A brief authoritative commentary addresses the role of medium-chain triglycerides (MCT) in lipid metabolism and in the nutritional management of various diseases and disorders (e.g., massive bowel resection, hyperchylomicronemia, steatorrhea, celiac disease, cystic fibrosis). Cautions and counterindications of MCT therapy are discussed and information on the availability of MCT oil and its therapeutic use is presented. Since pure MCT contaims no essential fatty acids it should not be given as the sole source of fat; MCT is ketogenic and large amounts can cause gastrointestinal disorders. (wz)
