Leishmaniose visceral em um cão

A leishmaniose visceral se apresenta como uma enfermidade infecciosa emergente e com altos índices de letalidade, classificada como uma zoonose, doença transmitida tanto aos animais quanto aos seres humanos. Sua transmissão ocorre através da picada de fêmeas infectantes de flebotomíneos, em especial das espécies Lutzomyia longipalpis e Lutzomyia cruzi, que carregam o agente etiológico Leishmania chagasi. O cão doméstico é o principal reservatório da infecção, também sendo acometido pela enfermidade. O objetivo do presente trabalho é relatar um caso de leishmaniose visceral canina (LVC) em um cão, enfatizando os sinais clínicos, os métodos diagnósticos utilizados, o tratamento e a evolução do paciente. Foi atendido em uma clínica veterinária particular, um cão macho, com 4 anos e 4kg, com queixa de alterações dermatológicas crônicas. No exame físico observou-se pele xerótica, alopecia generalizada, prurido intenso e redução na produção lacrimal. Em sua primeira consulta, os exames clínico e físico levaram a suspeita de LVC; diante disso foram solicitados exames laboratoriais como: hemogramas, Polimerase Chain Reaction (PCR), Reação de Imunofluorescência Indireta (RIFI), imuno-histoquímica e biópsias de pele que confirmaram o diagnóstico. Assim, foi dado início ao tratamento do paciente para o controle da doença e da qualidade de vida, apresentando uma melhora significativa e visível ao longo dele. O tratamento prescrito foi composto pelas seguintes medicações: prednisona, alopurinol, domperidona, miltefosina, cefalexina, tacrolimus, itraconazol, complexos vitamínicos e vacina recombinante contra leishmaniose. Com o presente relato podemos concluir que com o correto diagnóstico e tratamento, juntamente com a dedicação dos tutores, é possível que um cão soropositivo tenha uma vida digna, respeitando todos os protocolos vigentes.

Visceral leishmaniasis (VL) presents itself as an emerging infectious disease with high rates of lethality, classified as a zoonosis, a disease transmitted to animals and people. Its transmission occurs through the bite of infective females of sandflies, especially Lutzomyia longipalpis and Lutzomyia cruzi, which carry the etiological agent Leishmania chagasi. The domestic dog is the main reservoir of the infection, also being affected by the disease. The objective of the present work is to report a case of Visceral leishmaniasis canina (CVL) in a dog, emphasizing the clinical signs, the diagnostic methods used, the treatment and the evolution of the patient. In a private veterinary clinic, a male dog, aged 4 years and weighing 4 kg, complained of chronic dermatological changes. On physical examination, xerotic skin, generalized alopecia, intense itching and reduced tear production were observed. At their first consultation, clinical and physical examinations led to a suspicion of CVL; therefore, laboratory tests were requested, such as: complete blood count, Polymerase Chain Reaction (PCR), Indirect Immunofluorescence Reaction (RIFI), immunohistochemistry and skin biopsies that confirmed the diagnosis. Thus, the patient's treatment was started to control the disease and the animal's quality of life, showing a significant and visible improvement throughout the treatment, with a good response to the protocol used. The prescribed treatment consisted of the following medications: prednisone, allopurinol, domperidone, miltefosine, cephalexin, tacrolimus, itraconazole, vitamin complexes and recombinant vaccine against leishmaniasis. With this report we can conclude that with the correct diagnosis and treatment, together with the dedication of the tutors, it is possible for a seropositive dog to have a dignified life, respecting all the current protocols.
