wood and mixed fuel) were examined in 25 combustion tests and correlated with the emissions of particulate matter (PM), carbon monoxide (CO), total organic carbon (TOC) and 12 polycyclic aromatic hydrocarbons (PAHs with MW = 178–278 g/mol) focusing on particle phase. However, the distribution of 12 PAHs in gas phase was considered as well due to the presence mainly of lighter PAHs in gas phase. The PAHs, as well as the CO and TOC, are the indicators of incomplete combustion, and in this study PAH emission increased significantly with increasing emissions of CO and TOC. The PAHs were mainly detected on PM2.5, their contents were increasing linearly with increasing PM2.5 emissions. The highest emission factors of PAHs were measured for boilers of old construction, such as over-fire boiler (5.8–929 mg/kg) and boiler with down-draft combustion (3.1–54.1 mg/kg). Modern types of boilers produced much lower emissions of PAHs, in particular, automatic boiler (0.3–3.3 mg/kg) and gasification boilers (0.2–6.7 mg/kg). In general, the inefficient combustion at reduced output of boilers generated 1.4–17.7 times more emissions of PAHs than the combustion at nominal output of boilers. It is recommended to operate boilers at nominal output with sufficient air supply and to use the proper fuel to minimise PAHs emissions from domestic heating appliances.

Based on the 12-hour PM2.5 samples collected in an urban site of Beijing, sixteen PM2.5-bound Polycyclic Aromatic Hydrocarbons (PAHs) were measured to investigate the characteristics and potential source regions of particulate PAHs in Beijing. The study period included the summer period in July–August 2014, the APEC source control period during the Asia-Pacific Economic Cooperation (APEC) meeting in the first half of November 2014, and the heating period in the second half of November 2014. Compared to PM2.5, sum of 16 PM2.5-bound PAHs exhibited more significant seasonal variation with the winter concentration largely exceeding the summer concentration. Temperature appeared to be the most crucial meteorological factor during the summer and heating periods, while PM2.5-bound PAHs showed stronger correlation with relative humidity and wind speed during the APEC source control period. Residential heating significantly increased the concentrations of higher-ring-number (≥4) PAHs measured in PM2.5 fraction. Potential source contribution function (PSCF) and concentration weighted trajectory (CWT) analysis as well as the (3 + 4) ring/(5 + 6) ring PAH ratio analysis revealed the seasonal difference in the potential source area of PM2.5-bound PAHs in Beijing. Southern Hebei was the most likely potential source area in the cold season. Using black carbon (BC) and carbon monoxide (CO) as the PAH tracers, regional residential, transportation and industry emissions all contributed to the PAH pollution in Beijing.

Continuous measurements of PM2.5, PM10 and CO were conducted at an urban site of Udaipur in India from April 2011 to March 2012. The annual mean concentrations of PM2.5, PM10 and CO were 42 ± 17 μg m−3, 114 ± 31 μg m−3 and 343 ± 136 ppbv, respectively. Concentrations of both particulate and CO showed high values during winter/pre-monsoon (dry) period and lowest in the monsoon season (wet). Local anthropogenic emission and long-range transport from open biomass burning sources along with favourable synoptic meteorology led to elevated levels of pollutants in the dry season. However, higher values of PM10/PM2.5 ratio during pre-monsoon season were caused by the episodes of dust storm. In the monsoon season, flow of cleaner air, rainfall and negligible emissions from biomass burning resulted in the lowest levels of pollutants. The concentrations of PM2.5, PM10 and CO showed highest values during morning and evening rush hours, while lowest in the afternoon hours. In winter season, reductions of PM2.5, CO and PM10 during weekends were highest of 15%, 13% and 9%, respectively. In each season, the highest PM2.5/PM10 ratio coincided with the highest concentrations of pollutants (CO and NOX) indicating predominant emissions from anthropogenic sources. Exceptionally high concentrations of PM10 during the episode of dust storm were due to transport from the Arabian Peninsula and Thar Desert. Up to ∼32% enhancements of PM10 were observed during strong dust storms. Relatively low levels of O3 and NOx during the storm periods indicate the role of heterogeneous removal.

Data on particulate matter of diameter <2.5 μm (PM2.5) in the city of Chongqing were first announced in 2013. We wished to assess the effects of pollutants on asthmatic children in Chongqing, China. Daily numbers of hospital visits because of asthma attacks in children aged 0–18 years in 2013 were collected from the Children's Hospital of Chongqing Medical University. Data on pollutants were accessed from the nine air quality-monitoring stations in Chongqing. A time-stratified case–crossover design was applied and conditional logistic regression was undertaken to analyze the data. We found that short-term exposure to PM10, PM2.5, sodium dioxide, nitrogen and carbon monoxide could trigger hospital visits for asthma in children. Nitrogen dioxide had an important role, whereas ozone had no effect.

Both air pollution and meteorological factors in metropolitan areas increased emergency department (ED) visits from people with chronic obstructive pulmonary disease (COPD). Few studies investigated the associations between air pollution, meteorological factors, and COPD-related health disorders in Asian countries. This study aimed to investigate the relationship between the environmental factors and COPD-associated ED visits of susceptible elderly population in the largest Taiwanese metropolitan area (Taipei area, including Taipei city and New Taipei city) between 2000 and 2013. Data of air pollutant concentrations (PM10, PM2.5, O3, SO2, NO2 and CO), meteorological factors (daily temperature, relative humidity and air pressure), and daily COPD-associated ED visits were collected from Taiwan Environmental Protection Administration air monitoring stations, Central Weather Bureau stations, and the Taiwan National Health Insurance database in Taipei area. We used a case-crossover study design and conditional logistic regression models with odds ratios (ORs), and 95% confidence intervals (CIs) for evaluating the associations between the environmental factors and COPD-associated ED visits. Analyses showed that PM2.5, O3, and SO2 had significantly greater lag effects (the lag was 4 days for PM2.5, and 5 days for O3 and SO2) on COPD-associated ED visits of the elderly population (65–79 years old). In warmer days, a significantly greater effect on elderly COPD-associated ED visits was estimated for PM2.5 with coexistence of O3. Additionally, either O3 or SO2 combined with other air pollutants increased the risk of elderly COPD-associated ED visits in the days of high relative humidity and air pressure difference, respectively. This study showed that joint effect of urban air pollution and meteorological factors contributed to the COPD-associated ED visits of the susceptible elderly population in the largest metropolitan area in Taiwan. Government authorities should review existing air pollution policies, and strengthen health education propaganda to ensure the health of the susceptible elderly population.

Over 700 million people in Sub-Saharan Africa depend on solid biomass fuel and use simple cookstoves in poorly ventilated kitchens, which results in high indoor concentrations of household air pollutants. Switching from biomass to biogas as a cooking fuel can reduce airborne emissions of fine particulate matter (PM2.5) and carbon monoxide (CO), but households often only partially convert to biogas, continuing to use solid biomass fuels for part of their daily cooking needs. There is little evidence of the benefits of partial switching to biogas. This study monitored real-time PM2.5 and CO concentrations in 35 households in Cameroon and Uganda where biogas and firewood (or charcoal) were used. The 24 h mean PM2.5 concentrations in households that used: (1) firewood and charcoal; (2) both firewood (mean 54% cooking time) and biogas (mean 46% cooking time); and (3) only biogas, were 449 μg m⁻³, 173 μg m⁻³ and 18 μg m⁻³ respectively. The corresponding 24 h mean CO concentrations were 14.2 ppm, 2.7 ppm and 0.5 ppm. Concentrations of both PM2.5 and CO were high and exceeded the World Health Organisation guidelines when firewood and charcoal were used. Partially switching to biogas reduced CO exposure to below the World Health Organisation guidelines, but PM2.5 concentrations were only below the 24 h recommended limits when households fully converted to biogas fuel. These results indicate that partial switching from solid fuels to biogas is not sufficient and continues to produce concentrations of household air pollution that are likely to harm the health of those exposed. Programmes introducing biogas should aim to ensure that household energy needs can be fully achieved using biogas with no requirement to continue using solid fuels.

Environmental concerns have been raised on the adverse health effects of vehicle emissions in micro-scale traffic-crowded street canyons, especially for pedestrians and residents living in near-road buildings. Viaduct design is sometimes used to improve transportation efficiency but possibly affects urban airflow and the resultant exposure risk, which have been rarely investigated so far. The personal intake fraction (P_IF) is defined as the average fraction of total emissions that is inhaled by each person of a population (1 ppm = 1 × 10⁻⁶), and the daily carbon monoxide (CO) pollutant exposure (Et) is estimated by multiplying the average concentration of a specific micro-environment within one day. As a novelty, by considering time activity patterns and breathing rates in various micro-environments for three age groups, this paper introduces IF and Et into computational fluid dynamic (CFD) simulation to quantify the impacts of street layouts (street width/building height W/H = 1, 1.5, 2), source location, viaduct settings and noise barriers on the source-exposure correlation when realistic CO sources are defined. Narrower streets experience larger P_IF (1.51–5.21 ppm) and CO exposure, and leeward-side buildings always attain higher vehicular pollutant exposure than windward-side. Cases with a viaduct experience smaller P_IF (3.25–1.46 ppm) than cases without a viaduct (P_IF = 5.21–2.23 ppm) if the single ground-level CO source is elevated onto the viaduct. With two CO sources (both ground-level and viaduct-level), daily CO exposure rises 2.80–3.33 times but P_IF only change slightly. Noise barriers above a viaduct raise concentration between barriers, but slightly reduce vehicular exposure in near-road buildings. Because people spend most of their time indoors, vehicular pollutant exposure within near-road buildings can be 6–9 times that at pedestrian level. Although further studies are still required to provide practical guidelines, this paper provides effective methodologies to quantify the impacts of street/viaduct configurations on human exposure for urban design purpose.
