Abstract: A comprehensive technical literature review summarizes and discusses research findings concerning lactose and milk intolerance. Specifically, attention is focused on the nature and incidence of lactase enzyme deficiency as it relates to the clinical and behavioral symptoms of lactose and milk intolerance to provide a better understanding of the public health implications, particularly with respect to the emphasis on milk consumption in US domestic feeding programs. Topics include the pathophysiology of altered lactose absorption; diagnostic tests; the etiology and hypotheses of lactase deficiency; the prevalence of lactase deficiency and lactose malabsorption and intolerance and their effects on milk consumption and dietary intakes; the significance of lactase deficiency in the milk consumer, including the bioavailability of specific nutrients and long-term consequences; and some of the more important public health consequences.
