Isolierte Verschattungen dorsal im Fesselgelenk | ein Vergleich zwischen Lahmheitsuntersuchung und Kaufuntersuchung | Bony fragments dorsal in the fetlock joint | a comparison between the lameness examination and the prepurchase examination

In der vorliegenden retrospektiven Studie werden einerseits das Vorkommen und die Verbreitung isolierter Verschattungen dorsal im Fesselgelenk beim Pferd untersucht und andererseits die klinische Bedeutung eruiert. Es werden hierzu zwei Patientengruppen gebildet. Eine Gruppe beinhaltet Pferde, die zur Lahmheitsuntersuchung und die andere Gruppe Pferde, die zur Kaufuntersuchung vorgestellt werden. Voraussetzung zur Teilnahme der Patienten an der Studie ist bei der Kaufuntersuchung das Vorliegen der Röntgenbilder aller Zehen im seitlichen-90° Strahlengang und des Untersuchungsprotokolls. Bei der Lahmheitsuntersuchung ist die Voraussetzung das Vorliegen mindestens eines Röntgenbildes der Zehe im seitlichen-90° Strahlengang und einer diagnostischen Anästhesie. Insgesamt werden bei der Kaufuntersuchung die Röntgenbilder von 8792 Zehen beurteilt. Isolierte Verschattungen dorsal im Fesselgelenk sind bei 422 (4,8 %) der Fesselgelenke vorhanden. Bezogen auf die Gesamtzahl der Tiere von 2198 sind dies 341 (15,5 %) Pferde mit isolierter Verschattung dorsal im Fesselgelenk. Von 8792 durchgeführten Übersichtsbeugeproben sind 588 (6,7 %) gering- bis mittelgradig positiv. An den Gliedmaßen mit isolierter Verschattung sind 101 (23,9 %) von 422 Übersichtsbeugeproben gering- bis mittelgradig positiv. Bezogen auf die Gesamtzahl aller positiven Beugeproben machen Gliedmaßen mit isolierter Verschattung einen Anteil von 17,2 % aus. Bei der Lahmheitsuntersuchung liegen nur etwa ein Drittel aller Zehen zur Begutachtung vor. 278 (9%) der 3101 beurteilten Fesselgelenke enthalten isolierte Verschattungen. Die 278 Gelenke gehören zu 258 Pferden. Insgesamt haben 258 (11,2 %) der 2296 Pferde eine isolierte Verschattung im Fesselgelenk. Bei 70 (25,2 %) bzw. 76 (27,4 %) von 278 Fesselgelenken mit isolierter Verschattung ist eine Fesselgelenklahmheit, festgestellt mithilfe diagnostischer Injektionen, vorhanden bzw. nicht auszuschließen. Bezogen auf die Patienten sind das 64 (24,8 %) bzw. 70 (27,1 %) von 258 Pferden. Das Verhältnis von klinisch unauffälligen zu möglicherweise klinisch relevanten isolierten Verschattungen liegt bei etwa 3:1 bis 4:1. Dies lässt die Vermutung zu, dass auch in der Population die klinisch unauffälligen isolierten Verschattungen häufiger vorkommen.

In the present study on the one hand the incidence and the prevalence of bony fragments dorsally in the fetlock joint is examined and on the other hand the clinical meaning is investigated. Two patient's groups are formed moreover. One group contains lame horses which are introduced for lameness examination and the other group horses introduced for the prepurchase examination. Condition to the participation of the patients in the study with the purchase examination is the presentation of the x-ray pictures of all toes in lateral 90 ° ray way and the examination protocol. With the lameness examination the condition is the presentation at least of one x-ray picture of the toe in lateral 90 ° ray way and a diagnostic anaesthesia. All together the x-ray pictures are judged with the purchase examination by 8792 toes. Bony fragments dorsally in the fetlock joint exist with 422 (4.8%) of the fetlock joints. Covered to the total number of animals of 2198 there are 341 (15.5%) horses with bony fragments dorsally in the fetlock joint. From 8792 carried out flection tests 588 (6.7%) are positive slightly to moderately. In the limbs with bony fragments in the fetlock joint 101 (23.9%) of 422 flection tests are positive slightly to moderately. If covered to the total number of all positive flection tests limbs with bony fragments put out an interest of 17.2%. With the lameness examination only possibly one third of all toes are for the survey. 278 (9%) of 3101 fetlocks contained bony fragments dorsally. These 278 joints belong to 258 horses. All together 258 (11.2%) of 2296 horses have a bony fragment dorsally in the fetlock joint. With 70 (25.2%) or 76 (27.4%) of 278 fetlocks with a bony fragment dorsally a lameness, ascertained with the help of diagnostic injections, exists or is not to exclude. Covered to the patients these are 64 (24.8%) or 70 (27.1%) of 258 horses. The relation of clinically unobtrusive to possibly clinically relevant bony fragments lies with about 3:1 to 4:1. The supposition to that is that also in the population the clinically unobtrusive bony fragments seem more often.
