Hyperammonemia in neonates receiving intravenous nutrition

Blood ammonia (BA) concentrations in 1 group of 13 sick neonates on intravenous nutrition (IVN) receiving a commercial amino acid (AA) solution (containing an apparent sub-optimal arginine content) were compared with those of a second group of 13 sick neonates receiving a different AA solution (containing an adequate arginine content). Both solutions elevated BA levels during IVN, relative to pretreatment BA levels. Further, higher BA levels were more readily induced in neonates (especially septic neonates) receiving the lower arginine content AA solution. Other factors examined (birth weight, gestational age, oxygen requirements, congenital defects, heart disease, enteral nutritional intake) did not appear to correlate with the differences in BA levels between the 2 infant groups.
