Kollagenkonstrukte mit autologen Zellen im Sinuslift am Schaf | Collagen scaffolds with autologous cells for sinuslift in a sheep model

Knochenaufbauten im zahnlosen Oberkiefer sind eine der Hauptindikationen für Biomaterialien in der präprothetischen Chirurgie. Durch die zunehmende Alterung der Gesellschaft gewinnt dieses Problem an Relevanz. In der vorliegenden Arbeit ist in einer in vivo Studie zur Sinusbodenaugmentation am Schafmodell ein Biomaterial geprüft worden. Getestet wurde ein Konstrukt aus einer Kombination von Kollagen mit Hydroxylapatit und autologen Osteoblasten. Um einen intraindividuellen Vergleich zu ermöglichen, wurde ein Splitmouth- Model eingesetzt. Auf der Testseite wurde das Material mit autologen Osteoblasten besiedelt, die aus einer Knochenbiopsie in 6-8-wöchiger Zellkultur gezüchtet wurden. Als Kontrolle diente das Biomaterial ohne Zellen. Simultan wurden dentale Implantate eingebracht. Vitalfarbstoff-Markierungen erfolgten in den Zeiträumen 5-6 und 1-2 Wochen vor Euthanasie. Jeweils 3 der insgesamt 9 Tiere wurden nach 8, 16 und 24 Wochen euthanasiert. Mit der CT wurde das Volumen der Augmentate ermittelt und eine Volumenreduktion um bis zu 83 % des Ausgangsvolumens festgestellt. Die Augmentate und der angrenzende spongiöse Knochen wiesen auf der Testseite eine signifikant geringere Dichte auf, als auf der Kontrollseite. Die Knochenneubildung am Implantat und die Osseointegration wurden histomorphometrisch bestimmt. Eine bessere Osseointegration der Implantate konnte auch bei der Verwendung von Zellen nicht festgestellt werden. Ebenso fand keine Beschleunigung der Knochenneubildung durch die Vitalisierung mit kultivierten autologen Osteoblasten in klinisch relevantem Ausmaß statt. Das Material hat sich als biokompatibel, aber nicht als volumenstabil erwiesen, so dass es sich in seiner jetzigen Zusammensetzung nicht für große Knochenaufbauten empfiehlt. Wegen der unklaren Wirkung der autologen Osteoblasten muss hinterfragt werden, ob der erhöhte Aufwand des Tissue Engineering mit Zellen in dieser Form einen sinnvollen Ansatz darstellt. Für kleinere Augmentationen im Bereich primär- stabiler Implantate können Kollagenkonstrukte eine Alternative werden.

The augmentation of bone in the edentulous maxilla is one of the main indications for bio-materials in preprothetic surgery. This problem gains in importance because of the increasing age of society. This thesis describes an in vivo study for sinus floor bone augmentation in the sheep model. Bio- material made out of a combination from collagen and hydroxyapatite was tested. A split-mouth model was used to allow intra-individual comparison. On the test side, the material was seeded with autologous osteoblasts, cultured out of a bone biopsy in 6 to 8 weeks of cell culture. Bio-material without cells was used for control. Dental implants were inserted simultaneously. Over the period of week 5 to 6 and week 1 to 2 before euthanasia, vital dyes were applied to mark the newly formed bone. Out of the total of nine animals, three at a time were euthanized after 8, 16 and 24 weeks. The volume of augmented material was ascertained by means of CT-exposures. Subsequently the volume was reduced by up to 83 % of the volume on the outset. There was a significantly lower density in the augmented area and the adjacent cancellous bone on the test side compared to the control side. Osseointegration, new growth of bone on the implant, was defined histomorphometrically. Accelerated growth of new bone and an improved osseointegration of the implants by vitalisation with cultivated, autologous osteoblasts did not occur. The material has proven itself as biocompatible but not as stable in volume, so that it does not recommend itself for a larger augmentation of bone structures in its present composition. The effect of cultured osteoblast-like cells is questionable. For smaller augmentations in the area of primarily stable implants, collagen constructs may constitute an alternative.
