Monitoring the effect of air pollution episodes on health care consultations and ambulance call-outs in England during March/April 2014: A retrospective observational analysis

There is an increasing body of evidence illustrating the negative health effects of air pollution including increased risk of respiratory, cardiac and other morbid conditions. During March and April 2014 there were two air pollution episodes in England that occurred in close succession. We used national real-time syndromic surveillance systems, including general practitioner (GP) consultations, emergency department attendances, telehealth calls and ambulance dispatch calls to further understand the impact of these short term acute air pollution periods on the health seeking behaviour of the general public. Each air pollution period was comparable with respect to particulate matter concentrations (PM10 and PM2.5), however, the second period was longer in duration (6 days vs 3 days) and meteorologically driven ‘Sahara dust’ contributed to the pollution. Health surveillance data revealed a greater impact during the second period, with GP consultations, emergency department attendances and telehealth (NHS 111) calls increasing for asthma, wheeze and difficulty breathing indicators, particularly in patients aged 15–64 years. Across regions of England there was good agreement between air quality levels and health care seeking behaviour. The results further demonstrate the acute impact of short term air pollution episodes on public health and also illustrate the potential role of mass media reporting in escalating health care seeking behaviour.

Sediment cores and soil samples were taken from nine lakes and their catchments across England with varying degrees of direct human disturbance. Mercury (Hg) analysis demonstrated a range of impacts, many from local sources, resulting from differing historical and contemporary site usage and management. Lakes located in industrially important areas showed clear evidence for early Hg pollution with concentrations in sediments reaching 400–1600 ng g−1 prior to the mid-19th century. Control of inputs resulting from local management practices and a greater than 90% reduction in UK Hg emissions since 1970 were reflected by reduced Hg pollution in some lakes. However, having been a sink for Hg deposition for centuries, polluted catchment soils are now the major Hg source for most lakes and consequently recovery from reduced Hg deposition is being delayed.

Many populations of bat species across the globe are declining, with chemical contamination one of many potential stressors implicated in these demographic changes. Metals still contaminate a wide range of habitats, but the risks to bats remain poorly understood. This study is the first to present a national scale assessment of toxic metal (Cd, Pb) and essential trace metal (Cu, Zn) concentrations in bats. Metal concentrations in tissues (kidneys, liver, stomach -stomach content, bones and fur) were measured in 193 Pipistrellus sp. in England and Wales using ICP-MS, and compared to critical toxic concentrations for small mammals. The concentrations of metals determined in bat tissues were generally lower than those reported elsewhere. Strong positive associations were found between concentrations in tissues for a given metal (liver and kidneys for Cd, Cu and Pb; stomach and fur and fur and bones for Pb), suggesting recent as well as long term exposure to these contaminants. In addition, positive correlations between concentrations of different metals in the same tissues (Cd and Zn, Cu and Zn, Cd and Pb, Pb and Zn) suggest a co-exposure of metals to bats. Approximately 21% of the bats sampled contained residues of at least one metal at concentrations high enough to elicit toxic effects (associated with kidney damage), or to be above the upper level measured in other mammal species. Pb was found to pose the greatest risk (with 7–11% of the bats containing concentrations of toxicological concern), followed by Cu (4–9%), Zn (0.5–5.2%) and Cd (0%). Our data suggest that leaching of metals into our storage matrix, formaldehyde, may have occurred, especially for Cu. The overall findings suggest that metal contamination is an environmental stressor affecting bat populations, and that further research is needed into the direct links between metal contamination and bat population declines worldwide.

Public perception research evaluating awareness and attitudes towards marine protection is limited in the United Kingdom (UK) and worldwide. Given public opinion can help drive policy and affect its successful delivery we conducted nationwide surveys in 2005, 2010 and 2015 to assess public knowledge of UK (England, Scotland and Wales) sea ‘health’ and management. Respondents from all three surveys were relatively pessimistic about sea ‘health’, perceiving this as poor-fair and largely in decline. Enthusiasm for marine conservation was high with almost two-thirds of respondents in each survey wanting >40% of UK seas highly protected from fishing and damaging activities. In 2015 there was considerable dissatisfaction with the rate of progress in Marine Conservation Zone designation and over three-quarters of respondents considered dredging and trawling to be inappropriate in protected areas, contrary to management. The UK government and devolved administrations need to better align future conservation and management with public expectations.

Microplastics enter the environment as a result of larger plastic items breaking down (‘secondary’) and from particles originally manufactured at that size (‘primary’). Personal care products are an important contributor of secondary microplastics (typically referred to as ‘microbeads’), for example in toothpaste, facial scrubs and soaps. Consumers play an important role in influencing the demand for these products and therefore any associated environmental consequences. Hence we need to understand public perceptions in order to help reduce emissions of microplastics. This study explored awareness of plastic microbeads in personal care products in three groups: environmental activists, trainee beauticians and university students in South West England. Focus groups were run, where participants were shown the quantity of microbeads found in individual high-street personal care products. Qualitative analysis showed that while the environmentalists were originally aware of the issue, it lacked visibility and immediacy for the beauticians and students. Yet when shown the amount of plastic in a range of familiar everyday personal care products, all participants expressed considerable surprise and concern at the quantities and potential impact. Regardless of any perceived level of harm in the environment, the consensus was that their use was unnatural and unnecessary. This research could inform future communications with the public and industry as well as policy initiatives to phase out the use of microbeads.

The metals cadmium, mercury and lead, and the ICES7 CB levels were analysed in the common dab Limanda limanda to investigate whether concentrations of these determinants are at levels above established OSPAR assessment criteria around England and Wales. The results indicate that CB118 is above the OSPAR derived EAC at 22 out of 29 sampled stations, with 15 showing significant reductions. Cadmium is increasing significantly at six stations with two sites in the North Sea breaching corresponding OSPAR EAC. Mean results for all other PCBs and metals stayed below the EAC thresholds, with Hg concentrations even falling below BAC levels at South Eddystone located in the western English Channel. This study shows that PCBs banned over 20years ago are still found at concentrations in fish liver tissues which may pose a health risk to both the individual fish and their predators.

While the effects of coastal disposal of dredged material on benthic assemblage structure have been well studied, our understanding of the mechanism of such responses, and their potential ecological implications, remain relatively unknown. Data from a licenced disposal site off the northeast coast of England are analysed to address this and improve our ability to make informed licencing decisions for this activity. Assemblages within the disposal site displayed reduced number of species and total invertebrate density, an altered assemblage taxonomic structure, and a shift towards a greater numerical dominance of less-productive individuals. Following separate analyses of biological response and effect traits, a novel approach for marine benthic trait analysis, we identify the traits responsible (i.e. response traits) for the observed structural alterations. Furthermore, analysis of the effect traits revealed that the assemblages characterising the disposal site possess a greater bioturbative capability compared to those not directly impacted by disposal.

Stable isotope analyses of the abundant infaunal polychaete Hediste diversicolor, recognised as an indicator of sewage pollution, support the hypothesis that nutrient enrichment promotes surface deposit feeding, over suspension feeding and predation. At sewage-polluted sites in three estuaries in SE England Hediste mainly consumed microphytobenthos, sediment organic matter and filamentous macroalgae Ulva spp. At cleaner sites Hediste relied more on suspension feeding and consumption of Spartina anglica. There were no consistent differences in Hediste densities between the polluted and cleaner sites, probably because of increased densities at the cleaner sites too, facilitated by the planting of Spartina and nitrogen enrichment there too, including from agricultural run-off. Increased nutrient enrichment and the artificial availability of Spartina have probably increased densities of, and deposit-feeding by, Hediste in the past half-century and contributed indirectly to saltmarsh losses, since deposit-feeding by Hediste has been implicated in recent saltmarsh erosion in SE England.

Whereas the response of lotic benthic macroinvertebrates to different environmental stressors is a widespread practice nowadays in assessing the water and habitat quality, the use of hyporheic zone invertebrates is still in its infancy. In this study, classification and regression trees analysis were employed in order to assess the ecological requirements and the potential as bioindicators for the hyporheic zone invertebrates inhabiting four lowland chalk rivers (south England) with contrasting eutrophication levels (based on surface nitrate concentrations) and magnitude of land use (based on percentage of fine sediments load and median interstitial space). Samples of fauna, water and sediment were sampled twice, during low (summer) and high (winter) groundwater level, at depths of 20 and 35 cm. Certain groups of invertebrates (Glossosomatidae and Psychomyiidae caddisflies, and riffle beetles) proved to be good indicators of rural catchments, moderately eutrophic and with high fine sediment load. A diverse community dominated by microcrustaceans (copepods and ostracods) were found as good indicators of highly eutrophic urban streams, with moderate-high fine sediment load. However, the use of other taxonomic groups (e.g. chironomids, oligochaetes, nematodes, water mites and the amphipod Gammarus pulex), very widespread in the hyporheic zone of all sampled rivers, is of limited use because of their high tolerance to the analysed stressors. We recommend the use of certain taxonomic groups (comprising both meiofauna and macroinvertebrates) dwelling in the chalk hyporheic zone as indicators of eutrophication and colmation and, along with routine benthic sampling protocols, for a more comprehensive water and habitat quality assessment of chalk rivers.

It has been known that outdoor temperature influences seasonal fluctuation of blood pressure and cholesterol levels, but the role of indoor temperature has been less studied. Therefore, the aim of the present study was to examine the associations between indoor temperature and biomarkers in a countrywide and population-based setting. Data was retrieved from the English Longitudinal Study of Ageing, 2012–2013. Information on demographics, room temperature and a series of biomarkers measured in the blood and lung was obtained at household interviews. t test, chi-square test and a generalized linear model were performed cross-sectionally. Of 7997 older adults with the valid indoor temperature measurements, there were 1301 (16.3 %) people who resided in cold homes (<18 °C). Age was inversely associated with people who resided in cold homes or who tended not to have blood pressure check-up. Those who resided in cold homes had higher blood pressure readings, worse handgrip, lower vitamin D levels, higher cholesterol levels, higher insulin-like growth factor levels, higher haemoglobin levels, lower level of white blood cell count and worse lung conditions. One in six older adults aged 50 and above in England resided in cold homes and had poor biomarker values. For the future research direction, studies with a longitudinal approach to systematically monitor indoor temperature, biomarkers and health and wellbeing would be suggested. From the practice and policy perspectives, increasing health knowledge on the adverse effect of low indoor temperature on risks of cardiac and respiratory conditions, affording to the heating and re-designing of residential buildings to keep warm by using efficient energy, should be kept as priority.
