The midwife in the British health care system has the primary responsibility of establishing, managing, promoting, and educating the pregnant woman in breast feeding. The proper care and preparation of the nipples, the physiology of lactation, the importance of cuddling, breast engorgement, sucking, adequate rest, the let down reflex, expectations, management of domestic commitments, and flexibility in feeding routines are topics taught prenatally by the midwife. After the delivery, the midwife provides professional support for the new mother and infant until successful feeding is established.
