Vergleichende Untersuchung zur Wirksamkeit unterschiedlicher Behandlungsprotokolle mit Prostaglandin F2α im Puerperium bei Milchkühen | Comparison of the efficacy of different treatment protocols with prostaglandin F2α in the postpartum period of dairy cows

Ein ungestörtes Puerperium ist eine Voraussetzung für eine gute Fruchtbarkeit. Zwei Injektionen von PGF2α im Abstand von 14 Tagen wird in der tierärztlichen Praxis zur Östrusinduktion und zur Reinigung des Uterus im Sinne eines best practice Vorgehens im Puerperium genutzt. In einer zu Beginn dieser Arbeit aktuellen Studie zeigte eine zweimalige PGF2α Injektion im Abstand von 8 Stunden einen positiven Einfluss auf die Fruchtbarkeit bei Tieren mit unterschiedlichen Puerperalstörungen. Ein Vergleich dieses Vorgehens mit dem üblichen best practice Protokoll ist in der Literatur noch nicht beschrieben worden. Dafür wurden zwei Untersuchungen durchgeführt. In der ersten Untersuchung wurden zwei Behandlungsprotokolle mit Prostaglandin F2α mit einem existierenden Standardprotokoll verglichen. Das Ziel dieser randomisierten Non-Inferiority-Studie war es, die Auswirkungen auf die Fruchtbarkeit zu untersuchen. Betrachtet wurden dabei der Erstbesamungserfolg und die Wahrscheinlichkeit einer Trächtigkeit bis zum 200. Laktationstag. Insgesamt wurden 1.286 Milchkühe in die Studie aufgenommen und randomisiert einer von drei Behandlungsgruppen zugeordnet. Tiere in Gruppe I (n = 362) erhielten PGF2α im Abstand von 14 Tagen an Tag 20 (± 3) und an Tag 34 (± 3) postpartum. Kühe der Gruppe II (n = 358) bekamen zwei Injektionen von PGF2α an Tag 20 (± 3) im Abstand von 8 Stunden und eine Injektion von PGF2α an Tag 34 (± 3) postpartum. Die Tiere in Guppe III (n = 369) erhielten nur an Tag 20 (± 3) postpartum zweimal im Abstand von 8 Stunden PGF2α. Eine Puerperalkontrolle wurde an Tag 41 (± 3) postpartum durchgeführt. Alle Tiere mit einer Endometritis 2. (Anteil eitriger Bestandteile im Vaginalsekret über 50%) und 3. Grades (100% eitriges vaginalsekret) wurden mit 500 mg Cephapirin intrauterin behandelt, 7 Tage später erneut untersucht und gegebenenfalls nochmal behandelt. Für die abschließende Auswertung standen 1.089 Tiere (408 primipare Kühe, 37,4%; 681 multipare Kühe, 62,6%) zur Verfügung. Bei der Puerperalkontrolle an Tag 41 (± 3) waren numerisch (P = 0,06) mehr Tiere mit einem VDS von zwei und drei in Gruppe III (15,2%) als in Gruppe II (11,0%) und Gruppe I (9,8%). Die Behandlung mit PGF2α hatte keinen Einfluss auf den Erstbesamungserfolg (Gruppe I: 34,5%, Gruppe II: 33,4%, Gruppe III: 37,0%) und die Wahrscheinlichkeit einer Trächtigkeit bis zum 200. Laktationstag. Die Konzentrationen von ß–hydroxybutyrat an den Tagen 20 und 41 postpartum hatten keinen Einfluss auf den Erstbesamungserfolg und die Wahrscheinlichkeit einer Trächtigkeit innerhalb von 200 Laktationstagen. Der Temperatur-Feuchte Index zum Zeitpunkt der Besamung hatte einen signifikanten Einfluss (P < 0,01) auf den Erstbesamungserfolg. In der Hauptuntersuchung wurde auf eine unbehandelte Kontrollgruppe verzichtet, um mögliche negative Auswirkungen bei Tieren mit Endometritis zu vermeiden. Deshalb ist keine Aussage möglich, ob PGF2α bei der Behandlung sowohl von kranken, als auch von gesunden Kühen im Puerperium überhaupt einen positiven Effekt auf die Fruchtbarkeit der Tiere hat. Um diese Frage zunächst in einer Pilotstudie nachzugehen, wurde eine zweite Untersuchung durchgeführt. In dieser wurde eine Behandlungsgruppe (n = 112), welche eine zweimalige PGF2α-Injektion im Abstand von 14 Tagen erhielt (Tag 20 (± 3) und Tag 34 (± 3) postpartum) mit einer unbehandelten Kontrollgruppe (n = 114) verglichen. Interessanterweise traten keine Unterschiede bezüglich der in der Hauptuntersuchung betrachteten Fruchtbarkeitsparameter zwischen der behandelten Versuchsgruppe und unbehandelten Kontrollgruppe auf. Die Stichprobe (n = 226) ist jedoch zu klein um eine statistisch gesicherte Schlussfolgerung zu ziehen. Weitere Untersuchungen sind notwendig um die aufgestellten Hypothesen zu belegen.

An undisturbed postpartum period is required for optimum fertility of dairy cows or reproductive performance of a herd, respectively. Two injections of prostaglandin F2α 14 days apart is considered best practice and commonly used in the dairy industry to induce estrus and cleaning of the uterus in the postpartum period. More recently a protocol implementing two PGF2α injections 8 hours apart demonstrated a positive influence on fertility in animals suffering from multiple reproductive disorders after calving. A direct comparison with the best practice protocol, however, was not yet described in the relevant literature. Therefore, two studies were conducted. In the main study two treatment protocols for prostaglandin F2α were compared with the existing standard care. The objective of this randomized non-inferiority clinical trial was to compare the efficacy of 3 different prostaglandin F2α treatment protocols as measured by pregnancy per artificial insemination (P/AI) at the first AI and time to pregnancy within 200 DIM. A total of 1.268 lactating Holstein dairy cows from 4 commercial dairy farms were allocated randomly at 3 d in milk (DIM) to 1 of 3 treatment protocols. Cows in group I (n = 362) received two injections of PGF2α at 20 (± 3) and at 34 (± 3) days in milk (DIM). Cows in group II (n = 358) received two injection of PGF2α at 20 (± 3) DIM at an interval of eight hours and a third injection at 34 (± 3) DIM. Cows in group III (n = 369) received two injections of PGF2α at 20 (± 3) DIM at an interval of eight hours. Uterine health was examined at day 41 (± 3) postpartum. All cows with a purulent vaginal discharge score of two (i.e., > 50 per cent purulent vaginal discharge) and three (100 per cent purulent vaginal discharge) were treated with 500 mg Cephapirin intrauterine. Seven days later they were re-examined and if necessary re-treated. The final analysis included 1,089 animals (408 primiparous cows, 37,4%; 681 multiparous cows, 62.6%). At day 41 (± 3) there were more (P = 0,06) cows with a purulent vaginal discharge score of two and three in group III (15,2%) compared to group II (11,0%) and group I (9,8%). There was no influence of the treatment with PGF2α P/AI for the first service (group I 34,5%, group II 33,4%, group III 37,0%) and the probability for pregnancy within 200 DIM. Similarly, there was no effect of the concentration of ß-hydroxybutyrat at days 20 and 41 postpartum on the P/AI for the first service and the risk for pregnancy within 200 DIM. There was, however, a significant influence of the temperature- humidity-index at first service on the P/AI for the first service (P < 0,01). The main study was performed without an untreated control group to avoid plausible negative effects on cows with purulent vaginal discharge excluding ethical issues of an untreated control group and limiting potential financial losses to the dairy farms. Due to this approach a final conclusion whether the treatment with PGF2α has a positive influence on reproductive performance was not possible. Therefore, a second study was conducted to address this question. This trial was conducted as a pilot trial including two groups with a limited number of cows. Cows in the treatment group (n = 112) received two injections of PGF2α at an interval of 14 days (day 20 (± 3) and day 40 (± 3) postpartum) and the cows in the control group (n = 114) remained untreated. Interestingly, there were no differences of reproductive performance between treatment group and control group. As sample size was limited (i.e. 226 cows), this study was lacking power and the results must be taken with caution. However, these results are encouraging to perform more research to corroborate the hypothesis.
