Treatment of onychomycosis with amorolfine nail lacquer

Despite the availability of various treatment options ranging from topical and systemic antifungal therapy to surgical or chemical avulsion, the first line therapy for onychomycosis is still topical treatment. However, in onychomycosis of moderate severity, topical treatment are generally ineffective because of poor penetration of the drugs. To improve the efficacy of tropical treatment, transungual delivery systems have been developed to enhance the penetration of antifungals through the nail keratin. One of these is amorolfine 5 percent nail lacquer which is a broad spectrum antifungal and acts by inhibiting ergosterol biosynthesis in the membrane of fungi. Results from in vitro studies show it has fungicidal activity against medically important fungi. Amorolfine is recommended to be applied once or twice a week for 6-12 months

Terapi topikal tetap merupakan pilihan utama dalam pengobatan onikomikosis walaupun berbagai macam terapi telah tersedia seperti terapi sistemik, pembedahan maupun avulsi kimia. Namun demikian, pada kasus onikomikosis sedang sampai berat, terapi topikal tidak efektif karena penetrasi obat kedalam kuku kurang baik. Untuk memperbaiki efikasi terapi topikal dikembangkan transungual delivery system agar obat dapat menembus lapisan keratin kuku yaitu amorolfin 5 persen cat kuku. Amorolfin 5 persen cat kuku adalah antijamur berspektrum luas yang bekerja dengan menghambat biosintesis ergosterol pada membran sel jamur. Hasil penelitian in vitro menunjukkan bahwa amorolfin bersifat fungisidal terhadap jamur yang penting untuk kedokteran. Amorolfin dipakai 1-2 kali seminggu selama 6-12 bulan
