Human exposure to environmental health concern by types of urban environment: The case of Tel Aviv

This study classifies urban environments into types characterized by different exposure to environmental risk factors measured by general sense of discomfort and Heart Rate Variability (HRV). We hypothesize that a set of environmental factors (micro-climatic, CO, noise and individual heart rate) that were measured simultaneously in random locations can provide a better understanding of the distribution of human exposure to environmental loads throughout the urban space than results calculated based on measurements from close fixed stations. We measured micro-climatic and thermal load, CO and noise, individual Heart Rate, Subjective Social Load and Sense of Discomfort (SD) were tested by questionnaire survey.The results demonstrate significant differences in exposure to environmental factors among 8 types of urban environments. It appears that noise and social load are the more significant environmental factors to enhance health risks and general sense of discomfort.

Exposure of young children to toxic metals in urban environments is largely due to soil and dust ingestion. Soil particle size distribution and concentrations of toxic metals in different particle sizes are important risk factors in addition to bioaccessibility of these metals in the particles. Analysis of particle size distribution and metals concentrations for 13 soils, 12 sampled from urban gardens and 1 from orchard found that fine particles (<105 μm) comprised from 22 to 66% by weight of the tested soils, with Ba, Cu, Pb and Zn generally at higher concentrations in the finer particles. However, metal bioaccessibility was generally lower in finer particles, a trend most pronounced for Ba and Pb. Gastric was higher than gastrointestinal bioaccessibility for all metals except Cu. The lower bioaccessibility of Pb in urban garden soils compared to orchard soil is attributable to the higher organic matter content of the garden soils.

Studies indicated that occupationally exposed to metals could result in oxidative damage and inflammation and increase cardiovascular diseases risk. However, epidemiological studies about the associations of metals exposure with diabetes risk among coke oven workers were limited.This study aims to investigate the potential associations of 23 metals levels with the risk of diabetes among coke oven workers.The analysis was conducted in a cross-sectional study including 1493 participants. Urinary metals and urinary polycyclic aromatic hydrocarbons (PAHs) metabolites levels were determined by inductively coupled plasma mass spectrometer and gas chromatograph-mass spectrometer respectively. Multivariate logistic regression was used to investigate the associations of urinary metal levels with diabetes risk with adjustment for potential confounding factors including gender, age, BMI, education, smoking, drinking, physical activity, hypertension, hyperlipidemia and urinary PAHs metabolites levels.Compared with the normoglycemia group, the levels of urinary copper, zinc, arsenic, selenium, molybdenum, and cadmium were significantly higher in the diabetes group (all p < 0.05). Participants with the highest tertile of urinary copper and zinc had 2.12 (95%CI: 1.12–4.01) and 5.43 (95%CI: 2.61–11.30) fold risk of diabetes. Similar results were found for hyperglycemia risk. Besides, participants with the highest tertile of manganese, barium, and lead had 1.65(1.22–2.23), 1.60(1.19–2.16) and 1.45(1.05–1.99) fold risk of hyperglycemia when compared with the lowest tertlie.The results indicated that the urinary copper and zinc levels were positively associated with the risk of diabetes and hyperglycemia among coke oven workers. Urinary manganese, barium and lead levels were also associated with increased risk of hyperglycemia independently of other traditional risk factors. These findings need further validation in prospective study with larger sample size.

An association of influenza-like illness (ILI) with outdoor air pollution has been reported. However, the effect of indoor air pollution on ILI was rarely investigated. We aimed to determine an association of indoor air pollution from coal combustion (IAPCC) and lifestyle with ILI risk in housewives, and the modification effect of phase II metabolic enzyme genes. We recruited 403 housewives for a cross-sectional study in Shanxi Province, China, including 135 with ILI frequency (≥1 time per year in the past ten years) as the case group and 268 with ILI frequency (<1 times per year) as the control group. Information on their energy usage characteristics and lifestyle was collected by questionnaires, as well as the single nucleotide polymorphisms (SNPs) of epoxide hydrolase 1 (rs1051740 and rs2234922), N-acetyltransferase 2 (rs1041983), and glutathione S-transferase (rs1695). We used exposure index to indicate the level of IAPCC among housewives. Our results revealed that the exposure index was positively correlated with ILI frequency. A significant dose-response trend between the exposure index and ILI risk was found with or without adjusting for confounders. Cooking frequency in kitchen with coal as primary fuel and ventilation frequency in the living room or bedroom with a coal-fueled stove for heating during the heating season were two important risk factors to affect ILI frequency. Only rs1051740 was found to be associated with exposure index, whereas it didn’t have interaction effect with exposure index on ILI frequency. In conclusion, IAPCC and SNPs of rs1051740 were both associated with ILI frequency.

Airborne fine particulate matter (PM2.5) is a risk factor for respiratory diseases. However, little is known about the effects of PM2.5 on bronchi. The present study investigated the effect of airborne PM2.5 on rat bronchi and the underlying mechanisms. Isolated rat bronchial segments were cultured for 24 h. Endothelin (ET) receptor-mediated contractile responses were recorded using a wire myograph. The mRNA and protein expression levels of ET receptors were studied using quantitative real-time PCR, Western blotting, and immunohistochemistry. The results demonstrated that ETA and ETB receptor agonists induced remarkable contractile responses on fresh and cultured bronchial segments. PM2.5 (1.0 or 3.0 μg/ml) significantly enhanced ETA and ETB receptor-mediated contractile responses in bronchi with a markedly increased maximal contraction compared to the DMSO or fresh groups. PM2.5 increased the mRNA and protein expression levels of ETA and ETB receptors. U0126 (a MEK1/2 inhibitor) and SB203580 (a p38 inhibitor) significantly suppressed PM2.5-induced increases in ETB receptor-mediated contractile responses, mRNA and protein levels. SP600125 (a JNK inhibitor) and SB203580 significantly abrogated the PM2.5-induced enhancement of ETA receptor-mediated contraction and receptor expression. In conclusion, PM2.5 upregulates ET receptors in bronchi. ETB receptor upregulation is associated with MEK1/2 and p38 pathways, and the upregulation of ETA receptor is involved in JNK and p38 pathways.

Tuberculosis (TB) is still a serious public health problem in various countries. One of the long-elusive but critical questions about TB is what the risk factors are and how they contribute for its seasonality. An ecologic study was conducted to examine the association between the variation of outdoor PM2.5 concentration and the TB seasonality based on the monthly TB notification and PM2.5 concentration data of Hong Kong and Beijing. Both descriptive analysis and Poisson regression analysis suggested that the outdoor PM2.5 concentration could be a potential risk factor for the seasonality of TB disease. The significant relationship between the number of TB cases and PM2.5 concentration was not changed when regression models were adjusted by sunshine duration, a potential confounder. The regression analysis showed that a 10 μg/m3 increase in PM2.5 concentrations during winter is significantly associated with a 3% (i.e. 18 and 14 cases for Beijing and Hong Kong, respectively) increase in the number of TB cases notified during the coming spring or summer for both Beijing and Hong Kong. Three potential mechanisms were proposed to explain the significant relationship: (1) increased PM2.5 exposure increases host's susceptibility to TB disease by impairing or modifying the immunology of the human respiratory system; (2) increased indoor activities during high outdoor PM2.5 episodes leads to an increase in human contact and thus the risk of TB transmission; (3) the seasonal change of PM2.5 concentration is correlated with the variation of other potential risk factors of TB seasonality. Preliminary evidence from the analysis of this work favors the first mechanism about the PM2.5 exposure-induced immunity impairment. This work adds new horizons to the explanation of the TB seasonality and improves our understanding of the potential mechanisms affecting TB incidence, which benefits the prevention and control of TB disease.

We investigated whether there might be an excess of breast and prostate cancer mortality among the population residing near Spanish industries, according to different categories of industrial groups. An ecologic study was designed to examine breast and prostate cancer mortality at a municipal level (period 1997–2006). Population exposure to pollution was estimated by means of distance from town of residence to industrial facilities. Using Besag-York-Mollié regression models with Integrated Nested Laplace approximations for Bayesian inference, we assessed the relative risk of dying from these tumors in 2-, 3-, 4-, and 5-km zones around installations, and analyzed the effect of category of industrial group. For all sectors combined, no excess risk was detected. However, excess risk of breast cancer mortality (relative risk, 95% credible interval) was detected near mines (1.10, 1.00–1.21 at 4 km), ceramic industries (1.05, 1.00–1.09 at 5 km), and ship building (1.12, 1.00–1.26 at 5 km), and excess risk of prostate cancer was detected near aquaculture for all distances analyzed (from 2.42, 1.53–3.63 at 2 km to 1.63, 1.07–2.36 at 5 km). Our findings do not support that residing in the vicinity of pollutant industries as a whole (all industrial sectors combined) is a risk factor for breast and prostate cancer mortality. However, isolated statistical associations found in our study with respect to specific industrial groups warrant further investigation.

More than 35 million people in coastal Bangladesh are vulnerable to increasing freshwater salinization. This will continue to affect more people and to a greater extent as climate change projections are realised in this area in the future. However the evidence for health effects of consuming high salinity water is limited. This research examined the association between drinking water salinity and blood pressure in young adults in coastal Bangladesh. We conducted a cross-sectional study during May-June 2014 in a rural coastal sub-district of Bangladesh. Data on blood pressure (BP) and salinity of potable water sources was collected from 253 participants aged 19–25 years. A linear regression method was used to examine the association between water salinity exposure categories and systolic BP (SBP) and diastolic BP (DBP) level. Sixty five percent of the study population were exposed to highly saline drinking water above the Bangladesh standard (600 mg/L and above). Multivariable linear regression analyses identified that compared to the low water salinity exposure category (<600 mg/L), those in the high water salinity category (>600 mg/L), had statistically significantly higher SBP (B 3.46, 95% CI 0.75, 6.17; p = 0.01) and DBP (B 2.77, 95% CI 0.31, 5.24; p = 0.03). Our research shows that elevated salinity in drinking water is associated with higher BP in young coastal populations. Blood pressure is an important risk factor of hypertension and cardiovascular diseases. Given the extent of salinization of freshwater in many low-lying countries including in Bangladesh, and the likely exacerbation related to climate change-induced sea level rise, implementation of preventative strategies through dietary interventions along with promotion of low saline drinking water must be a priority in these settings.

Mercury contamination in the River Beni basin is an important health risk factor, primarily for indigenous communities that live along the river. Among them are the Tacana, living in their original territory with sustainable use of their natural resources, consuming fish, Caiman yacare, and other riverine resources as their main source of protein. To assess mercury exposure to Tacana people, total mercury (THg) was evaluated in the muscle of seven commercial fish, and Caiman yacare (yacare caiman) during 2007 and 2008. THg was extracted by acid digestion and concentrations were determined by atomic absorption spectrometry. Mean mercury concentrations in C. yacare was 0.21 ± 0.22 μg g−1Hg w.w. (wet weight), which is lower than expected given its high trophic level, and its long life-span. It is possible that mercury in C. yacare is accumulated in other organs, not included in this study; but it is also possible that physiological mechanisms are involved that help caimans get rid of ingested mercury, or simply that C. yacare’s diverse diet reduces THg accumulation. Carnivorous fishes (Pygocentrus nattereri, Pseudoplatystoma tigrinum, Zungaro zungaro, Plagioscion squamosissimus, and Leiarius marmoratus) had the highest total mercury concentrations, ranging from 0.35 to 1.27 μg g−1Hg w.w. moreover, most were above the limit recommended by WHO (0.5 μg g−1Hg w.w.); except for Leiarius marmuratus, which presented a mean of 0.353 ± 0.322 μg g−1Hg w.w. The two non-carnivorous fish species (Prochilodus nigricans, and Piaractus brachypomus) present mean concentrations of 0.099 ± 0.027, and 0.041 ± 0.019 μg g−1Hg w.w., respectively. Finally, recommendations on the consumption habits of Tacana communities are discussed.

Pesticide exposure is hypothesized as one of the risk factors for the development of childhood brain tumors (CBT). This hospital-based case–control study evaluated the association of pyrethroid pesticide exposure with the risk for CBT in a children population in East China. In total, 161 CBT cases and 170 controls were recruited from 2 children's medical centers in Shanghai (Xinhua Hospital and Shanghai Children's Medical Center) between September 2012 and June 2015. The cases and controls were matched for age, sex, and province of residence. Pyrethroid pesticide exposure was evaluated by urinalysis of 3 nonspecific metabolites of pyrethroids (cis-DCCA, trans-DCCA, and 3-PBA) using gas chromatography-mass spectrometry (GC-MS) detection and by administering a questionnaire. Unconditional logistic regression showed that trans-DCCA, 3-PBA, and total metabolites (sum of the 3 metabolites) were positively associated with the increased risk of CBT. Children in the highest quartile had a nearly 3-fold increased risk of CBT compared with those in the lowest quartile after adjusting for confounding factors (trans-DCCA, odds ratio (OR) = 2.58, 95% confidence interval (CI), 1.38–4.80, p = 0.003; 3-PBA, OR = 3.26, 95% CI, 1.73–6.14, p < 0.0001; total metabolites, OR = 3.60, 95% CI, 1.87–6.93, p < 0.0001). We also found that exposure to both mosquitocide and cockroach killer was related to the increased risk of CBT (mosquitocide, OR = 1.68, 95% CI, 1.06–2.67, p = 0.027; cockroach killer, OR = 1.83, 95% CI, 1.13–2.95, p = 0.013). These findings indicate that exposure to pyrethroid pesticides might be associated with increased risk of CBT. Prospective cohort studies with larger sample sizes are required to confirm this conclusion.
