Studies on the human body burden of dibenzo-p-dioxins and dibenzofurans (PCDD/Fs) in populations around municipal solid waste incinerators (MSWIs) in China are limited. The objective of this study was to assess the potential adverse health effects of an 8-year MSWI on the surrounding population and identify possible exposure pathways. We hypothesized that the MSWI would result in different environmental impacts and population health outcomes between upwind and downwind of its 3 km vicinity. We conducted a 10-year retrospective mortality survey on the population surrounding the MSWI. Then, we selected 50 residents aged 50 years or older on each of the upwind and downwind sides of MSWI to test serum PCDD/Fs. Meanwhile, environmental and food exposures to PCDD/Fs were tested for selected residents. The age-adjusted mortality rates were significantly higher for residents downwind than upwind, but no significant difference was found in the standardized mortality ratio before and after the MSWI operation. The toxic equivalents (TEQ) and major congeners of PCDD/Fs were significantly higher in the sera of the downwind residents than in the upwind. The PCDD/Fs in air, soil, dust, and vegetables on the downwind side were not significantly different from those on the upwind side, but the mean concentrations of PCDD/Fs in downwind hen eggs was significantly higher than those from upwind. In conclusion, downwind residents living within 3 km of the MSWI had higher age-adjusted mortality and serum level of PCDD/Fs than upwind residents. This higher mortality rate among downwind residents was not associated with MSWI. However, the higher levels of PCDD/Fs in downwind hen eggs suggest that the downwind population dioxin exposure was related to their location.

Contaminant vulnerability in the critical zones like groundwater (GW)-seawater (SW) continuum along the entire Gujarat coast was investigated for the first time through an extensive water monitoring survey. The prime focus of the study was to evaluate whether or not: i) seawater intrusion induced metal load translates to toxicity; ii) in the coastal groundwater, metal distribution follows the pattern of other geogenic and anthropogenic contaminants like NO₃- and F-; and iii) what future lies ahead pertaining to metal fate in association with saturation conditions of the coastal aquifers. The spatial distribution of contaminants depicts that the Gulf of Khambhat area is highly contaminated. Ecological risk assessment (ERA) indicates that the Gujarat coast is experiencing a high ecological risk compared to the southeast coast of India. Investigation results revealed that metals, pH, NO₃, and CO₃ are more vulnerable at the SW-GW mixing interface. An increase in pH is reflected in fewer ionic species of metals in the GW. Salinity ingress due to seawater intrusion (SWI) reduces the toxicities of all trace metals except Cu, attributed to the increase of Ca in GW, leading to dissociation of CuCO₃. Reactive species are dominant for Zn and Cd; and M-CO₃ ligands are dominant for Cu and Pb owing to the undersaturation of dolomite and calcite in the aquifer system. SWI tends to increase the metal load but the toxicity of metals varies with the density of industries, anthropogenic activities, changes in the mixing-induced saturation conditions, and intensive salt production across the coast. Multivariate analysis confirmed that the hydrogeochemical processes change due to GW-SW mixing and dictates over natural weathering. The ecological risk index (ERI) for the Arabian sea is experiencing moderate (300 ≥ ERI>150) to high ecological risk (ERI >600). Children population is likely to encounter a high health risk through ingestion and dermal exposure than adults. Overall, the study emphasizes the complexity of toxicity-related health impacts on coastal communities and suggests the dire need for frequent water monitoring along the coastal areas for quick realization of sustainable development goals.

Eighty eight adult gardeners and their relatives volunteered to provide urine and blood samples for a human biomonitoring survey among users of one of the biggest allotment garden from Wallonia, showing high trace metal(oid) concentrations in soils. The purpose was to determine if environmental levels of lead (Pb), cadmium (Cd) and arsenic (As) led to concentrations of potential health concern in the study population. Blood and urine biomarkers were compared to reference and intervention cut-off values selected from the literature. The study population exhibited (i) moderately high blood lead levels with median value of 23.1 μg/L, (ii) high urinary concentrations of speciated As (inorganic arsenic and its metabolites) with a median value of 7.17 μg/g.cr., i.e. twice the median values usually observed in general populations, and (iii) very high Cd levels in urine with a median value of 1.23 μg/L, in the range of 95th-97.5th percentiles measured in general adult populations. Biomarker levels in the study population were also mostly above those measured in adults from local populations living on contaminated soils, as reported in the current literature. All biomarkers of Pb, Cd and As showed weak to strong statistically significant correlations, pointing towards a joint environmental source to these three contaminants as being at least partially responsible for the high exposure levels observed. Urine and blood biomarkers show statistically significant associations with variables related to individual characteristics (age, smoking status, …) and Pb domestic sources (Pb pipes, cosmetics, …) but involves also behavioral and consuming habits related to gardening activities on the contaminated allotment garden. At such levels, owing to co-exposure and additive effects of Cd, As and Pb regarding renal toxicity known from literature, the study strongly suggests that this population of gardeners is at risk with respect to chronic kidney diseases.

Metals may affect adversely cardiovascular system, but epidemiological evidence on the associations of priority-controlled metals including antimony (Sb), arsenic (As), cadmium, lead, and thallium with children's blood pressure (BP) was scarce and inconsistent. We conducted two panel studies with 3 surveys across 3 seasons among 144 and 142 children aged 4–12 years in Guangzhou and Weinan, respectively. During each seasonal survey, urine samples were collected for 4 consecutive days and BP was measured on the 4th day. We obtained 786 BP values and urinary metals measurements at least once within 4 days, while 773, 596, 612, and 754 urinary metals measurements were effective on the health examination day (Lag 0), and the 1ˢᵗ, 2ⁿᵈ, and 3ʳᵈ day preceding BP measurement (Lag 1, lag 2 and lag 3), respectively. We used linear mixed-effect models, generalized estimating equations and multiple informant models to assess the associations of individual metal at each lag day and accumulated lag day (4 days averaged, lag 0–3) with BP and hypertension, and Bayesian Kernel Machine Regression to evaluate the relations of metals mixture at lag 0–3 and BP outcomes. We found Sb was positively and consistently related to systolic BP (SBP), mean arterial pressure (MAP), and odds of having hypertension within 4 days, which were the strongest at lag 0 and declined over time. And such relationships at lag 0–3 showed in a dose-response manner. Meanwhile, Sb was the only contributor to the relations of mixture with SBP, MAP, and odds of having hypertension. Also, synergistic interaction between Sb and As was significant. In addition, modification effect of passive smoking status on the association of Sb and SBP was more evident in passive smokers. Accordingly, urinary Sb was consistently and dose-responsively associated with increased BP and hypertension, of which Sb was the major contributor among children.

Growing epidemiological evidence has shown that exposure to ambient air pollution contributes to poor sleep quality. However, whether variability in air pollution exposure affects sleep quality remains unclear. Based on a large sample in China, this study linked individual air pollutant exposure levels and temporal variability with subjective sleep quality. Town-level data on daily air pollution concentration for 30 days prior to the survey date were collected, and the monthly mean value, standard deviations, number of heavily polluted days, and trajectory for six common pollutants were calculated to measure air pollution exposure and its variations. Sleep quality was subjectively assessed using the Pittsburgh Sleep Quality Index (PSQI), and a PSQI score above 5 indicated overall poor sleep quality. Multilevel and negative control models were used. Both air pollution exposure and variability contributed to poor sleep quality. A one-point increase in the one-month mean concentration of particulate matter with aerodynamic diameters of ≤2.5 μm (PM₂.₅) and ≤10 μm (PM₁₀) led to 0.4% (95% confidence interval (CI): 1.002–1.006) and 0.3% (95% CI: 1.001–1.004) increases in the likelihoods of overall poor sleep quality (PSQI score >5), respectively; the odds ratios of a heavy pollution day with PM₂.₅ and PM₁₀ were 2.2% (95% CI: 1.012–1.032) and 2.2% (95% CI: 1.012–1.032), respectively. Although the mean concentrations of nitrogen dioxide, sulfur dioxide, and carbon monoxide met the national standard, they contributed to the likelihood of overall poor sleep quality (PSQI score >5). A trajectory of air pollution exposure with maximum variability was associated with a higher likelihood of overall poor sleep quality (PSQI score >5). Subjective measures of sleep latency, duration, and efficiency (derived from PSQI) were affected in most cases. Thus, sleep health improvements should account for air pollution exposure and its variations in China under relatively high air pollution levels.
