The clinical presentation of lumbosacral disease (LS) may be different from other spinal cord syndromes because manifestations of both central (lumbal and sacral spinal cord segments) and peripheral (cauda equina nerves) neurological abnormalities may be observed. Typical signs in the former include lower motor neuron (LMN) paresis or paralysis and in the latter tail paralysis and urinary and/or fecal incontinence. Information from the history of patient regarding in particular the time of onset and the type of sign progression is helpful in determining the potential causes of LS disease. The neurological examination helps to identify clinical signs and leads to the establishment of the lesion localization. Differential diagnosis is outlined taking into consideration the lesion localization, history and signalment of the animal and using disease categories outlined by the acronym VITAMIN-D. The diagnostic workup is planned according to the most likely etiologies and includes a minimum data base of laboratory diagnostic tests, imaging, cerebrospinal fluid analysis and even electrophysiologic testing.
