Electromyography of the pelvic diaphragm and anal sphincter in dogs with perineal hernia.

The innervation of the levator ani and coccygeal muscles and the external anal sphincter was studied by anatomic dissection in 6 clinically normal male dogs and by electrical stimulation in 5 clinically normal male dogs. Variations in innervation occasionally were found that were comparable to those reported in previous studies. Electromyographic recordings were made from the levator ani and coccygeal muscles and from the anal sphincter in 40 dogs during perineal hernia repair. Spontaneous potentials of 4 types were found in 35 dogs: fibrilation potentials, positive sharp waves, complex repetitive discharges, and fasciculations. Biopsy specimens of the cranial part of the levator ani muscle were taken in 12 dogs during perineal hernia repair. Histologic examination revealed atrophy in 7 specimens. Spontaneous potentials were recorded from all muscles with histologic evidence of atrophy. All examinations of the levator ani muscle concerned the cranial, part of this muscle, because the caudal part was absent in all 40 dogs. From combined results of electromyography and histologic examination, it was concluded that atrophy of the muscles of the pelvic diaphragm, which develops in some dogs with perineal hernia, is likely to be of neurogenic origin. Nerve damage is localized in the sacral plexus proximal to the muscular branches of the pudendal nerve or in the muscular branches separately.

OBJECTIVE: To assess the analgesic efficacy of an IV constant rate infusion (CRI) of a morphine-lidocaine-ketamine (MLK) combination in calves undergoing umbilical herniorrhaphy. ANIMALS: 20 weaned Holstein calves with umbilical hernias. PROCEDURES: Calves were randomly assigned to receive a CRI of an MLK solution (0.11 mL/kg/h; morphine, 4.8 μg/kg/h; lidocaine, 2.1 mg/kg/h; and ketamine, 0.42 mg/kg/h) for 24 hours (MLK group) or 2 doses of flunixin meglumine (1.1 mg/kg, IV, q 24 h) and a CRI of saline (0.9% NaCl) solution (0.11 mL/kg/h) for 24 hours (control group). The assigned CRI was begun after anesthesia induction. A pain-scoring system and incisional algometry were used to assess pain, and blood samples were obtained to measure serum cortisol concentration at predetermined times for 120 hours after CRI initiation. RESULTS: Mean pain scores did not differ significantly between the MLK and control groups at any time. Mean algometry score for the MLK group was significantly greater (calves were less responsive to pressure) than that for the control group at 4 hours after CRI initiation. Mean cortisol concentration decreased over time for both groups and was significantly greater for the MLK group than the control group at 1, 4, and 18 hours after CRI initiation. CONCLUSIONS AND CLINICAL RELEVANCE: A CRI of MLK provided adequate postoperative analgesia to calves that underwent umbilical herniorrhaphy. However, the technical support required for CRI administration limits its use to hospital settings. Kinetic analyses of MLK infusions in cattle are necessary to establish optimal dosing protocols and withdrawal intervals.

A 2-year-old (63 days) pregnant female dachshund dog was presented to the Teaching Veterinary Clinical Complex (TVCC) with complaint of a progressively growing swelling in the caudal ventral abdominal area. History, physical examination, radiography and ultrasonography confirmed an inguinal hystrocoele. A ventral midline incisionon the mass was made to relieve the foetuses and reduction of contents, and herniorrhaphy was performed. Sutures were removed on 7th post-operative day and the animal had an uneventful recovery.

OBJECTIVE To describe the pharmacokinetics of morphine, lidocaine, and ketamine associated with IV administration of a constant rate infusion (CRI) of a morphine-lidocaine-ketamine (MLK) combination to calves undergoing umbilical herniorrhaphy. ANIMALS 20 weaned Holstein calves with umbilical hernias. PROCEDURES Calves were randomly assigned to receive a CRI of an MLK solution (0.11 mL/kg/h; morphine, 4.8 μg/kg/h; lidocaine, 2.1 mg/kg/h; and ketamine, 0.42 mg/kg/h) for 24 hours (MLK group) or 2 doses of flunixin meglumine (1.1 mg/kg, IV, q 24 h) and a CRI of saline (0.9% NaCl) solution (0.11 mL/kg/h) for 24 hours (control group). For all calves, the CRI was begun after anesthesia induction. Blood samples were obtained immediately before and at predetermined times for 120 hours after initiation of the assigned treatment. Noncompartmental analysis was used to estimate pharmacokinetic parameters for the MLK group. RESULTS During the CRI, steady-state serum concentrations were achieved for lidocaine and ketamine, but not morphine. Mean terminal half-life was 4.1, 0.98, and 1.55 hours and area under the concentration-time curve was 41, 14,494, and 7,426 h•μg/mL for morphine, lidocaine, and ketamine, respectively. After the CRI, the mean serum drug concentration at steady state was 6.3, 616.7, and 328 ng/mL for morphine, lidocaine, and ketamine, respectively. CONCLUSIONS AND CLINICAL RELEVANCE During the CRI of the MLK solution, steady-state serum concentrations were achieved for lidocaine and ketamine, but not morphine, likely owing to the fairly long half-life of morphine. Kinetic analyses of MLK infusions in cattle are necessary to establish optimal dosing protocols.

Levator ani and coccygeus muscle estrogen and androgen receptors were measured in 6, healthy, greater than or equal to 5-year-old, noncastrated, male Beagles (controls) and in 24 dogs with perineal hernia. Estrogen and androgen receptor analyses were performed on levator ani and coccygeus muscle specimens obtained from control dogs at the time of castration; contralateral levator ani and coccygeus muscle specimens were assayed 2 months after castration. During herniorrhaphy of dogs with perineal hernia, levator ani (noncastrated, n = 12; castrated, n = 7) and/or coccygeus (noncastrated, n = 5; castrated, n = 4) muscle biopsy specimens were obtained for estrogen and androgen receptor analyses. For estrogen and androgen receptor assays, each muscle biopsy specimen was homogenized in Tris-EDTA-glycerol buffer, and centrifuged at 30,000 X g; extracts were used for binding with ligands: [3H]methyltrienolone (3H-R1881) for androgen receptors, and [3H]estradiol-17 beta for estrogen receptors. Extracts were incubated overnight at 0 to 4 C. Nonspecific binding was estimated, using 100-fold concentration of cold ligands. Bound and free hormones were separated, using hydroxylapatite batch assay. Receptor numbers for each tissue were calculated as femtomoles (fmol) per milligram of protein. Quantified data were compared between precastration and postcastration controls, using a paired t-test. One-way ANOVA and Bonferroni post-hoc test were used to compare values for precastration controls, postcastration controls, castrated dogs with perineal hernia, and noncastrated dogs with perineal hernia. Significance was set at P < 0.05. Estrogen receptors were not detected. Androgen receptors were characterized by Scatchard analysis (dissociation constant = 3.16 to 6.6 nM R1881, receptor number = 23 to 175 fmol/mg of protein). Post castration controls had significantly higher numbers of androgen receptors in levator ani and coccygeus muscles than did precastration controls. Dogs with perineal hernia (castrated and noncastrated) had lower numbers of androgen receptors than did either control group. The paucity of androgen receptors in pelvic diaphragm muscles of dogs with perineal hernia, compared with controls, suggests that decreases in quantity of androgen receptors contribute to the etiopathogenesis of perineal hernia in dogs.

Somatosensory evoked potentials (SEP) were recorded at the scalp and at various levels along the lumbar and caudal thoracic parts of the spine in response to tibial nerve stimulations. The SEP were observed in 24 diseased dogs, 2 with a vertebral fracture, 1 with a spinal cord tumor, 1 with a vertebral tumor, and 20 with disk herniation. Cord compression location was confirmed by myelography, laminectomy, or both. The clinical state had significant (P < 0.0001) influence on SEP characteristics. The scalp-recorded SEP latency changed only in association with the most severe lesions; spine-recorded SEP conduction velocity was lower in association with mild lesions; scalp-recorded SEP amplitude changed with lesions of intermediary severity. Because these 3 electrophysiologic variables were influenced differently by cord damage, it was possible to discriminate the various clinical grades by use of these techniques. However, dogs with signs of pain only could not be differentiated from clinically normal dogs. The evoked injury potential was observed in all but 4 diseased dogs, and its maximal amplitude corresponded, in all cases, with cord damage location. Increased duration (P < 0.05) of the spine-recorded SEP was associated with longstanding problems, but not necessarily with clinically detectable malfunction. Use of SEP and evoked injury potential for identifying lateralized cord damage may be of value.

Age, breed, and gender distributions of 168 horses with umbilical hernia treated at the University of Pennsylvania School of Veterinary Medicine were analyzed to determine risk factors for this disease. For the 3 breeds that constituted the largest proportion of hospital and case populations, Thoroughbred, Standardbred, and Quarter Horse, the overall ratio of females to males was 1.63. In a hospital population of the same age group, 0 to 48 months, the female to male ratio was 0.93. Compared with males, females were at significantly higher risk for umbilical hernia after adjustment for breed and age (odds ratio, 2.01; 99% confidence interval, 1.31 to 3.10; P = 0.00002). Of the 2 major breeds, Thoroughbreds were at greater risk than Standardbreds for umbilical hernia, after adjustment for gender and age (odds ratio, 1.80; 99% confidence interval, 1.10 to 2.95; P = 0.0020). The results provide information about a common congenital defect in horses that can be used for future genetic research.

Data on 5,711 Duroc-sired, 2,227 Landrace-sired, and 2,494 Yorkshire-sired male pigs born over a 9-year period were used to evaluate the genetic influence on scrotal herniation. Differences in frequency of this defect among boar breeds (Duroc, Landrace, and Yorkshire) were significant (P < 0.01). Differences among sires within the Duroc and Landrace boar groups were significant (P < 0.001 and P < 0.05, respectively), but differences within the Yorkshire group were not significant. Frequency of scrotal hernia among male full siblings of affected males was consistently higher than the overall frequency of the defect among progeny in each of their respective breed of boar groups. Percentage of affected pigs among male full siblings of affected males for Duroc, Landrace, and Yorkshire groups, respectively, was 3.0, 3.0, and 2.7 times greater than the overall percentage affected in their respective breed groups. Heritability of susceptibility to scrotal hernia development was estimated to be 0.29 +/- 0.17, 0.34 +/- 0.23, and 0.34 +/- 0.19 in Duroc, Landrace, and Yorkshire-sired pig groups, respectively.
