Dietary treatment of anorexia nervosa

The details of an outpatient treatment program suitable for those with anorexia nervosa are presented. The approach is predicated upon valid nutritional and psychological principles and encourages the resumption of normal eating patterns, with the patient maintaining control and responsibility for restoration of normal weight. Elements of the treatment plan described include: principles of treatment (involves joint efforts of a physician and a dietitian); preparation for treatment (selection of patients based on motivation); the goals of dietary treatment (to re-establish a normal eating pattern); the role of the diet history; caloric content of initial diet (based on basal kilocalorie requirements for present height, weight, age, and sex); the diet plan design (includes adequate nutrient content); diet progression (highly individualized); weight gain expectations (stopping weight losses primary objective); and the diet plan for weight maintenance (based on goal weight). This educational approach, emphasizing moderation, variety and regularity in eating, offers an alternative to hospitalization of the anorexia nervosa patient. (kbc)
