Occurrence of caffeine in the freshwater environment: Implications for ecopharmacovigilance

Owing to the substantial consumption of caffeinated food, beverages, and medicines worldwide, caffeine is considered the most representative pharmaceutically active compound (PhAC) pollutant based on its high abundance in the environment and its suitability as an indicator of the anthropogenic inputs of PhACs in water bodies. This review presents a worldwide analysis of 132 reports of caffeine residues in freshwater environments. The results indicated that more than 70% of the studies reported were from Asia and Europe, which have densely populated and industrially developed areas. However, caffeine pollution was also found to affect areas isolated from human influence, such as Antarctica. In addition, the maximum concentrations of caffeine in raw wastewater, treated wastewater, river, drinking water, groundwater, lake, catchment, reservoir, and rainwater samples were reported to be 3.60 mg/L, 55.5, 19.3, 3.39, 0.683, 174, 44.6, 4.87, and 5.40 μg/L, respectively. The seasonal variation in caffeine residues in the freshwater environment has been demonstrated. In addition, despite the fact that there was a small proportion of wastewater treatment plants in which the elimination rates of caffeine were below 60%, wastewater treatment is generally believed to have a high caffeine removal efficiency. From a pharmacy perspective, we proposed to adopt effective measures to minimize the environmental risks posed by PhACs, represented by caffeine, through a new concept known as ecopharmacovigilance (EPV). Some measures of EPV aimed at caffeine pollution have been advised, as follows: improving knowledge and perceptions about caffeine pollution among the public; listing caffeine as a high-priority PhAC pollutant, which should be targeted in EPV practices; promoting green design and production, rational consumption, and environmentally preferred disposal of caffeinated medicines, foods, and beverages; implementing intensive EPV measures in high-risk areas and during high-risk seasons; and integrating EPV into wastewater treatment programs.

Parabens, broad-spectrum antimicrobial preservatives widely used in various consumer products and food, are suspected to be linked with several adverse health effects in humans, especially newborn babies, infants, and young children. While human exposure to parabens has been frequently reported by measuring the concentration of parabens in urine, similar measurements in breast milk have rarely been made. To determine paraben concentrations in breast milk and possible sources of exposure, four major parabens, including methylparaben (MP), ethylparaben (EP), propylparaben (PP), and butylparaben (BP) were measured in breast milk samples collected from 260 lactating women in South Korea. Demographic, socioeconomic, and behavioral factors associated with the presence of parabens in breast milk were determined. EP concentrations were detected at the highest levels in breast milk samples, followed by MP, PP, and BP. Pre-pregnancy BMI, parity, use of basic skin care products, use of cosmetics, canned beverage, and type of milk consumption were associated with higher frequencies of paraben detection. In addition, type of milk, parity, and drinking status were significantly associated with the concentration of EP. Multiple regression analyses showed that colostrum and transitional milk samples had higher levels of EP than mature milk samples. The estimated daily intake of parabens in infants via breastfeeding appears to be negligible when compared to the acceptable daily intake values set forth by the European Food Safety Authority (EFSA); however, considering the vulnerability of breastfed infants and ubiquitous sources of exposure from daily use of household and personal toiletries, efforts to identify sources and mitigate exposure are warranted.

We investigated whether there might be excess ovarian cancer mortality among women residing near Spanish industries, according to different categories of industrial groups and toxic substances. An ecologic study was designed to examine ovarian cancer mortality at a municipal level (period 1997–2006). Population exposure to pollution was estimated by means of distance from town to facility. Using Poisson regression models, we assessed the relative risk of dying from ovarian cancer in zones around installations, and analyzed the effect of industrial groups and pollutant substances. Excess ovarian cancer mortality was detected in the vicinity of all sectors combined, and, principally, near refineries, fertilizers plants, glass production, paper production, food/beverage sector, waste treatment plants, pharmaceutical industry and ceramic. Insofar as substances were concerned, statistically significant associations were observed for installations releasing metals and polycyclic aromatic chemicals. These results support that residing near industries could be a risk factor for ovarian cancer mortality.

Epidemiological studies have demonstrated that the general population’s exposure to bisphenol A (BPA) substitutes is ubiquitous. Bisphenol F (BPF), one of the main BPA substitutes, is increasingly replacing BPA in plastics for food and beverage applications. Accumulating evidence suggests that BPA exposure is associated with nonalcoholic fatty liver disease (NAFLD)-like changes. However, the potential effects of BPF on lipid homeostasis remain poorly understood. In the present study, an epidemiological analysis with LC-MS-MS revealed that the BPF concentrations in the serum of NAFLD patients were significantly higher than those in a control group. Supporting this result, using Oil Red O, BODIPY 493/503, LipidTox Deep Red staining and gas chromatography-time-of-flight mass spectrometry (TOF-MS) assays, we found that BPF exposure induced NAFLD-like changes, with obvious lipid droplet deposition, triglyceride (TG) and fatty acids increase in mouse livers. Meanwhile, lipid droplet deposition and TG increase induced by BPF were also observed in HepG2 cells, accompanied by autophagic flux blockade, including autophagosome accumulation and the decreased degradation of SQSTM1/p62. Using adenoviruses dual-reporter plasmid RFP-GFP-LC3, RFP-GFP-PLIN2 transfection, AO staining, and EGFR degradation assays, we demonstrated that BPF treatment impaired lysosomal degradative capacity, since BPF treatment obviously impaired lysosomal acidification, manifested as decreased lysosomal hydrolase cathepsin L (CTSL) and mature cathepsin D (CTSD) in HepG2 and mouse liver issues. Additionally, v-ATPase D, a multi-subunit enzyme that mediates acidification of eukaryotic intracellular organelles, significantly decreased after BPF exposure in both the vitro and in vivo studies.This study ascertained a novel mechanism involving dysfunctional of lysosomal degradative capacity induced by BPF, which contributes to lipophagic disorders and causes lipid droplet deposition. This work provides evidence that lysosomes may be a target organelle where BPF exerts its potential toxicity; therefore, novel intervention strategies targeting lysosome are promising for BPF-induced NAFLD-like changes.

Plastic in the environment is considered an emerging pollutant of global concern. In spite of intensive research, many questions remain open, such as the processes that drive the deposition and remobilization of plastic debris on river beaches. The objectives of this study were: i) to analyze the influence of the natural hydrological fluctuations and wind intensity on the distribution of mesoplastic (0.5–2.5 cm) and macroplastic (>2.5 cm) debris in beach sediments of a large river, ii) to describe the type of plastic debris found and iii) to explore potential relations between the number of items and weight of macro- and mesoplastics. Our results suggest that, during lowering water levels, flow removes the plastic debris and transports it further downstream. Conversely, when the beach sediments remain exposed during long periods, the plastic debris accumulates considerably. Nevertheless, the influence of wind intensity on plastic debris transport was comparatively negligible. In other words, in our study the water flow had a greater capacity to remobilize and transport plastic debris than the wind. The most abundant mesoplastic items were foam, hard plastic, film and small fragments of fishing line. The dominant macroplastic items recorded were pieces of fishing line (nylon) and cigarette filters (cellulose acetate), typically discarded by beach users. Other items found in large quantities were soft packaging elements (expanded polystyrene), hard plastic containers (polystyrene, polyethylene terephthalate) and beverage bottles (polyethylene terephthalate), typical items of domestic use in the Paraná River region. Finally, we found that the density of macroplastic items is highly correlated to the density of mesoplastic items, serving as surrogate for further estimations. Our results could help to develop better mitigation strategies in seasonal riverscapes, based on the influence of the hydrological cycle and the characteristics of the most abundant meso- and macroplastics.

Gastric cancer is the fifth most frequent tumor worldwide. In Spain, it presents a large geographic variability in incidence, suggesting a possible role of environmental factors in its etiology. Therefore, epidemiologic research focused on environmental exposures is necessary.To assess the association between risk of gastric cancer (by histological type and tumor site) and residential proximity to industrial installations, according to categories of industrial groups and specific pollutants released, in the context of a population-based multicase-control study of incident cancer conducted in Spain (MCC-Spain).In this study, 2664 controls and 137 gastric cancer cases from 9 provinces, frequency matched by province of residence, age, and sex were included. Distances from the individuals’ residences to the 106 industries located in the study areas were computed. Logistic regression was used to estimate odds ratios (ORs) and 95% confidence intervals (95%CIs) for categories of distance (from 1 km to 3 km) to industries, adjusting for matching variables and potential confounders.Overall, no excess risk of gastric cancer was observed in people living close to the industrial installations, with ORs ranging from 0.73 (at ≤2.5 km) to 0.93 (at ≤1.5 km). However, by industrial sector, excess risks (OR; 95%CI) were found near organic chemical industry (3.51; 1.42–8.69 at ≤2 km), inorganic chemical industry (3.33; 1.12–9.85 at ≤2 km), food/beverage sector (2.48; 1.12–5.50 at ≤2 km), and surface treatment using organic solvents (3.59; 1.40–9.22 at ≤3 km). By specific pollutant, a statistically significant excess risk (OR; 95%CI) was found near (≤3 km) industries releasing nonylphenol (6.43; 2.30–17.97) and antimony (4.82; 1.94–12.01).The results suggest no association between risk of gastric cancer and living in the proximity to the industrial facilities as a whole. However, a few associations were detected near some industrial sectors and installations releasing specific pollutants.

The aim of this study was to determine the amount, composition and origin of plastic debris in one of the world largest river, the Paraná River in Argentina (South America), focusing on the impact of urban rivers, relationships among macro, meso and microplastic, socio-political issues and microplastic ingestion by fish.We recorded a huge concentration of macroplastic debris of domestic origin (up to 5.05 macroplastic items per m2) dominated largely by bags (mainly high- and low-density polyethylene), foodwrapper (polypropylene and polystyrene), foam plastics (expanded polystyrene) and beverage bottles (polyethylene terephthalate), particularly downstream from the confluence with an urban stream. This suggests inadequate waste collection, processing and final disposal in the region, which is regrettably recurrent in many cities of the Global South and Argentina in particular.We found an average of 4654 microplastic fragments m−2 in shoreline sediments of the river, ranging from 131 to 12687 microplastics m−2. In contrast to other studies from industrialized countries from Europe and North America, secondary microplastics (resulting from comminution of larger particles) were more abundant than primary ones (microbeads to cosmetics or pellets to the industry). This could be explained by differences in consumer habits and industrialization level between societies and economies.Microplastic particles (mostly fibres) were recorded in the digestive tract of 100% of the studied Prochilodus lineatus (commercial species).Contrary to recently published statements by other researchers, our results suggest neither macroplastic nor mesoplastics would serve as surrogate for microplastic items in pollution surveys, suggesting the need to consider all three size categories.The massive plastic pollution found in the Paraná River is caused by an inadequate waste management. New actions are required to properly manage waste from its inception to its final disposal.

Breast cancer is the most frequent tumor in women worldwide, although well-established risk factors account for 53%–55% of cases. Therefore, other risk factors, including environmental exposures, may explain the remaining variation. Our objective was to assess the relationship between risk of breast cancer and residential proximity to industries, according to categories of industrial groups and specific pollutants released, in the context of a population-based multicase-control study of incident cancer carried out in Spain (MCC-Spain). Using the current residence of cases and controls, this study was restricted to small administrative divisions, including both breast cancer cases (452) and controls (1511) in the 10 geographical areas recruiting breast cancer cases. Distances were calculated from the respective woman's residences to the 116 industries located in the study area. We used logistic regression to estimate odds ratios (ORs) and 95% confidence intervals (95%CIs) for categories of distance (between 1 km and 3 km) to industrial plants, adjusting for matching variables and other confounders. Excess risk (OR; 95%CI) of breast cancer was found near industries overall (1.30; 1.00–1.69 at 3 km), particularly organic chemical industry (2.12; 1.20–3.76 at 2.5 km), food/beverage sector (1.87; 1.26–2.78 at 3 km), ceramic (4.71; 1.62–13.66 at 1.5 km), surface treatment with organic solvents (2.00; 1.23–3.24 at 3 km), and surface treatment of plastic and metals (1.51; 1.06–2.14 at 3 km). By pollutants, the excess risk (OR; 95%CI) was detected near industries releasing pesticides (2.09; 1.14–3.82 at 2 km), and dichloromethane (2.09; 1.28–3.40 at 3 km). Our results suggest a possible increased risk of breast cancer in women living near specific industrial plants and support the need for more detailed exposure assessment of certain agents released by these plants.

Marine litter is a major global challenge, even in the remote reaches of the Arctic. Monitoring temporal trends in litter loadings and composition is key to designing effective preventative and mitigative measures, and to assess their impact. Few data are available, however, by which to do this in the Arctic region. Citizen science data organised by the local waste management company in the Lofoten archipelago in the Norwegian Sea is an exception to this. We analysed volunteer cleanup data (total weight and counts of select litter types, standardised to density per 100 m) from over 200 locations from 2011 to 2018. Results indicate a general decline in beach litter in the region, and particularly in litter types related to private use, such as beverage bottles. These declines are most likely the combined result of extensive cleanup activities and a considerable reduction in local litter inputs.

Possible access routes of macro and mesoplastics, acting as the main sources of pollution in the coastal ecosystem, are examined as a case study on four beaches along the Istanbul coast. A total number of 3787 items belonging to 12 categories of debris types were collected as follows: macroplastics 47.8%; mesoplastics 9.2%; and others 43.0% such as paper pieces, glass pieces and metal beverage cans. Clean-coast and carbonyl indexes were also used to identify the level of cleanliness of the sampling sites. The clean-coast index revealed that all of the sample sites were categorized as extremely dirty. Moreover, carbonyl index results indicate that 35.7% of the samples were at a high oxidation level indicating that some of the particles relatively spend more time on beaches. The litter concentration and surface oxidation results were addressing issues that should be taken into consideration to improve litter management strategies of the beaches.
